Informatics in Professional Nursing

This week’s graded topics relate to the following Course Outcome (CO).

How do you use informatics (not technology) in your nursing practice? What ethical issues have arisen or might arise from use of informatics in professional nursing?

Professor’s post: Nursing has come a long way since the era of Florence Nightingale.  Everywhere we look we are surrounded by technology and informatics.  Every day we are learning new processes in the world of nursing informatics. Due to this we need to be prepared and ready for any changes to informatics in the future.  Massachusetts Department of Higher Education Nursing Initiative (2016) stated that the Nurse of the Future “uses informatics, and knowledge of the larger healthcare delivery system, to support and enhance patient care” (p. 27).  Along with this we need to make ourselves aware of the challenges we might face, such as ethical issues that may arise from the use of informatics.

Our healthcare needs an update on how we pay for our treatments. There are four ways to pay for insurance in the United States. The way we pay for our healthcare is Medicare, Medicaid, Private insurance, or out of pocket (Laureate, 2012). My parents are from the Baby Boomer generation and they are living longer than their parents. Insurance companies whether private or government inform doctors to push patients through healthcare as fast as possible to cut down costs (Laureate, 2012).  I have seen working in the emergency room this does not work. Patients are returning to the ER due to insurance not paying for them to stay longer. These patients are sicker on their second admit. This is doing a disservice to patients and causing our patients to think we are in it for money and not to help people.

As healthcare provider, we took an oath to do no harm to patients while they are in our care. I am not a fan of drug companies over pricing medications for the sole purpose to get rich. Walgreens is in a class action lawsuit and accused of overpricing generic medications to patients (Berman & Shapiro, 2017).  There should be a better way to regulate and prevent this if the accusation is true.  In the case of Provenge, medication for prostate cancer, these patients should have the choice to use it. If it was your own family member you would want as much time as possible to spend with them. Studies show that this medication will extend life by at least four months (Stein, 2010). There has to be a better way to deliver this medication in a cheaper way. 

Another option is to bypass our drug company in the United States all together. Canada’s online pharmacies were lower than our Medicare drug coverage (Sean, Young, Na-Eun, Andy, & Jongwha, 2017). I have had patients in the Emergency room who had prescription bottles from Canada and they stated it was the only way they could afford their prescriptions.

            My experience with using Grammarly and Safe Assign were not as daunting as I expected.  Grammarly is an app-based program that can be attached to Office 365 and Firefox.  The link for Grammarly was found on the academic guides page.  I used the site search box to find the app’s location. From there it was as easy as following directions and downloading the app.  I copy and pasted the paraphrased assignment and received an 83/100 for performance.  Other scores given were for word and sentence length, those scores were a 4.9 and a10.9.  The only issue found was the “Go Premium” offer that was $11.99 a month or $139.95a year.  There will not be a subscription ordered.  Grammarly will be used in my Word program on a trial basis. It can’t hurt to see what needs to be fixed in real time.  Another academic integrity tool is Safe Assign.  This tool is defined in the academic answers portion of the academic guide by stating, “SafeAssign is a tool that helps students prevent plagiarism and aids in identifying opportunities to add properly cited sources rather than just paraphrasing.” (Walden, 2011) Finding Safe Assign in Blackboard was very easy.  It is in the toolbar on the bottom left-hand side of the page.  Once on the site, it is as easy as browsing your computer for the document and simply upload it.   The only issue I have with Safe Assign is it is not in real time, you must wait for the site to review your work then make the changes needed.  In my opinion, Grammarly is a better tool for writing and having a real-time view of the paper being written.

A resource that I would recommend is the Online Writing Lab (OWL).  It is the base from where the American Psychological Association (APA) is explained in detail.  In the APA overview and workshop section of the OWL, it states, “Establish your credibility or ethos in the field by demonstrating an awareness of your audience and their needs as fellow researchers.” (Purdue Writing Lab, 2018) This is why we should use APA when scholarly writing.  Credibility is key when writing anything at a master’s level.  When using APA, we research ideas and information from scientific journals, and at the same time distribute that knowledge in the form of scholarly writings.  Using the OWL for any APA formatting question creates a solid base for building an educational publication to be reviewed by your peers. (APA, 2010, pg.9)

