I am currently treating a child that is immobile.  I provide care to her in multiple ways, but one specific way is ulcer prevention.  When I first started treating her, she had a stage 1 ulcer.  Treatment included wound care with antiseptics.  When I treated her, I was given a prescription and I investigated what the benefits of this ointment was on ulcers. I always like to inform myself and make sure the doctor has prescribed what is best and most effective for my patients.  An example of implementing EBD and its effects on ulcers, would be the article Antibiotics and antiseptics for pressure ulcers by Christie, Dumville, Goto, Tanner, Moore, and Norman. When applying EBP practice, I can consider whether to use antiseptics and/or antimicrobial treatment. In doing so, I research the effects and outcome on both types of treatments.  Per Christie, Dumville, Goto, Tanner, Moore, and Norman (2016) when comparing both of ointments on ulcers, “There was no consistent evidence of a benefit to using any particular antimicrobial treatment for pressure ulcers. However, there was some limited evidence that more ulcers healed when treated with some types of alternative dressings without antimicrobial properties than when treated with povidone iodine”. This research can be used by myself and nurses to treat ulcers, avoid unnecessary procedures, be a voice to our patients and physicians, and explore other possibilities of treatments of ulcers.

According to Youngblut and Brooten (2001) “Evidence-based practice (EBD) provides opportunities for nursing care to be more individualized, more effective, streamlined, and dynamic, and to maximize effects of clinical judgment.” Evidence based practices are the latest advances and knowledge developments, not supporting existing practices. In addition, it includes nurses being active and taking part in the clinical data and application of evidence.  In my opinion, applying EBD in practice will increase the value and reputation of nurses.  One way nurses can promote evidence based research is to be an “opinion leader” (Flodgren , Parmelli , Doumit , Gattellari , O’Brien , Grimshaw , Eccles 2011).

Rochelle, you have a great post. Mrs. Sloan is a sound mind and cognitively intact. She has the right to refuse and right to choose her treatment. She is a competent adult that can make decisions for herself and determine what shall be done with own body. As an advance practical nurse, we should have an excellent communication skills necessary to engender trust and draw patients into a partnership of care (Flaherty & Resnick, 2014). We have to ensure that Mrs. Sloan has a clear understanding about the advance directives and know the pros and cons of her decisions. Although all patients who require CPR have acute illness by virtue of needing CPR, there may be differences in the type or severity of the acute illness leading to CPR that could influence outcomes. I will emphasize to her the benefits or pros and cons that she might get from CPR. There are some patients that get terrified when we talk about advance directives or end-of-life care, so we have to make sure that we explained it right and not sound to be morbid. The value of the APN role extends beyond specialized and expanded knowledge, critical thinking, and evidence-based practice to enhanced communication skills that facilitate direct patient care through informed decision-making, patient and family education, and psychosocial-spiritual care (Yeager, 2014). Lisa, you have a very informative post. I certainly agree with you to encourage Ms. Strearn to elaborate on what other needs she would like to have fulfilled and who she would like to involve in the decision making. In case study #2, Ms. Stearn was already 81 years old and disoriented at times. She even performed poorly on a standardized mental state exam. Even though, she appropriately answered questions, her cognitive status is mildly cognitively impaired. As her nurse practitioner, we should establish a good patient-relationship so that she will be more comfortable in expressing her concerns why she is refusing bladder catheterization. We should have the ability to listen which is the most valuable skill that can be used and allow her the opportunity to tell her story. We have to have open communication and show to her that we are willing to take care of her and address her concerns (Knox, 2010). We should show empathy and be with the patient so she will feel her importance as a person and a human being. We should also explain well to the patient the complications that she might get if she refuse the treatments. Also, we have to find out the reason why she is refusing bladder catheterization, it may be because she was very conservative and ashamed to examine her private area since she had no sexual intercourse ever since. As a nurse practitioner, we should always remember to promote excellent communication with the patient and family, encourage appropriate advance care planning and decision making, and demonstrate empathy for patient and family emotions (Rabow, 2004). If the patient still refuse despite of all the explanations that we did, I will talk to the family and consult a psychologist to evaluate her capability in decision-making. Great post.

In order to provide effective teaching, clear objectives need to be applied. Health educators need to establish both clear and measurable learning objectives. Learning objectives should be specific. Both the patient and educator should come to in agreement on the subject matter that is to be addressed. Not only should the objective be specific, but it should also be explained at the patients/family level of understanding and learning style.

 

According to Petti & Fetro (2006) there are seven habits that need to be involved in teaching in order to be an effective. The first habit is being proactive. This involves the educator taking the time to understand patients learning style, incorporate proper teaching, and provide frequent teaching. The second habit is to begin with an open mind. Having a open mind will allow one to establish specific goals and outcomes that can be measurable. The third habit is to put first things first. This involves prioritizing patients and families needs. The fourth habit is to think win/win. This involves success for patient, patients family, and educator. The fifth habit is to seek first to understand then to be understood. The educator should take the time to learn as much information from patient as needed in order to establish an effective plan and obtain patients trust. This will also make patient feel he/she is important. Educators need to be able to communicate with a variety of cultural and socioeconomic backgrounds. The sixth habit is synergize. Synergize involves the collaboration between the learners and educator. The more people involved the more effective the teaching will be. For example the more patients families involves in patient care the more support the patient will receive. The last habit is to sharpen the saw. This allows for continuation of growth, expanding knowledge and change. An example of a specific and measurable teaching can involve teaching a patient about their medication metformin’s side effects. The outcome would be for the patient to name two side effects. The teaching will be provided at the patients/family educational level and learning style.

 

Health educators teaching will be more effective if they incorporate patients and their families. Learning objectives should be patient centered. Outcomes should be measurable, because only measurable things can be evaluated for effectiveness.

The theoretical perspective that I find to be the most helpful and effective would be the risk-factor perspective. Family risk factors are those that come from lifestyle, biological factors, environmental factors, social, psychological, culture and spiritual dimensions and the health care system (Edelman, Kudzma & Mandle, 2014). Nurses need to explore work pressures, peer pressure, stress, anxieties, tensions, and air, noise or water pollution. There are also cultural and spiritual aspects that include certain traditions that help prevent illness, such as daily prayer and meditation (Edelman, Kudzma & Mandle, 2014).

 

To reduce risk factors, nurses must help families focus on influencing on health behaviors of the members of the family. Having an awareness of certain risk factors can help families to reduce risk factors that are modifiable. Being healthy and having healthy behaviors are very important to the family and are a big responsibility. The probability of a risk could change depending on the activities of health promotion and disease prevention that the family follows (Edelman, Kudzma & Mandle, 2014).

 

This theory appeals to me more than others because there are so many risk factors out there that can affect a family. Once nurses explore certain areas of family members that could cause an issue within the family, they can help try to fix the issue and in turn make the family healthier. There are certain habits that one family may take on and another member, such a child, could take it on as well and it may not be a healthy one, such as eating fatty foods. It is best if the family eats healthy and it starts with figuring out the areas of the family that are considered a risk to other members and each individual.

Discussion 1: Social Technologies in the Workplace

 

As a nurse manager, it is imperative to understand how social technologies can impact productivity, peer-to-peer relationships, and patient safety within the workplace. Collaborating with HR to understand policies and the organization’s expectations related to the use of social technologies allows nurse managers to more effectively and appropriately integrate today’s social platforms while mitigating the occurrence of inappropriate behaviors.

For this week’s Discussion, your Instructor will assign you to one of the scenarios below. You will then use that scenario to investigate the social, ethical, and legal ramifications of social technologies.

