Nursing discharge project ischemic stroke power point

Objective: Learners will develop and clearly communicate a comprehensive discharge plan of care that demonstrates critical thinking and accurately links community and medical surgical theory to practice.

Guidelines:

1. Each students will work alone to complete this project. Students will select a client that he/she cared for during the clinical rotation and who, at the time of care, was experiencing a complex alteration in health.

2. The discharge planning project will be presented in the form of a PowerPoint presentation. Dates of presentations will be determined by the clinical instructor.

3. Citations should be provided for information that is not the student’s own – i.e. pathophysiology, stats, etc. Personal communications should also be cited. Please follow APA for citations.

4. A reference list for citations should be included at the end of the PowerPoint presentation and should be submitted as a hard copy word document to your clinical instructor.

5. One PowerPoint slide should address each element that is bulleted with a blank box – i.e.

6. Submit the PowerPoint presentation in the electronic dropbox on the assigned date.

7. Submit one black and white “hard” copy of your PowerPoint presentation and a word processed reference list to the clinical faculty member evaluating your presentation on the day that you present.

8. Your presentation will be scored using the Discharge Project Presentation Scoring Rubric and the points earned will be entered into the course gradebook.

9. Students are encouraged to read and follow rubric in developing their projects and to note point deductions for critical errors noted at the end of the rubric.

Discharge Planning Project: Scoring Rubric

Elements:

· Provide a clinical picture of your patient using the SHARE form report as a guide. (SHARE form located in course resources within D2L.)

· Develop concept map connecting primary problem and secondary problems.

· Explain the relationships depicted on the concept map – i.e. DM contributed to atherosclerosis which lead to peripheral arterial disease affecting the feet.

· Using the concept map again, indicate which pathophysiological process you will focus on in your presentation.

Using the example of DM and PAD above, I may want to limit my discussion to PAD. Note: Slide need not be duplicated.

· Describe the physiological changes that occur in relation to the pathophysiological focus of your presentation– i.e. discuss cellular changes that occur with peripheral arterial disease (PAD).

· Identify key statistical information concerning primary disease/condition, include geographic and/or cultural variables – i.e. provide statistics on PAD, perhaps statistics on associated limb loss, and specific cultures most affected.

· Discuss key diagnostics and lab studies used to support or confirm primary disease or condition.

Elements:

· Medications prescribed for patient (not simply meds you administered) with an emphasis on meds used to treat primary condition/disease – i.e. for pt with PAD, perhaps emphasize Trental, warfarin, simvastatin. This section should include dose, frequency, and drug classification.

· Treatments prescribed for patient with an emphasis on treatments used to treat primary condition/disease – i.e. bypass and amputation

· Identify any patient variables – culture, finances, etc that influenced medication and treatment choices.

Elements:

· Identify and discuss inpatient sources of support and the respective services that can be used to prepare this client for discharge.

· Identify and discuss client’s financial sources of support – i.e. Medicare and physical therapy post-discharge

· Identify and discuss family/friend sources of support following discharge – i.e. Husband is in good health and is supportive and attentive to client, able to provide personal cares and assist with ADLs.

· Identify and discuss realistic community sources of support for client post-discharge – i.e. Physical therapy will come to home two times per week, completely paid by 3rd party payer.

· Identify and discuss specific needs, if any, specific to nursing following patient’s discharge. If none, you must indicate this and say why.

Elements:

· Present a comprehensive overview of teaching needs prior to discharge that shows an understanding of patient’s condition and situation.

· Develop and present a one [1] page patient teaching guide (PTG) related to the pathophysiological focus of your presentation. While this teaching guide should be something that you could easily transfer to a single page word document or folded pamphlet, you may in this instance include it in your PowerPoint as a slide

· Teaching guide should be aesthetically pleasing.

· Teaching guide should have words that do not exceed a 6th grade reading level.

· Limits PTG to a particular area of focus. (Do not attempt to cover too much content.)

· Includes source citations to support the PTG

Elements:

· Describe client’s potential for complete recovery and/or anticipated deficits and long-term prognosis.

· Describe what lessons you have learned in developing and presenting this project.

· Provide a reference page for citations; include this as a PowerPoint page and as a hard copy to be given to the instructor.

Elements:

· Presents project in an aesthetically pleasing manner using PowerPoint.

· Includes images, good use of white space, sound, and font colors that show well with selected PowerPoint backgrounds.

· May add other visual aids to enhance presentation.

Elements:

· Organization: Follows project guidelines and rubric with information presented in the appropriate section. Uses slide titles and bullets to facilitate organization of slides.

· Slide structure: Uses correct punctuation, grammar, and spelling in all presentation materials – PowerPoint presentation, any handouts, teaching guides, and reference pages. Adheres to APA writing style in reference list and citations included in PowerPoint and printed materials. Citations should be clearly visible on each slide as needed. Reference slide includes paper or online resources.

· Professional demeanor: Uses language and terminology commensurate with status in program. Presents self in professional manner. Shows sound knowledge of patient and is able to answer questions posed by fellow students and faculty.

Provides citation within slide and full APA reference on reference page. (Remember, your reference page is both a PowerPoint slide and hard copy sheet given to the instructors).

· 5 point deduction for each calendar day late.

· 1 to 5 point deduction if time of presentation less than 15 minutes or more than 20 minutes.

· 1 to 5 point deduction if selected patient for case presentation not preapproved by instructor.

· 1 to 5 point deduction if source citation not given for the patient teaching guide.

· 5 point deduction if any text portion of the teaching guide cut and pasted from a primary source.
