Psychiatric notes are a way to reflect on your practicum experiences and connect the experiences to the learning you gain from your weekly Learning Resources. Comprehensive psychiatric evaluation notes, such as the ones required in this practicum course, are often used in clinical settings to document patient care.

For this Assignment, you will document information about a patient that you examined at your practicum site, using the Comprehensive Psychiatric Evaluation Note Template provided. You will then use this note to develop and record a case presentation for this patient.

To Prepare

· Review this week’s Learning Resources and consider the insights they provide about impulse-control and conduct disorders.

· Select a patient for whom you conducted psychotherapy for an impulse control or conduct disorder during the last 6 weeks. Create a Comprehensive Psychiatric Evaluation Note on this patient using the template provided in the Learning Resources. There is also a completed template provided as an exemplar and guide. All psychiatric evaluation notes must be signed, and each page must be initialed by your Preceptor. When you submit your note, you should include the complete comprehensive evaluation note as a Word document and pdf/images of each page that is initialed and signed by your Preceptor. You must submit your note using SafeAssign. Please Note: Electronic signatures are not accepted. If both files are not received by the due date, Faculty will deduct points per the Walden Grading Policy.

· Then, based on your evaluation of this patient, develop a video presentation of the case. Plan your presentation using the Assignment rubric and rehearse what you plan to say. Be sure to review the Kaltura Media Uploader resource in the left-hand navigation of the classroom for help creating your self-recorded Kaltura video.

· Include at least five scholarly resources to support your assessment and diagnostic reasoning.

· Ensure that you have the appropriate lighting and equipment to record the presentation.

The Assignment

Record yourself presenting the complex case study for your clinical patient. In your presentation:

· Dress professionally with a lab coat and present yourself in a professional manner.

· Display your photo ID at the start of the video when you introduce yourself.

· Ensure that you do not include any information that violates the principles of HIPAA (i.e., don’t use the patient’s name or any other identifying information).

· Present the full complex case study. Be succinct in your presentation, and do not exceed 8 minutes. Include subjective and objective data; assessment from most recent mental status exam; current psychiatric diagnosis including differentials that were ruled out; current psychotherapeutic plan (include one health promotion activity and one patient education strategy you provided); and patient progress toward treatment goals.

· Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What was the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?

· Objective: What observations did you make during the psychiatric assessment?

· Assessment: Discuss the patient’s mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses in order of highest to lowest priority and explain why you chose them. What was your primary diagnosis and why? Describe how your primary diagnosis aligns with DSM-5 diagnostic criteria and is supported by the patient’s symptoms.

· Plan: What was your plan for psychotherapy (including one health promotion activity and one patient education strategy)? What was your plan for treatment and management, including alternative therapies? Include nonpharmacologic treatments, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan.

· Reflection notes: What would you do differently with this patient if you could conduct the session again?

To gain the knowledge, you require a good understanding of how to search for scholarly resources, as well as identify which databases and websites are credible for the purposes of implementing evidence-based changes in practice.

Scenario

You are supervising three nurses working on the medical-surgical floor of a local teaching hospital. This hospital is nationally recognized as a leader in education and has a computer lab with an online library where staff has access to medical research databases (that is, CINAHL, PubMed, Medline, and Cochrane library) and online sources of all hospital policies, procedures, and guidelines, and computers at nurse workstations that also have access to these resources. (For this scenario, use the Capella University Library to simulate the hospital’s online library.) You have given the nurses their patient assignments and you have all participated in shift report. A new nurse who just completed orientation and training a week ago approaches you and tells you that one of the assigned patients has a diagnosis he or she is very unfamiliar with. Knowing that patient-centered care based on best practices is imperative to positive patient outcomes, you want to assist this nurse to find research that can be utilized to provide the best care for this patient. Describe how you would communicate with this nurse to encourage him or her to research the diagnosis. Assume you will assist in the quest to locate evidence, then describe where you would go within the facility and what resources you would look for. These resources may include websites, journals, facility policies or guidelines, or any other sources of online information.

You may choose the diagnosis for the patient in this scenario or health care issue/diagnosis of your choice. Or you may choose something you would find interesting to research or that applies to a clinical problem/diagnosis you would be interested in addressing. Create a list of at least five sources that could be used to find evidence, with the best source listed first, and explain why the sources you chose are best to find evidence for the diagnosis you chose and the clinical scenario. You are only evaluating the sources of evidence (database, website, policy database or website, journal article, et cetera). You are not actually completing a search and selecting evidence. Consider the following examples: a nursing journal in CINAHL may not be the best source of evidence for information on how to administer medications through a central-venous catheter, whereas a hospital policy database found on a website may not be the best source of information on caring for a patient with a rare chromosomal abnormality.

Developing a Clinical Question and Selecting a Framework

Nursing research and evidence-based practice are generated from clinical problems identified in practice, often in search of a solution to the problem. Once a problem is selected, practice in asking and writing the right question about the problem is needed. Distinguishing between a background and a foreground question is the first step in this process.  In general, nurses with less experience begin with a background question, but as experience increases, foreground questions become prominent.  A focused foreground question in a Population, Intervention, Comparison, Outcome and Time (PICOT) framework provides the components of an answerable, searchable question.  The language used in the question represents the purpose of the question.  For example, research questions generally have a directional outcome. (i.e., to reduce or improve); clinical questions gather the evidence related to an outcome (i.e., who, what, and where); and quality improvement questions are focused on processes influencing the outcomes (i.e., why) (Fineout-Overholt & Stillwell, 2019).  Once a clinical question is drafted, a literature search is conducted using key search terms, and a framework or model from which to develop a method for answering the question should follow.

