Pain Assessment

Complete a THOUROUGH pain assessment including PQRST on a classmate, family member or friend, write it up and submit.

P-Provoked ( what causes pain? what makes it better or worse?)

Q-Quality (what does it feel like? dull?sharp?stabbing?burning?)

R-Region/Radiation (where is the pain?is it always only in that spot or does it spread?)

S-Severity (what is the intensity of the pain? use a pain scale for this 0-10)

T-Timing (when did it start? how often does it occur ?how long does it last or is it continuous?
