Improved medical care has resulted in an increased number of elderly populations. This population has the most significant risk for chronic diseases such as diabetes, hypertension, heart diseases, and asthma. The primary issue among nursing care of this population involves the provision of care and management of these chronically ill in various healthcare settings.The management issue has several implications in the practice of nursing. First, the implication involves applying information technology such as telehealth to enhance and increase care delivery to the patients in different settings. Model of transition care is one of the appropriate approaches to providing healthcare services to these patients with chronic illnesses (LeBerre et al., 2017). This approach is designed to ensure the continuation of patient care once they leave the hospital. Telehealth application, especially to the elderly population during care transition, enhances proper treatment and management of health complications of the patient.The second implication involves the need for the nurses to take a leading role in care coordination among different healthcare professionals involved in taking care of the chronically ill aged population in different healthcare settings. The transitional model of care for those aged with terminal and chronically ill patients requires a multidisciplinary approach involving nurses, nutritionists, various fields of medical specialists, and social workers (Scott et al., 2017).
