  1. Within our health care system there are a number of stakeholders that influence the health care policy development process and ultimately help shape our health care policies.
    • Government (primarily federal and state governments) and interest groups like the American Medical Association, AARP, and the Henry J. Kaiser Family Foundation can have a direct impact on the outcome on the type of health care we receive.
    • Patients (the largest consumers of health care) are stakeholders in the health care system, and the needs and wants of the public also drive the decisions made by health care decision-makers.
    • This assignment will help you better understand the role and influences of the main stakeholders in the health care policy development process.

      Research the main stakeholders in the health care policy development process.

  1. Identify      a person from a culture example include (African, Native American, Asian, Pacific      Island, and Latin American and Middle eastern American) with an illness or      chronic condition. The illness needs to be such that it impacts the      person’s life. Please keep in mind that the person with an acute      illness may be under too much stress to answer questions. You may not      interview a member of your own family. You will use eight questions as      defined by Arthur Kleinman to shape your interview. Be cognizant that you      are incorporating therapeutic communication techniques throughout the      interview.
  2. Once      you choose the person you would like to interview, obtain verbal agreement      and be sure to let him or her know that you will maintain confidentiality      throughout this project. Ask the person which cultural group he or she      identifies with. Read about that culture so you will be prepared for the      interview.
  3. In 300 words review what is known about the cultural values of the      group.
  4. During the interview, you may have others present if the person      requests. If so, you will want to look for and recognize the family      dynamics within his or her culture.
  5. Complete      the interview and submit the typed paper in APA format. A minimum of two      references must be used.

Eight Simple Questions by Arthur Kleinman

The Kleinman questions, developed out of medical anthropology research, are designed to elicit health beliefs and expectations, or concerns, about the illness and treatment. You should be familiar with these questions before conducting the interview. Using the eight simple questions provided by Arthur Kleinman, you should be able to individualize care and understand the person you are caring for. You will interview the person concerning the health problem that he or she considers the most significant. Arthur Kleinman’s eight questions for eliciting the person’s explanatory model (EM) of his or her illness are as follows:

1. What do you call the problem?

2. What do you think has caused the problem?

3. Why do you think it started when it did?

4. What do you think the sickness does? How does it work?

5. How severe is the sickness? Will it have a short or long course?

6. What kind of treatment do you think you should receive? What are the most important results you hope to receive from this treatment?

7. What are the chief problems the sickness has caused?

8. What do you fear most about the sickness?

The paper must include the following items, as well as general elements of a well-written paper (clear, specific introduction and conclusion, good transitions between paragraphs and new ideas/points, proper APA format, etc.):

· Discuss what you learned about conducting a culturally sensitive interview.

· If you heard any comments or beliefs that were different than your own views, knowledge, or beliefs, did you respond to what you heard? If you responded, do you think it was the most useful way to respond? If so, how, if not, what would you do in the future?

· In the future, if you conduct an interview with someone from a different culture, what would you do the same and why? What would you do differently and why?

· What did this contribute to your understanding of the healthcare values, beliefs, and/or practices of different cultures?

For this project, you will identify a case study and then develop a nutritional care plan for the client associated with that case. You should begin working on your project in advance of Week 5 as you construct the details of your case. The completed nutritional care plan for your client is due

Project tasks to be completed in Week: 5

Task 1:

  1. Choose from one of the following stages: pregnancy, infancy and childhood, adolescence, adulthood, and old age.
  2. List the specific nutrition issues affecting individuals in your selected stage.

Task 2:

  1. Choose a disease or medical condition that could possibly, or commonly, affect your individual. This could be anything ranging from diabetes to cirrhosis.
  2. Using the ABCDs of nutritional assessment, create a case study based on an actual client or patient. You can also make one up. Include the following in your case study:
    1. Anthropometrics, relevant biochemical tests, clinical assessment, and dietary intake analysis.
  3. Be sure to include a list of common medications that may be used to treat your patient’s condition and identify potential herb/nutrient/drug interactions that may be relevant.
  4. Identify your client’s cultural background and give clues as to their socioeconomic status and psychosocial variables. For example, your patient may be a senior living alone on a fixed income or might be living in a nursing facility with reputed staff.

  1. Create a nutritional care plan for your client. This will include an evaluation of nutritional risk, a list of interventions, and a list of expected outcomes.

Are enteral feedings by tube necessary? If so, what formula will you use and why? Describe your recommended method of administration.

Task 2:

  1. Describe your nutrition education teaching session with your patient and/or their family. What teaching methods will you use (explanation, discussion, demonstration, handouts, etc.)? In your own words, write a paragraph detailing three specific points that you will need to teach your patient about his/her new diet. PATIENT INTRODUCTION

Report from day shift nurse:

Situation: Christopher Parrish is an 18-year-old male who was admitted at 1900 today. His mother visited him at his college dormitory and was very concerned with his health; he seemed weak and had lost weight since she last saw him. She took him to see his primary care provider, and the provider admitted him and has ordered a tube feeding. I placed an 8-Fr, 42-inch feeding tube in his right nares about an hour ago, and x-ray just called and confirmed placement in the stomach. The pump is in his room. He is up to the bathroom prn; otherwise bed rest.

Background: Christopher was diagnosed with cystic fibrosis as a child and has had frequent hospitalizations previously. He reports fatigue and has recently lost 6 kg (13.2 lb) after he registered at the local college and moved to live in a dormitory. Chris’s mom was here earlier, but she is a single parent and has two younger boys, so she had to go home.

Assessment: Christopher is awake and alert. His heart rate and rhythm are regular at 80-85/min. Breath sounds are fine with a respiratory rate at 18/min. His color is a bit pale. Blood pressure is 118/78 mm Hg. He reports no pain and states he’s not had much appetite the past few weeks. His belly is flat and nontender. Bowel sounds are normoactive.

Recommendation: Christopher is due for vital signs and assessment. The tube feeding just arrived, and you will need to start it on the pump. He needs 720 kilocalories over 8 hours overnight. His regular diet is high calorie, high fat, but he wasn’t too hungry this evening; just had a bit of his chocolate shake. You will need to give his pancreatic enzymes orally before you start the tube feeding. You should also assess his diet and reinforce patient education on nutrition.