As a doctorally prepared nurse, the ability to understand how to ask the clinical question, search the literature, appraise the information obtained, and organize it is imperative. The purpose of this course module is to practice building the skill for these critical features. This module is an exercise in asking an answerable, searchable question for study, performing a scholarly literature search, appraising the publications selected and creating an organized evidence (evaluation) table for easy reference when drafting the introduction and literature review for a clinical scholarship project.  An evidence-based framework or model will also be selected to guide the draft of the clinical scholarship project.

Student 1 For very many American citizens, the fight over repealing Obamacare on the “Affordable Care Act (ACA)” isn’t about electoral strategy or vote counting, whether or not they will indeed be able to pay for their hospitalization and medications once the issues have passed is a major concern for many families. (Willison & Singer 2017). Regarding healthcare practitioners, this is about being able to service all of their consumers without being overburdened by the expenses of uninsured patients. In addition to protecting employment and companies, states must have the resources to deal with the opioid issue and other health care crises, such as the Ebola outbreak. Therefore, it is essential to look at the “repeal and replacement of the affordable care act” through legislators. There’s no doubt that the “Obama administration’s Patient Protection and Affordable Care Act of 2010 (ACA)” was a ground-breaking piece of legislation. (Thompson et al. 2018). A significant endeavor was made for the first time to address healthcare equity in the US. With Republican members publicly opposing the measure, the policy was plagued from the beginning by political connotations. When Trump took office, one of his top priorities was repealing, defunding and replacing the “Affordable Care Act.” The Senate approved a motion in the beginning of 2017 by a majority of 51-48, a move to begin discussing the budget measure. (Thompson et al. 2018). Budget and tax-related provisions of the ACA were suggested for removal in the budget resolution. The final law was anticipated to abolish measures that gave tax credits for low-income persons to purchase health insurance, mandated all individuals to have insurance, and compelled companies to provide insurance to their workers to repeal these provisions. In a vote taken on July 25, 2017, the Senate approved a move to advance to the passed repeal and replace measure, the “American Health Care Act of 2017 (AHCA).”  The vote was 51-50 in favor of the proposal. Senators Murkowski and Collins voted against the proposal, as did all Senate Democrats. It was Vice President Pence’s voting that broke the 50-50 tie. In spite of his recent diagnosis with brain cancer, Republican Senator McCain came to Congress to vote in support of the measure. In the absence of his vote, the proposal would have been defeated. Finally, there was a tax reform budget to be reviewed and the bill was not voted on.

The impact of re-election prospects on the ACA repeal and replacement effort It quickly became clear to the lawmakers, both Republican and Democrat, that ‘repeal and replace’ was not going to help them win re-election because of its wide-ranging negative effects. (Willison & Singer 2017). Legislators quickly realized that the new ‘repeal and replace’ ideas would have a negative impact on the average American voter. A growing number of Republican lawmakers began to realize that their prospects of re-election were diminishing as a result of their support for repealing and replacing Obamacare. According to research, the “repeal of the Affordable Care Act” would save the central government $91 billion in 2021 plus $930 billion from 2017 and 2026, which seems to be among the aims of those favoring repeal. That’s a drop of 21 percent, which is a significant change. (Willison & Singer 2017) The new approach, as shown above, would have left many Americans without access to affordable medical care. (Jacobs et al 2021).  The “American Academy of Family Physicians,” for example, criticized the AHCA’s measures because they increased health vulnerability for Americans. Because of their age, health, or socioeconomic condition, the AAFP says that this new policy is driving up expenses and denying coverage to people. (Jacobs et al 2021).  Additionally, the expected effects on budgetary allocations and the entire employment market show just how essential the health care industry to our economy is. The research encourages us to look for unforeseen repercussions in whatever changes we contemplate making. Student 2 A cost-benefit analysis is a systematic approach to estimating the strengths and weaknesses of alternatives (2021). Many companies across the world use cost-benefit analysis to make decisions. Congress also sometimes allows cost-benefit analysis to be taken into consideration by legislators. The first job of a legislator is to be re-elected, and to be re-elected, the legislator must consider the wishes of their constituents.  Cost-benefit analysis is considered a decision helping, not a decision-making tool by legislators. The risks, costs, and benefits under scrutiny are usually difficult to establish with precision (2021). The cost-benefit analysis turns all outcomes into a monetary value, allowing lawmakers multiple options to ensure the best results for the lowest cost (2022).  As some legislators talk about repealing or replacing the Affordable Care Act, the cost-benefit analysis could come into play. When the ACA was passed, it was meant to expand affordable health care to all Americans, lower costs and improve quality and care coordination (Management, 2015). Less than 50% of Americans statistically support the Affordable Care Act. Many Americans oppose the government’s role in healthcare and have little trust in the government. Republicans posted many negative television ads against the ACA in 2012, which skewed Americans’ knowledge of what the ACA truly is. Almost 60% of Americans say what they know about the Affordable Care Act is from television (JS).   Although there are many positives to the Affordable Care Act, health insurance markets can only stay afloat because of federal subsidies, and middle-income families without employer-provided insurance and small businesses have increased healthcare costs. ACA has damaged the individual market for healthcare insurance (Blase, 2022). National policy is affected by a legislator’s conservative versus liberal viewpoint and by the desire to be re-elected by their constituents.   The ACA is complex, and there has been no reasonable alternative to repeal and replace it. Alternatives mentioned are health care savings accounts and working with individual companies to lower costs. Repealing the Affordable Care Act without replacing it is not an option due to leaving millions without healthcare.
