Discussion:
The New Regulatory Model for APRNs
Review content in the Hamric textbook that refers to the proposed new Regulatory Model for APRNs. Go to the Internet to review the full text of the Consensus Model. Review regulations imposed by your state board of nursing. How will the new model improve practice parameters and patient safety in your state?
R/ APRN is used to describe advanced practice nursing in a regulatory context. As times change and the need for certain healthcare services are altered, education, practice, and regulations of advanced practice nursing must also change. In order for these regulations to be understood by the profession and the patients, a clear and standardized language must be used across all forums. Some states have different credentialing methods and do not recognize some APN roles for title protection. APRN credentialing is composed of: licensure, accreditation, certification, and education. It is important for new APRN graduates to follow the regulations for their state when beginning to practice as an advance practice nurse (Hamric, Hanson, Tracy, & O’Grady, 2014).
“The Consensus Model provides guidance for states to adopt uniformity in the regulation of APRN roles” (NCSBN). The target completion date for the implementation of the Consensus Model was 2015 (the model itself was completed in 2008); many states have adopted sections but there still exists variation from state to state. This model is supported and maintained by more than forty nursing organizations. The Consensus Model is supposed to increase job satisfaction and promote advance practice nurse independence. Each of the four APRN roles are defined within the Consensus Model: certified registered nurse anesthetist (CRNA), certified nurse-midwife (CNM), clinical nurse specialist (CNS), and certified nurse practitioner (CNP). The American Nurses Credentialing Center (ANCC) has initiated plans to change certification processes in order for them to reflect the model’s regulations. For a smooth transition with the implemented model, look to the ANCC for current licensure guidelines. It is important to keep APRN certification up-to-date, especially with the transition of the Consensus Model (ANCC, 2017).
The state Board of Nursing is a governing body specific to each state that enforces the Nurse Practice Act and nurse licensure, accredits universities, and develops practice standards and policies. According to the Consensus Model implementation status, Virginia is at 14-20 points, which equates to about 50-71%. In Virginia, the goal of the Consensus Model is to allow more independence of APRNs. Changes are prone to occur with titling and licensure (Douglas, 2012). I believe the new Consensus Model, once fully implemented in Virginia, will result in positive change. It will give APRNs more autonomy and respect within their communities. Healthcare practices and policies will be better regulated, which will increase patient safety.
The Consensus Model defines each category of advance practice nursing and makes licensure, accreditation, certification, and education a streamlined process. The purpose is to increase the stamina of APRNs. Even though there might still be differences from state to state based upon populations and care that is needed, this model is also lessoning the gap of those differences.
