The process of providing a partial health
assessment on your selected population/group using the Population Health
Promotion Model was started. Some useful references were found relating to
seniors.    The population/group assessed
was unclear- see comments in paper. The purpose of this assignment is to apply
the concepts of this course and provide a partial assessment of a
population/group, identifying their strengths and their needs using 3
determinants of health from the Population Health Promotion Model that results
in a community analysis that logically leads to the community nursing
diagnosis. What you are looking for is an understanding of the population
focusing on the commonalities of the group. 
How are the selected population/group taking care of themselves? When
specifying the determinants consider their current health status and how they
deal with stressors? What are their strengths? What are their values and
beliefs that are common as a group? What factors lead to a good health status?
(diet, exercise, etc.) What issues does this population/group see as being the
most important to them?  These
considerations provide you with a broad knowledge of the population/group.
Using literature and statistics to support your findings is an essential part
of this assignment.

 

Then you will be able to analyze and
identify common themes or issues to develop the community nursing diagnosis.
This paper reveals an unsatisfactory understanding of the topic.

 

Present the one priority community
diagnoses in a short concise manner that includes the 4 parts as indicated by
Vollman, Anderson, and McFarlane (Canadian Community as Partner text (4th ed.,
page 256-258).  (Issue description-Risk,
concern, state includes potential/actual; Focus-Boundaries of the population segment
of interest; Etiology-Causal factors; Manifestations-Data that supports the
etiological inference-Signs and symptoms) that gives direction for the next
step-planning an implementation program. For the final assignment use the
following template as a guide to apply so you have all the components of the
community diagnosis:
