Understanding that research and applying it to medical practice can be challenging and will require careful consideration.  Epidemiology and the application of established approaches help to guide practitioners to diagnosis, prognosis, and treatment of patients (Croft, 2015).

The evidence for evidence-based medicine is all collected via research, which uses a variety of study designs.  Different study designs provide information of different quality. In observational studies, the researcher observes and collects information, but does not try to change the people being observed. By contrast, in an experimental study, the researcher intervenes to change something (e.g. gives the patient a drug) and then observes what happens. In an observational study, there is no intervention (uOttawa, 2017).

The traditional model of clinical practice incorporates diagnosis, prognosis, and treatment. Diagnosis classifies the sick patient as having or not having a disease. Historically, diagnosis was regarded as the primary guide to prognosis and treatment of the patient. Disease diagnosis often occurred without effective treatment leading to a poor outcome for the patient (Fletcher, 2014).

But now, there is a demand for health care to deliver proven quality and better outcomes for the patient. The science of prognosis focuses with improving the precision, accuracy, and usefulness of measures of likely future outcomes. Prognosis offers an alternative starting point looking at all factors that are relevant to patient outcomes other than diagnosis alone (Croft, 2015).

There are six core functions in epidemiology: public health surveillance, field investigation, analytic studies, evaluation, linkages, and policy development. All six of these relate to each other and are necessary in the study of epidemiology. I am going to focus on the evaluation step. This is an important step in the process of determining the relevance, effectiveness, efficiency, and impact of activities with respect to established goals. Evaluation of an immunization program, for example, might assess the efficiency of the operations, the proportion of the target population immunized, and the apparent impact of the program on the incidence of vaccine-preventable diseases (CDC, 2012). Evaluation of outcomes based on evidence in an essential step in the process (Melnyk & Fineout-Overholt, 2015).

An example of how an advanced practice nurse (APN) might involve the evaluation step in the epidemiology process is through weighing benefits against harms of prevention. When educating patients about the importance of annual mammograms, an APN would review the guidelines with the patient, evaluate the patient’s current circumstances, and offer recommendations. Such an approach would help clinicians and patients understand what is involved in the evaluation process when making the decision to screen. (Fletcher, Fletcher, Fletcher, 2014).

The two physicians that are also the husband’s friends should not be the ones allowed to sign off on her being admitted to the psychiatric unit. Although the law in New York State allows two physicians to sign off on the paperwork to admit someone, they should be two physicians that are not already related to the client and the case in some way. Currently, the wife is not in danger to herself or anyone else, she is only intoxicated and being loud. Which does not meet the requirement for involuntary admission to the psychiatric ward. It is the nurse’s responsibility to be the patient’s advocate in situations such as these. Due to the patient being intoxicated she is unable to make sound decisions herself so the nurse must follow her code of ethics to protect this patient. Beneficence is the expectation of a nurse to do good and prevent harm to all patients (Westrick, 2013). Nursing codes for professional practice and ethics were developed to protect both the nurse and the patients (Ingham-Broomfield, 2017).

As the neighbor you are aware that the individuals are currently going through a divorce, so you cannot rule out that he is putting her at greater risk of losing her children. The involuntary commitment of a possible psychiatric client requires two types of evaluations. First, ethically a clinical evaluation of the patient’s condition must be made in assessing their medical needs (Rotvold, & Wynn, 2015). Second, an assessment of the legal basis is essential as it relates to confidentiality and legal rights of individuals (Rotvold, & Wynn, 2015). It seems that the husband is strongly using his personal affairs to influence the physicians to commit his wife. To be sure that this situation was handled accordingly for the patient I would discuss my concerns with the charge nurse/supervisor on the floor that night as well to further advocate for my patient.
