Central line associated blood stream infection (CLABSI) is a major contributor to in-hospital morbidity and mortality and is linked with increased expenditure and length of intensive care unit (ICU) stay. The incidence of CLABSI is more in the ICUs due to emergency catheter placement, longer duration and repeated manipulation for sampling, administration of drugs and fluids, the additional confounding factors being chronic illness, old age, sepsis and immunosuppression (Atilla et al., 2016). My proposed solution of addressing this problem is nursing and patient education. Among the health care workers, nurses have the most direct and continuous role in handling CVCs, being involved with both insertion assistance and maintenance of central lines. Thus, they have a unique opportunity to contribute towards preventing these infections.Bedside nurses have the responsibility to implement the right interventions to prevent them. Appropriate training and education in central line management can go a long way in preventing this problem. Nurses are in a unique position to prevent CLABSIs across the health care spectrum. It would not be an overstretch to say that CLABSI prevention is completely a nursing responsibility. Let us consider the current health care scenario: the nursing scope of practice has increased vastly over the past decade and our profession continues to gain significance (Atilla et al., 2016).Educating the patients and families on the best practices of central line care and infection prevention is the responsibility of nursing staff.

Making patients and caregivers partners in therapy by creating educational materials in simple language will help motivate adult learners to assimilate the knowledge (Patel et al., 2019). An interactive nurse-led demonstration accompanied by an illustrated guide to best practices of central line management ensure compliance to strict infection prevention practices. Again, this responsibility of educating patients falls on nurses, and patient education is a powerful tool to prevent CLABSIs (Patel et al., 2019). Nancy Halpert (N. H.) is a 75-year-old African American female who lives in a one-bedroom apartment in Washington D.C. She is widowed. She had a daughter who passed away at the age of 40 due to breast cancer. Her remaining two children live in different states, one in Ohio and the other in California. Her 25-year old granddaughter stays with her on the weekends when she is not working. Nancy’s children call her every week to see how she is doing. She has a neighbor who checks on her daily to see if she needs anything. Nancy visits the senior citizen center located one block from her apartment once per week.

 

N. H. was diagnosed as a Type 2 diabetes five years ago. Her last physical exam was two years ago. She doesn’t like to go to the doctor because she is afraid she will need insulin. She takes glyburide daily and “tries to watch what I eat.” Her blood glucose has been consistent with an average AM reading of 180 mg/dl. She eats fast food from nearby restaurants frequently because she doesn’t feel like cooking for just herself and she likes to have her meals delivered. She often says, “I like to get visitors. I enjoy the company.” She sleeps only 3-4 hours per night, and sleep is often interrupted. She reports having difficulty sleeping ever since she turned 70. She complains of feeling tired most days. She has frequent periods of restlessness at night and must urinate frequently. She does not have difficulty with hygiene activities but states, most days she doesn’t feel like grooming herself since she doesn’t go anywhere. N. H. is 35 pounds overweight and reports she would like to lose some weight but doesn’t know what to do.

 

For the past five days, N. H. has been feeling “dizzy and more tired than usual.” She has been unable to keep any food down and reports nausea, vomiting, and abdominal pain. She noticed a fruity odor to her breath. When she checked her glucose this morning, it was

690 mg/dl. Also, she has been experiencing headaches, frequent urination, and increased thirst. When her neighbor came to check on her, she noticed N. H. was confused and had trouble concentrating. Her neighbor bought her to the to the E.R. immediately. Her blood pressure was 135/80. Lungs were clear upon auscultation. Her face was flushed, and she was restless and having some difficulty breathing. Lab tests revealed a blood glucose level of 750 mg/dl and a hemoglobin A1c of 8%. N. H. was treated with intravenous fluids, electrolyte replacement and insulin. After she was stabilized, she was admitted for two days for observation, and then released.

 

On discharge, the medical diagnoses were as follows:

1. Uncontrolled diabetes mellitus

2. Diabetic ketoacidosis

3. Obesity

4. Possible depression

 

Her discharge orders were as follows:

1. Metformin 500 mg po bid

2. 2000 calories ADA diet

3. Diabetic teaching classes

4. Follow up with primary provider within two weeks for physical exam and depression screening

 

You are the nurse working in the primary care office. Since N. H.’s last physical exam was one year ago, a complete health history and physical exam is indicated at the next scheduled visit.

 

When N. H. arrives for her exam, she denies any headaches, nausea, vomiting, but still has frequent urination. She reports since being in the hospital she has tried to cook just for herself but has had to eat fast foods for at least one meal a day. On the day of her physical, N. H.’s weight is 235 pounds, height 5’2”, which calculates to a BMI of 43 indicating obesity. She is talkative during the assessment, answers questions politely and appropriately, and makes frequent eye contact. She is alert and oriented x 4, speech is clear and not pressured, and thoughts are logical.

 

As you begin taking her health history, N. H. admits she is “worried” and nervous about having to need insulin. She wants to change her diet so that she can live a healthier lifestyle. You explain to N. H. the role of insulin in her body and the benefits of adding an insulin sliding scale to the treatment plan to control her glucose and prevent the complications of hyperglycemia. You also teach her how to self-administer insulin and provide her with the opportunity to return the demonstration. You also include the manifestations of hypoglycemia and how to prevent it from occurring.

The definition of the traditional family is “is the parents and offspring living together as a societal or social unit with the same ancestry” (Green, 2018). However, the definition of family that encompasses the different family structures today would be “societal units of single-parent families; lesbian, bisexual, gay, transgender, queer, and questioning (LBGTQ+) families; children raised by grandparents or other relatives; multigenerational units; and people who are together out of friendship and love, not bloodlines. Pets, often part of a household, may acquire the status of “family” members. Thus, family tends to connote a complex, diverse, and dynamic unit, often consisting of parents and children related by legal, genetic, or emotional ties” (Green, 2018). For example, I know some people who are unable to have children of their own and consider their pets as their children. I am sure many people have even heard the term ‘dog-mom’ being used throughout society as well. The family systems theory states that “Family members influence each other and the family as a whole” (Green, 2018), furthermore, each “…individual does not exist in isolation, but is part of a group or family system. Each family member experiences the togetherness, attachment, and intimacy of the family system, but the family member remains an independent thinker with individual purpose-driven actions” (Green, 2018). Whether or not a patient’s family is considered traditional or nontraditional, it is important for nurses to understand the family systems theory when providing and planning care because a patient’s family truly impacts their environment.

I think the definition of family is one that is likely to change per the patient and their individual take on the definition. What a family to me could be and is probably very different than what a family is to one of my patients. Family is either by choice, so the person or people have chosen this as their family and it could range from a best friend to a significant other or a family could be blood, the “biological” part of what makes people “family” and lastly, it could be a mixture of both choice and “biologic” that makes a family. It is important to acknowledge a non-traditional family because like I said before the people in the “family” does not matter but how they interact and the dynamic in which they live is what is more important than the who of the family. “A family systems approach to individuals and their families provides a means to better understand the interactions of a societal family unit ”  and family systems theories help us to understand what we, as nurses, need to look for when assessing and evaulating a family and their health.
