Overview:

Your Practicum experience includes working in a clinical setting that will help you gain the knowledge and skills needed as an advanced practice nurse. In your practicum experience, you will develop a practicum plan that sets forth objectives to frame and guide your practicum experience.

As part of your Practicum Experience Plan, you will not only plan for your learning in your practicum experience but also work through various patient visits with focused notes as well as one (1) journal entry.

Part 2: Individualized Practicum Learning Objectives

Refer to the instructions in Week 2 to create individualized practicum learning objectives that meet the requirements for this course. These objectives should be aligned specifically to your Practicum experience. Your objectives should address your self-assessment of the skills found in the “PMHNP Clinical Skills Self-Assessment Form” you completed in Week 1.

As you develop your individualized practicum learning objective, be sure to write them using the SMART format. Use the resources found in Week 2 to guide your development. Once you review your resources, continue and complete the following. Note: Please make sure each of your objectives are connected to your self-assessment. Also, consider that you will need to demonstrate how you are advancing your knowledge in the clinical specialty.

Soap Note #1 Acute or Chronic Conditions

-Pick any “ADULT” Acute or Chronic Disease.  Must use the sample template for your soap note .

-Templates used from another classes will not be accepted. Student must use the template provided in this class which must clearly contain the progress note (in the Assessment section) of the encounter with the patient ( this section is clearly mark in bold, highlighted  and underlined).

– Follow the MRU Soap Note Rubric as a guide.

Your faculty member will provide a scenario for you to address in your paper. Choose an ending to the scenario, and construct your paper based on those reflections. Choose one of the following outcomes for the end of the scenario:

Below is the scenario; please do not use any other scenario for this assignment. Logistics and details, including scenario ending choices, regarding the paper can be found in the RUA: We Can But Dare We grading rubric found in the Unit 4 module.

Scenario: You are an emergency department nurse responding to a severe freeway accident. As a triage registered nurse determining the severity of each case and maintaining proper flow, it is your duty to review all patients impacted by this event. While reviewing patients coming in, you stumble across a familiar name, which happens to be the name of a famous actor, Governor Gru. Upon determining this, you happen to take a photo of the data and post about the finding on your social media account.