 

Scenario One: You have recently been promoted to charge nurse for the day shift in your department. As a result of this promotion, your former peers are now reporting directly to you. You have been working in your setting for five years and consider many of these peers to be your friends. The way that one of your friends posts on social media sites has always bothered you. Many times, her comments are inappropriate, discussing her negative feelings about “ignorant superiors” and “annoying patients.” You also know that she frequently accesses these sites for extended periods of time while patients are waiting to be helped. Now that she reports directly to you, how do you address this? If your workplace does not have a formal policy on social media use, how can you adapt her behaviors to align with the expectations of nursing professionalism?

 

Scenario Two: It has been a little over a year since you accepted your nurse management position at a local pediatrics office. Since then, you have observed that many nurses seem uneasy when they work with one of your top physicians. You wonder why so many nurses are hesitant to work with such an experienced physician, particularly since most of your families rave about her dedication and caring nature toward their children. One day, while taking careful observation of your staff, you watch a nurse take out a patient chart and begin to text. The nurse becomes visibly upset as she sees you approach. When you question her about her behavior, she confides that the physician asks all nurses to text patient test results to her. The nurse admits that she feels uncomfortable sending private information via text message and only did so after multiple requests and increasing pressure from the physician. According to ethical and legal guidelines, who is at fault for this error in judgment? Since the nurse is the employee who sent the information, should action be taken against her, against the physician, or against both parties?

 

Scenario Three: A physician in your setting is an avid user of social media. On many of his personal pages, which include blogs about his various outdoor hobbies, he plasters pictures of himself and his friends out drinking. He also tends to post extreme comments about politics and the economy. Many in your setting joke with him about the intensity of his social life, to which he always comments, “Work hard, play hard.” Though his actions are not hurting the morale of the setting, and his posts are always before or after work hours, should anything be said to this physician? In the future, could your setting experience any ramifications because of his presence in social media?

 

Scenario Four: The environment in your critical care unit has always been somewhat hectic. However, over the last few months, the morale in your unit has noticeably deteriorated. You learn that nurses have begun to share mass e-mail chains complaining about unfair scheduling treatment. As the nurse manager, you have always tried your best to schedule staff fairly and even allow them to switch their shifts when personal problems arise. Since your effort to talk with staff individually does not seem to be working, you decide to hold a town hall meeting to openly discuss these issues. As you prepare the meeting announcement, you notice e-mails from your hospital administrator, chief operating officer and director of human resources. All are wondering why their inboxes have been inundated with e-mails from upset nurses in your unit. How do you respond to these e-mails? Furthermore, how should you address this situation to improve the morale of your unit?

Review this week’s Learning Resources, focusing on the appropriate and inappropriate use of social technologies in the workplace.

Conduct further research on the social, ethical, and legal issues that result from inappropriate use of social technologies both inside and outside work hours.

For example, what laws protect the privacy and free speech of employees? How can workplaces legally safeguard themselves from various social technology issues such as defamation, misrepresentation, or misuse by individual employees? Of what legal ramifications, such as patient privacy and confidentiality laws, should managers be aware?

 

Reflect on the scenario to which you have been assigned.

How might employee use of social technologies impact the rest of the setting? How might it impact the care given to patients?

 

As the nursing manager who oversees the employee(s), what strategies (if any) would you employ to effectively address this situation?

 

Consider the social technology policies of your past or present workplace.

With regard to employee use of social media, do you believe the workplace policy is effective? Why or why not? As a nurse manager, could you use this policy to effectively address the behaviors in your assigned scenario?

Post an explanation of the possible social, ethical, and/or legal ramifications of your assigned scenario. Explain the policy your current or past workplace has on the use of social technologies and how effective you believe the policy is. If your workplace does not have a policy, explain what the accepted practices or expectations are for your setting. Has lack of policy led to any problems? Support your responses by referencing authentic examples from the workplace and this week’s Learning Resources as appropriate.

Respond to at least two of your colleagues on two different days using one or more of the following approaches:

Ask a probing question, substantiated with additional background information, evidence, or research.

Share an insight from having read a colleague’s posting, synthesizing the information to provide new perspectives.