Grammar in its basic form decides how a writer will be received by the reader.  Incorrect grammar sidetracks the reader from the tone an author has set.  A format that I constantly work on is the use of parallel construction.  On page 84 of, the “Publication Manual of the American Psychological Association (6th ed.)” parallel construction is covered.  This is two ideas covered in a sentence that will improve the reader’s comprehension.  (APA, 2010, pg.84) Another format important to consider is the explanation of abbreviations.  Abbreviations in scientific writing are used to shorten long drawn out titles. The key in using abbreviations is to initially explain what the abbreviation is and use it at least four times.  There is a balance that must be attained.  Using too many abbreviations jumbles up the sentence and make it difficult to read, but not using enough causes the reader to lose interest with repetitive phrasing. During the analysis conducted with my mentor, we believe that from the clinical point of view the results obtained from the patients will be positively impacted through the implementation of the EBP, and the application of the knowledge acquired from the planned educational proposal. The professionals who participate in the educational project will be able to learn the importance of the application of EBP in the daily routine and this will result in better health care provided to patients. This increase in positive results is synonymous with quality, therefore the organization will benefit from providing their patients with a high-quality service which will make the standards of the same, stay above what is required. All the aforementioned, makes the financial cost of implementing the educational project is not noticed. In any case, the expenses are expected to be minimal, but the benefits that the plan will bring will make disappear the possible economic barrier that could represent the implementation of the proposal for the administration of the organization. Healthcare expenditures must be correlated with high quality and efficiency in the delivery of services to improve health outcomes. In a very good study carried out by Hussey, Wertheimer, and Mehrotra, 2013, they point out that “One possibility is that improvements in quality will require increases in cost (or conversely, cost reductions could reduce quality). In reality, the association between cost and quality probably falls between these 2 extremes, that some types of health care costs are associated with high quality and others with poor quality. The effect depends on where the money is spent”.

Evidence-based practice is associated with higher quality care and better patient outcomes than care that is steeped in tradition. However, the integration of evidence-based practice implementation into daily clinical practice remains inconsistent, and the chasm between research and bedside practice remains substantial. Practice changes most of the time requires an upfront investment of time and resources. These investments are designed to create an improvement in outcomes, whether clinical (e.g., reduced infections), organizational (e.g., shorter length of stay), or fiscal (e.g., reduced unplanned readmissions, which helps the organization avoid Medicare penalties). Any of these may have cost implications. Some EBP improvements may not create cost savings but may achieve an equally valuable outcome (e.g., patient satisfaction) that helps fulfill the organizational mission.

4-Evidence based practice research has changed the field of nursing in many ways. It is one of the key principles in medicine today adhering to standards for high quality and safety, all while focusing on patients’ needs. Old practices may not work and need to change, evidence-based practice provides the latest research in improving these practices.

In emergency nursing, evidence-based practice is used daily. One very common practice is infection control. “This includes keeping the healthcare environment clean, wearing personal protective clothing, using barrier precautions and practicing correct handwashing” (Arkansas State University, 2016). Even though the emergency is very fast paced, nurses in my facility are very aware of infection control and make efforts to improve their infection control practices daily. Another evidence-based practice used frequently in the ED, is oxygen use in patients with COPD. “Despite the belief by some that providing oxygen to these patients can create serious issues such as hypercarbia, acidosis or even death, the evidence-based protocol is to provide oxygen to COPD patients. This practice can help prevent hypoxia and organ failure. Giving oxygen, which is the correct treatment based on the evidence, can enhance COPD patients’ quality of life and help them live longer” (Arkansas State University, 2016). It is very important that nurses know that giving to much oxygen can increase their respiratory drive, causing more distress, but some oxygen is key to helping improve the patients’ status.

5-Evidence-based practice is something that is embedded into our minds. During my first quarter of nursing school we were required to write down interventions for all nursing diagnoses and were required to cite evidence as to why we do the nursing interventions that we do. As nurses it is important to know the WHY as to why we do what we do. Evidence-based practice changes everyday because science and studies are frequently changing.

