Research methods such as qualitative and quantitative are the tools that researchers use while they investigate their topics or study (Almalki, 2016). According to Walliman (2011) it is the researcher’s responsibility to pick the appropriate tool for their study. There are three methods that connects research information-qualitative, quantitative, and mixed method.

 

There are advantages and disadvantages to using both qualitative and quantitative studies together. The advantages are it gives the study more depth, validity, and understanding. This also allows researchers to compensate for unexpected methods weaknesses and strengths and offset biases (Greene, 2007). However, combining the two methods into a mixed method can be very time consuming for the researcher and participates. Secondly, deciding on the mixed method that would work best for their study can be confusing. These mixed methods could be triangulation design, embedded design, explanatory design and explanation design. Even though it can be daunting, mixed method research can provide positive benefits and allow and offer the researcher an opportunity for a more realistic link between methods and a stronger understanding of the study being investigated (Almalki, 2016).

Extraneous Variables are undesirable variables that influence the relationship between the variables that an experimenter is examining. Another way to think of this, is that these are variables the influence the outcome of an experiment, though they are not the variables that are actually of interest. These variables are undesirable because they add error to an experiment. A major goal in research design is to decrease or control the influence of extraneous variables as much as possible (Hall, 2011). Extraneous variables that vary with the levels of the independent variable are the most dangerous type in terms of challenging the validity of experimental results. These types of extraneous variables have a special name, confounding variables. Any experiment that involves a comparison of men and women is inherently plagued with confounding variables, the most commonly cited of which is that the social environment for males and females is very different.

An example of extraneous variable is if a study focused on the effect of relaxation therapy on the perception of incisional pain, the researchers would have to control the extraneous variables, such as type of surgical incision and time, amount, and type of pain medication administered after surgery, to prevent their influence on the patient’s perception of pain. Selecting only patients with abdominal incisions who are hospitalized and receiving only one type of pain medication intravenously after surgery would control some of these extraneous variables. Controlling extraneous variables enables researchers to determine the effects of an intervention or treatment on study outcomes more accurately

Formulating a question that targets the goal of your research is a challenging but essential task. The question plays a crucial role in all other aspects of the research, including the determination of the research design and theoretical perspective to be applied, which data will be collected, and which tools will be used for analysis. It is therefore essential to take the time to ensure that the research question addresses what you actually want to study. Doing so will increase your likelihood of obtaining meaningful results.

In this first component of the Course Project, you formulate questions to address a particular nursing issue or problem. You use the PICOT model—patient/population, intervention/issue, comparison, and outcome—outlined in the Learning Resources to design your questions.

To prepare:

    • Review the article, “Formulating the Evidence Based Practice Question: A Review of the Frameworks,” found in the Learning Resources for this week. Focus on the PICOT model for guiding the development of research questions.
    • Review the section beginning on page 71 of the course text, titled, “Developing and Refining Research Problems” in the course text, which focuses on analyzing the feasibility of a research problem.
    • Reflect on an issue or problem that you have noticed in your nursing practice. Consider the significance of this issue or problem.
    • Generate at least five questions that relate to the issue which you have identified. Use the criteria in your course text to select one question that would be most appropriate in terms of significance, feasibility, and interest. Be prepared to explain your rationale.
    • Formulate a preliminary PICO question—one that is answerable—based on your analysis. What are the PICO variables (patient/population, intervention/issue, comparison, and outcome) for this question?

Note: Not all of these variables may be appropriate to every question. Be sure to analyze which are and are not relevant to your specific question.

  • Using the PICOT variables that you determined for your question, develop a list of at least 10 keywords that could be used when conducting a literature search to investigate current research pertaining to the question.

Spiritual care is part of the art of nursing and professional care. Spiritual care is defined by the literature as recognizing, respecting, and meeting patients’ spiritual needs; facilitating participation in religious rituals; communicating through listening and talking with clients; being with the patient by caring, supporting, and showing empathy; promoting a sense of well-being by helping them to find meaning and purpose in their illness and overall life; and referring them to other professionals, including chaplains/pastors (Baldcchino, 2015). My strengths are that I am genuinely not a judgmental person and I am interested in learning about different cultures and beliefs. I feel that I can learn just as much from my patients as they can learn from me. I am also good at building trust with my patients. My charge nurse always jokes when a patient is being “difficult” by saying “give the crazy to Eve.” I usually do end up with the difficult patients but I often find that they were feeling disrespected or neglected by the staff prior to my shift. I think that my weakness is that I am to emotionally invested with my patients. I have a hard time with not providing words or services that I feel will help the patient even when it is against policy. For example, we are not allowed to hug our patients but I have been hugged by a patient and did not feel right about not returning the gesture.

If I were a patient and had difficult decisions to be made, my sister would make the decisions. Both my husband and my sister agree that my husband would not be able to carry out my wishes. I love my husband to death but he is not capable of making the decision to take me off life support if necessary, especially because he doesn’t believe that anyone should be taken off because of stories he has read about people waking up after years and being perfectly fine. My sister is very matter of fact and I know that she will do exactly what I want even if it’s hard.

SCENARIO OPTIONS. Pick one of the scenarios below. Your role is the manager in charge of an ambulatory surgery center (ASC). Restate the problem you are addressing. Make some logical assumptions. Make brief recommendations as to a reasonable solution to the problem in the scenario.

2)     Another challenge facing ambulatory surgery centers is the recruitment of new physicians. Following a national trend, many local hospitals and health systems are purchasing physician practices, which could eventually lead to doctors referring surgical cases to a hospital’s same day surgery department. Assuming there is ample opportunity to increase surgical volume, how do you intend to recruit physicians to practice via your ASC? What are some strategies/offerings you could employ?
3)     The ASC you are currently managing is experiencing out of network challenges. Additionally, the proposed reimbursement rates from payers do not match Medicare rates for your market. Most of your ASC contracts are based on the Medicare fee schedule and because many of your surgeries are not on that schedule, you have to negotiate a fee schedule on a case-by-case basis. What strategies could you employ as you work with payors to contract for reasonable rates that could save you both money in the short term?
4)     In this changing environment, you are required to do more with less. Reimbursement rates are declining; supply and implant costs, taxes, utilities, rent, salaries and benefits and other operating costs are increasing and there are more regulatory requirements i.e., increased standards for accreditation (Joint Commission’s additional requirements as an example), ICD-10, adoption of an EHR, etc. Additionally, your staff is wearing multiple hats; many working to register, bill, code, schedule and assist with back house operations. What strategies and tools can you implement to increase productivity, reallocate resources and boost employee morale?
