The PubMed database is a unique tool to help the practitioner search and find reliable and valid results with less effort. Two features of the database are its ability to differentiate the priority of search terms in the results list using MeSH terms, and to manipulate the search algorithms to help the practitioner answer clinical questions (Chapman, 2009). These tasks are accomplished by additional options tabs during navigation of the search functions. Another unique feature of PubMed is the use of a sub database called Entrez, which is a text search and retrieval system that incorporates the database into thirty-eight other databases to increase the ability for precise search results (Entrez Help, 2016). This addition separates PubMed from other databases by allowing for more layers of refinement of searches conducted to increase the likelihood of finding exactly what the researcher is looking for.

In contrast, the Cochrane Collaboration and Medline databases entail two different search processes when conducting research. The Cochrane collaboration is a stand-alone database that is comprised of systematic reviews and meta-analyses conducted by internal researchers. Medline is the National Library of Medicine’s search engine encompassing many different databases, including PubMed.

The three key questions in research are: Are the results valid, what are the results, and will the results help me in caring for my patients (Fineout-Overholt, & Melnyk, 2015). These three questions measure a search results’ validity, reliability, and applicability to practice, respectively. Systematic use of these questions has helped me to evaluate research results from the various databases to determine the quality of material in them.

The health social determinants refer to the immediate environment surrounding human population that is critical to the health of the people. The social determinant includes the place where an individual is born, grow, learn, work, worship and develop including the conditions that either favor a healthy living or the vice versa. The social determinants of health include; language literacy, access to efficient health facilities, access to quality education, culture, the social norms surrounding g the community, for example, the racism, and distrust of government, public safety (Raphael, 2009). Additionally, residential segregation, availability of resources to meet social needs, access to mass media and emerging technologies, transportation options, and Availability of community-based resources in support of community living and opportunities for recreational and leisure-time activities

 

In the process of determination of a healthy community, the social environments play an important role both directly and indirectly. First, the access to education is a detrimental factor. According to statistics, most of the educated communities, experience minimal health issues as compared to the uneducated. In a case where there is no access to education, the population is therefore not aware of the effects of some practices which are harmful to their health (Raphael, 2009). Secondly, the employment environment which enables the employees to manipulate their environment to reduce stress and strain is important in ensuring a healthy environment which enhances the human health. The access to health services is important in prevention and treatment of the diseases which would otherwise affect health. The income and social status are also related to the healthiness of the community. High income is directly related to good health. The greater the gap between the high-income earners and the poor, then the more the health issues will arise.

 

The communicable disease chain is the process of passing an infection from one individual to another, for example if a child is infected with the common cold on Monday in school, and then the probability that the rest of the class will be infected by the end of the week is high. The communicable diseases are passed from one individual to another through direct contact or exchange of the body fluids. The chain is made up of six different links that play unique roles in the process. The links include pathogen (infectious agent), reservoir, portal of entry, portal of exit, means of transmission, and the new host (Lang, 2015).

 

Nurses play a key role in the breaking of the chain of a communicable infection. For an infection to be transmitted from on individual to another, it is essential that all the links are available. Therefore, the nurses are mandated to breaking the chain by removal of one of the links in the chain. For example, in the process that the nurses have been handling a patient with a communicable infection, the chances are high that they are carrying the pathogens on their hands which act as the means of transmission (Lang, 2015). It is thus important that they clean the hands using water and an antiseptic soap for fifteen minutes before handing the patient. The washing process is important in ensuring that the pathogens are destroyed and thus the removal of the transmission means. It is recorded that some of the pathogens like methicillin-resistant Staph aureus (MRSA) are resistant to non-medicated soaps and thus it is recommended that medicated are used.

Recent reports suggest that, there are 80% of reported cases of STDs reflecting on limited screening of these infection in the company. Despite high campaigns on safe sex, STDs still continues to be menace especially amongst youths who does not use contraceprectives like condoms. According to recent report about 60% of new HIV infection reported in the country are young people. Therefore, it is important to control the disease especially amongst the young generation who are the future of the country. The prevention of the disease can be done through education, identification of asymptomatic and symptomatic people as well as effective treatment and early diagnosis of the patient.

Geriatric patients have many nutritional and hydration concerns that impact their health and ability to acquire sufficient nutrients. Advanced practice nurses evaluating these patients must be able to account for all barriers that prevent elders from obtaining adequate nutrition, including medical conditions, transportation, finances, physiologic changes, and functional abilities. When evaluating patients, it is important to consider how they eat, what their diet consists of, and whether they have any special dietary needs that are not being met. Assessment tools, such as the Lawton Instrumental Activities of Daily Living (IADL) Scale, are an integral part of this evaluation process as they help providers identify potential obstacles for patients. In this Discussion, you assess a patient at your current practicum site and consider strategies for improving any nutrition or hydration issues.

To prepare:

    • Review this week’s media presentation, as well as Chapters 28 and 29 of the Flaherty and Resnick text.
    • Assess a patient using tools for inpatient and long-term patient care, such as the Lawton IADL Scale.

Note: You should coordinate this opportunity with the Preceptor at your practicum site.

  • Consider whether nutrition and/or hydration might be impacted by the patient’s functional abilities. Reflect on whether the patient is able to go out and get food to eat, cook meals, safely use the stove, etc.
  • Consider the patient’s diet and whether they have any special dietary needs due to medical conditions, such as congestive heart failure, end-stage kidney disease, diabetes, oral health issues, etc. Reflect on whether or not the patient is attempting to compensate for a medical issue and thus creating a deficiency or excess in his or her diet.