As nurses, we practice numerous nursing interventions daily. For bedridden patients who cannot move on their own, we are required to turn them at least every 2 hours because research shows that by turning these patients, the chances of developing bed sores are greatly decreased. We had a patient on our unit for about 6 months who was not able to move on his own and he did not develop a single bed sore because frequent turning was practiced. Other common evidence-based practice interventions include scrubbing the IV and central line hubs with alcohol for 10-15 seconds to prevent CLABSI. Washing your hands with soap and water before and after entering a patient’s room is the number one method of preventing the transmission of disease. We practice evidence-based interventions numerous times a day and we probably do not even realize that we are doing it because it becomes second nature to us such as something as simple as raising the head of the bed, picking out the correct blood pressure cuff, or grabbing some ice to apply to our patient’s knee, etc.

Arkansas State University states that nurses have room for improvement when it comes to EBP. They state the follo wing are some areas where nurses could better adhere to EBP:

  • Communication      involving changes in a patient’s status.
  • Soft      skills that improve patient interaction.
  • Training      and onboarding new nurses.
  • Shift      scheduling and the effect on care.

6-From the perspective of a nurse, research or
evidence based practices are of vital importance when it comes to determining
the best course of action to take with clients, and determining which route to
take with an intervention plan. For instance, when examining an individual’s
symptoms or ailment, using evidence based practices can show the best steps to
take in order to provide the patient with the most in depth and intensive
treatment options which can be afforded to them (Melnyk & Fineout-Overholt,
2011).

           When
treating a patient with a number of symptoms which do not have an immediately
known cause, using evidence based practices can assist in determining which
specific ailment the patient is suffering from. By using research that
highlights similar symptoms the specific condition can be examined in more
detail which could lead to new treatment methods being devised (Koehn &
Lehman, 2008).

           The
benefits of evidence based practice are not limited only to the treatment of
patients, but can also be utilized in how to properly communicate and interact
with patients, and can prove vital in establishing connections with them as
well (Melnyk & Fineout-Overholt, 2011). For instance, when handling a
patient who is being difficult in their treatment methods, using the best
evidence based practices can give insight into ways to interact with the client
that can lead to more positive breakthroughs in their recovery (Koehn &
Lehman, 2008).

           Overall,
evidence and research based practices give nurses the ability to utilize a wide
range of resources to determine which course of action is the correct step to
take in ensuring that their patients receive the best cares possible (Koehn
& Lehman, 2008).

                               Scholarly Writing and Plagiarism

Learning is a journey and not a destination. A part of this journey was to use Grammarly and SafeAssign. I will discuss my experience with those two platforms as well as my perception of paraphrasing and plagiarism. The internet is full of resources, so I will be sharing an online resource that supports scholarly writing. Finally, I will share two American Psychological Association (APA) formatting rules that I believe are useful for academic writers.

 I have never used Grammarly before, but I like that I can check grammar in real time. I did not know such a tool was in existence and I will be using Grammarly for all my assignments going forward before sending. However, SafeAssign is not a new tool for me. When I went to Western Governors University, I used TurnItIn, which is similar to SafeAssign. It checks how much of your paper matches other known scholarly works. I wrote a sample paragraph with SafeAssign, and it was a 0% match. I like to see my works matching at 5% or less as a matter of philosophy. It is easy to avoid plagiarism if two things are remembered. Use thoughts that originate from your cranium and cite works according to APA rules and Walden University requirements. During this process, my insight was learning how to paraphrase better. Walden University (2012) states that paraphrasing is beyond simply rearranging a sentence and choosing synonyms. I realized paraphrasing is about turning the whole idea into your own words and then citing the author.

Scholarly writing is a term that can be confusing for many students. I had no idea what it meant until I researched the topic. According to Jackson State University (2015), scholarly writing is structured and seeks feedback from the audience. It would seem that this is the purpose of using discussion boards in an online learning environment. I believe that this online resource is useful, and I plan on adding it as a bookmark as a reminder of the essentials of scholarly writing.

As previously mentioned, proper APA style and formatting is essential in scholarly writing. A rule that I was not versed in was related to in-text citations with more than five authors. The APA Manual states, “when a work has six or more authors, cite only the surname of the first author followed by et al” (American Psychological Association, 2010, p. 175). Also, I did not know that the following rule existed. According to the American Psychological Association (2010, p. 170), presenting your existing work as new when it is not is the definition of self-plagiarism. I found this to be an interesting rule that I had not considered before and will make sure I never do.
