“Nursing research is a systemic process of inquiry that uses rigorous guidelines to produce unbiased, trustworthy answers to questions about nursing practice” ( Houser, 2018, pg. 5). Nurses can improve quality of life for patients by finding solutions to problems by implementing Evidence based practice and nursing research. Nursing practices that are based on research uses a common format called the PICOT method. According to Riva, Malik, Burnie, Endicott, and Busse, ( 2012), ” Clinicians interested in research pursuits, related to patient care, should consider the use of a literature research and the PICOT format when engaging clinical researchers.” This method allows those researching the direction needed to answer questions of most importance. Upon reading case studies from our reading by Kumar, ( 2015), I chose the middle aged active woman with type 2 diabetes.

PICOT Question:

Can the use of Degludec insulin and dietary modification control blood sugar and reduce hypoglycemic episodes better than the use of another type of insulin with dietary modification in middle aged active females with type 2 Diabetes?

P:  Active middle aged women with type 2 Diabetes

I:   Use of Degludec insulin and dietary modification

C:  Use of insulin pump and dietary modification

O:  Better glycemic control and decreased hypoglycemic episodes

T:   2 months

The focus point of this question would be treatment related. This case study proved that through the use of Degludec insulin therapy, and lifestyle changes, the desired outcome were proved in this 2 month trial, thus allowing this patient to maintain as normal a lifestyle as possible with this treatment. Though the insulin pump worked well for her, it did not mesh well with her current activities.

Mrs. G. is a 75 year old Hispanic woman who has been relatively well all of her life. She had been married for 50 years and had five children. Her children are grown with families of their own. All but one of her children live in other states. Mrs. G.’s husband passed away last year, which was devastating for her. She had been very close to him and relied upon him for everything. He was “the life of the party” she always said and was a loving and caring man. Since his passing, Mrs. G. has continued to live in the house they shared for 35 years. In the last month, Mrs. G. has fallen twice sustaining injuries, though minimal. Her home health nurse comes weekly to check in on her. Mrs. G. likes her very much and wishes she could come more often. Mrs. G.’s daughter who lives in the next town over, has been worried and decided with the urging of her siblings and the doctor to start looking for an assisted living facility for her mother. She found one last week and talked with the Director who said she would be happy to help in whatever way was best. The daughter decided to tell her mother that it was time for her to move, so she can be cared for and be safe. When she told her mother, Mrs. G. cried and said, “This will not happen ever. I plan to stay in this house of loving memories for the remainder of my life.”

In 3 – 4 pages answer the following questions:

The United States health care system is lacking in many areas when it is compared to other countries. There are many areas that we can improve where health care is concerned. Even though our health system is not the best our cost for health care continues to rise. Where is our money going? A survey from the Harvard School of Public Health revealed that 55% of American adults barely gave the US health care a passing grade, more than 11% gave the quality of care a fair rating, and 47% felt that the hospital care failed to provide the care that they deserved. Reading this was awful to say the least. How can we fall behind in quality of care in the hospital setting when our goal is to have the best outcome for our patients? Many studies show how there are multiple errors due to lack of communication and misuse of equipment that leads to accidents and even death of the patient. Health care providers need to talk to each other and respect one another and always remember that the patient should be their priority. The way we can achieve this is by holding everyone accountable for the care that’s being provided. Reporting poor quality of care will also help open the eyes of those who choose to work blindly.

In regards to the high price of health care The Affordable Care Act was established to help. It gives people that cannot afford insurance or are not offered insurance at their jobs a discounted rate. 20 million Americans have received coverage under this act that was initiated May 1, 2014, (The commonwealth fund, 2016)). Many individuals work hard to earn enough money to support their families and provide food and clothes for them. They don’t have enough money to spare on health insurance just in case. Health care insurance is not cheap to buy even purchasing a HMO costs an average family $700-$900 a month, that’s just the monthly fee and does not take into consideration the copays and cost of medication that goes along with treatments. We have the biggest and best that the world has to offer when it comes to science and medicine we should be able to provide care to all not just a select few that can afford it.
