1-There are many areas in nursing where evidence-based practice has improved care processes and patient outcomes including the bundles of checklist to prevent pneumonia related to mechanical ventilation and central line-associated blood stream infection or CLABSI.

To prevent healthcare-associated infection such as the ventilator-associated pneumonia (VAP) for patient who had been intubated for more than 48 hours, the VAP bundles of care are often used. Recommended bundle of interventions for the prevention of VAP includes elevation of the head of the bed at a 30° to 45° angle to prevent aspiration of gastric content, reducing the duration of mechanical ventilation by daily sedation interruption (DSI) and daily spontaneous breathing trials (SBT), peptic ulcer disease (PUD) prophylaxis, and deep-vein thrombosis (DVT) prophylaxis. Moreover, performing oral care every two hours and as needed, maintaining ET tube cuff pressure above 20 cm H20, and managing ventilator circuits and changing only when visibly soiled showed improvements in VAP in critically ill patients.

Another area where evidence-based practice has improved patient outcome is through the use of bundles or interventions to prevent or decreased central line-associated blood stream infections. Those recommended bundle of interventions for the prevention of CLABSI includes proper hand hygiene, skin preparation using chlorhexidine solution greater than 0.5%, adhering to aseptic technique and use of sterile gloves, regular assessment and inspection of site, and daily evaluation whether the use of central line is still necessary.

According to the ANA (2018), when patient received the optimal intervention bundle, the expected time for any ventilator-associated pneumonia to occur takes almost 3.5 times longer that those who did not. At the hospital I work for, when we chart our lines/drains/airways one of the question we are asked is if the patient still meets the criteria for continuing the line. When no longer necessary, it prompts immediate consideration for removal. The bundles/checklist does not only act as a learning tool but also serves as a guideline. By knowing the bundles of interventions we can better care for our patients and avoid unnecessary and preventable infections.

2-In wound care it seems like new products are becoming available everyday and current products are continually evolving to improve patient outcomes. Thankfully, the clinic that I work in is smaller and there are only three nurses on staff, so our continuous education is typically done as a group. We usually meet with the product representative face to face and receive instant feedback regarding any questions and concerns we may have about the product. When information needs to be disseminated throughout the facility and to all nursing staff, the education happens is various forms. Least common is face to face in servicing as it is often difficult to get multiple staff members in the same location at the same time (due to staffing shortages). The most utilized form of education is online via an electronic learning forum (E-Learning) staff members can access the “assignments” 24 hours a day 7 days a week from most computers within the facility. Assignments are assigned with a due date and typically have a test following the educational portion in which the staff members must pass to receive credit and meet the educational requirement.

 

3-The implementation phase of any change can be difficult. In the nursing profession, evidence-based practices have been observed as the best and safest practices as there is proof in the research. However, there will always be those who question, or doubt said proof because some people are afraid of change, even if the change benefits them or others. To implement change, I would begin by addressing the following key factors: why, what, and how. First, the why- what is the issue that lead to the change. Next, the what- the research, the facts, the failures and successes that became the change. Lastly, the how- the demonstration or explanation of the change. Depending on the change and the aspects of our practice that would be affected determines the length of time the implementation phase would take. In my experience (when allowable) taking time to incorporate change over a period has proven least likely to create resistance from others.

Concerns have been raised recently that the strides the specialists are making in prenatal genetic testing are leading us down the path of eugenics. 

Eugenics, of course, is the theory and practice of human improvement by way of genetic manipulation to produce desired traits in a child. If history is any indication, there is cause for alarm where ethics issues and eugenics are concerned. 

Today we are talking specifically about a blood test used to screen for Down Syndrome during the first trimester pregnancy. The genetic test in question gives us the ability to identify Down syndrome in the womb without causing any harm to the fetus through the use of a simple blood test. Some agree that this information in advance means parents have more options in general, and an opportunity to raise a health child, while others argue that by promoting this test, we are stepping into a grey area where we are engineering human improvement based on a subjective evaluation of what is a “better” human being which is morally wrong. Besides that, because Down Syndrome has no cure, the “opportunity to raise a healthier child” implies aborting a child after a positive blood test.

     Abdominal Assessment

Performing a thorough assessment is an essential skill. The treatment for any client is based on the diagnosis, and that diagnosis relies entirely on the assessment. To assess a 78-year-old male with a chief complaint of abdominal pain, bloody diarrhea, fever, and weight loss; there are many important factors to take into consideration. First, it is crucial to assess more in-depth the chief complaint, known as a history of current illness. These will include an assessment of the abdominal pain following the pqrst. Also, an assessment of his bowel movement (diarrhea), when did it started, what causes it, what makes it subsides; did he took anything to stop it, did he had any changes in his eating habits, ask him to describe his bowel movements, and did any nausea or vomiting accompany it. Next, an assessment of the fever to find out when did start it, what was his highest temperature reading, what causes the fever to subside, was he exposed to any diseases like Clostridium difficile or food poisoning and is it accompanied by chills. Additionally, an assessment of his weight loss to see how much weight did he lost and in how long, was the weight loss intentionally, what type of diet does he follows, is there any cultural or belief factors that affect his diet.  

After the history of the current illness is obtained, the assessment continues into the risk factors. These will include a list of medications, and if the client is taking any NSAIDs or blood thinners; food, drugs, and environmental allergies; use of tobacco, alcohol and recreational drugs; history of vaccinations and traveling out of the country; past medical history (PMH), past surgical history (PSH), family history, previous hospitalizations, history of previous gastrointestinal (GI) problems like GERD, peptic ulcers, inflammatory bowel disease (IBD), anemia, and celiac disease, history of previous diagnostics GI tests and results like endoscopies and lab work. Also, assessment of living conditions, whom does he lives with, who prepares his meals, does he has access to drinking water and electricity. Even though certain diseases are not common in the US, the healthcare provider should never exclude them from the assessment. Assessing for parasitic diseases is a critical component for patients with GI symptoms and fever. Even though is more common in developing countries, it should not be ignored in the US. Commonly caused by ingestion of contaminated food or water, parasitic diseases are often misdiagnosed for tumors. The common symptoms are abdominal pain, diarrhea, bloody stool, fever, and weight loss (Çiftci, Bulut, Özdaş & Yıldırım. 2018). Amebomas are variable in sizes and on assessment are found as a palpable mass that causes obstructive symptoms and mimics many other diseases.

The next step is the physical examination. These start with the inspection, looking for the color, contour, size, shape,  pulsations, and lessions (Jensen, 2015). Then, the assessment continues with the auscultation of the bowel sounds over the four quadrants and the vascular sounds. The next step is percussion of the four abdominal quadrants assessing for the size of the organs and looking for tympanic sounds over the hollow areas and dullness over the organs, masses, fluids or obstructions (Jensen, 2015). The palpation is performed at the end, beginning with light palpation and assessing for pain, then follow by deep palpation to check for organs, masses, or tenderness (Jensen, 2015).

The documentation of the assessment findings is done using the SOAP format. All subjective data is documented under S, and all objective data is documented under O.  The assessment or diagnosis is written under A., And the plan of treatment is documented under P.
