Marcia Stapleton    2 posts   Re: Topic 10 DQ 2  Evidence based practice (EBP) is a large key to improving patient outcomes and therefore is an essential component of the BSN prepared nurse.As nurses, if we strive for improving the capacity for evidence based care in our facilities, we will make significant strides in achieving the best possible outcomes for our patients.We must, continue to look outside our current practice to seek better practices for our patients (Halm, 2010).  In continuing to integrate EBP into my personal work environment, two ways would be through my role as a clinical nurse leader and through exemplifying a culture of learning and valuing applied research to the everyday routines of practice.Working as a nurse supervisor, I have a role as a clinical nurse leader.This role includes working with policies, workflows and new ways of doing things.Clinical nurse leaders support EBP in all of these roles by always asking the question “What is best practice?” Clinical nurse leaders also can assist in directing other nurses to data bases available to look into EBP research.In our facility, we have Up to Date embedded into our electronic medical record, which gives us quick access to EBP. Asking the “What is best practice?” question leads nursing into looking for EBP research to support the practices they are working with and exemplifies EBP as a culture.  Two obstacles that challenge the ability to integrate EBP into a work environment include time to read and implement new research and support from other staff (Halm, 2010). Reading and implementing EBP research takes time. Some ideas for overcoming this barrier are allowing BSN staff to have a certain number of hours allotted each month to EBP. The concept of EBP teams is awesome and seems like a great idea to overcome both of these barriers (Halm, 2010). If a facility could give these EBP teams allotted time each month to pursue a EBP project relevant to their practice this would overcome the barrier of time. Maybe we could have EBP committees a requirement for all nursing. In clinical settings, advanced practice nurses often encounter patients with blood disorders such as anemia. Consider the case of a 17-year-old girl who is rushed to the emergency room after suddenly fainting. The girl’s mother reports that her daughter has had difficulty concentrating for the past week, frequently becomes dizzy, and has not been eating normally due to digestion problems. The mother also informs the nurse that their family has a history of anemia. With the family history of anemia, it appears that this is the likely diagnosis. However, in order to properly diagnose and treat the patient, not only must her symptoms and family history be considered, but also factors such as gender, ethnicity, age, and behavior. This poses the question: How do patient factors impact the incidence and prevalence of different types of anemia?

To Prepare

  • Review Chapter 21 in the Huether and McCance text. Reflect on the pathophysiological mechanisms of iron deficiency anemia.
  • Select one of the following types of anemia: pernicious anemia, folate deficiency anemia, sideroblastic anemia, chronic inflammation anemia, or post-hemorrhagic anemia. Identify the pathophysiological mechanisms of the anemia you selected.
  • Consider the similarities and differences between iron deficiency anemia and the type of anemia you selected.
  • Reflect on how patient factors such as genetics, gender, ethnicity, age, and behavior might impact these anemic disorders.

Many of us can think of leaders we have come to admire, be they historical figures, pillars of the industry we work in, or leaders we know personally. The leadership of individuals such as Abraham Lincoln and Margaret Thatcher has been studied and discussed repeatedly. However, you may have interacted with leaders you feel demonstrated equally competent leadership without ever having a book written about their approaches.

What makes great leaders great? Every leader is different, of course, but one area of commonality is the leadership philosophy that great leaders develop and practice. A leadership philosophy is basically an attitude held by leaders that acts as a guiding principle for their behavior. While formal theories on leadership continue to evolve over time, great leaders seem to adhere to an overarching philosophy that steers their actions.

The biggest challenge to implementing evidence-based practice (EBP) within the organization according to my mentor is resistance from physicians and upper administration. She explains numerous individuals want to continue the path that they have always used. With change comes conflict. Overcoming the barriers experienced by the organization with implementing evidence-based nursing into practice has started by updating the policies and procedures to reflect current evidence-based practice measures. The nursing managers, directors, and quality/risk team hold meetings to review and revise practice standards to demonstrate present evidence-based nursing practice. When it comes to executing new policies, there is a level of opposition from nursing staff and providers alike. A possible solution as described by Straka, Brandt, and Brytus, (2013), could be monthly 30-minute lunch and learn continuing education sessions. These meetings would review topics such as searching and understanding literature, and leading staff through practice change (Straka et al., 2013). Getting staff to support EBP is also important to resolving the current issues. O.A.S.I.S. which means open your eyes, ask questions, search the literature, implement the findings and share the results is explained by Straka et al. (2013) to inspire nurses to examine clinical problems and examine the literature for best practices. Empowering staff is key to getting them to endorse an EBP culture.  Reference  Straka, K. Asthma

 

Complications of asthma can be sudden. Consider the case of Bradley Wilson, a young boy who had several medical conditions. He appeared in good health when he went to school, returned home, and ate dinner. However, when he later went outside to play, he came back inside wheezing. An ambulance took him to the hospital where he was pronounced dead (Briscoe, 2012). In another case, 10-year-old Dynasty Reese, who had mild asthma, woke up in the middle of the night and ran to her grandfather’s bedroom to tell him she couldn’t breathe. By the time paramedics arrived, she had passed out and was pronounced dead at the hospital (Glissman, 2012). These situations continue to outline the importance of recognizing symptoms of asthma and providing immediate treatment, as well as distinguishing minor symptoms from serious, life-threatening symptoms. Since these symptoms and attacks are often induced by a trigger, as an advanced practice nurse, you must be able to help patients identify their triggers and recommend appropriate treatment options. For this reason, you need to understand the pathophysiological mechanisms of chronic asthma and acute asthma exacerbation.

To Prepare

  • Review “Asthma” in Chapter 27 of the Huether and McCance text. Identify the pathophysiological mechanisms of chronic asthma and acute asthma exacerbation. Consider how these disorders are similar and different.
  • Select a patient factor different from the one you selected in this week’s Discussion: genetics, gender, ethnicity, age, or behavior. Think about how the factor you selected might impact the pathophysiology of both disorders. Reflect on how you would diagnose and prescribe treatment of these disorders for a patient based on the factor you selected.
  • Review the “Mind maps—Dementia, Endocarditis, and Gastro-oesophageal Reflux Disease (GERD)” media in the Week 2 Learning Resources. Use the examples in the media as a guide to construct two mind maps—one for chronic asthma and one for acute asthma exacerbation. Consider the epidemiology and clinical presentation of both chronic asthma and acute asthma exacerbation.
