During a job interview, it is highly suggested to always make eye contact with your potential employer or anyone in the room for that manner. Always be sure to greet with a smile and although you may be nervous, you want to maintain confidence. The best way to maintain confidence is to study your resume so that you are prepared to answer any potential questions in regard to skills and prior experience. Another form of adding to your confidence during an interview would be to research the company prior to your interview date. This will not only give you some insight on the company, but it will allow you to ask questions during your interview and many companies and potential employers love this quality.

What is your biggest Weakness?

I would have to say my biggest weakness might be that at times I may attempt to perform too many tasks at once, which I may be overwhelmed and it stress out myself. However at that time I use my biggest strength that consist in take a moment for deep breath, mentally organize my task list and determinate those with priority.

Describe how you resolved conflict with a co-worker or patient?

Working in a BH unit as a psych nurse I can said that in my job environment the most difficult situation involving a patient is management of an aggressive situation. A patient had become highly aggressive both verbally and physically. At this time, my team and I performed the techniques of de-escalation as per protocol. When this was not successful, I, as the lead nurse had to make the difficult decision to call the psychiatrist and receive an order for physical and chemical restraint for the safety of all, the patient, milieu and staff. The psychiatrist led the order and the patient was placed on a four-point restraint in addition to receiving medication to assist in calming the patient. The goal should be always, controlling the situation with the minimal physical and psychological damage for patients and staff members. Always keeping a professional, calmed, team working and positive attitude.

Tell me about yourself?

I am naturally from Cuba. I am privileged enough to be a mother whilst attaining a professional education and career. In 1997, I had the honor of graduating from General Medicine in Cuba, which would be the equivalence of Family Medicine in the United States. I then came to the U.S. and obtained my Registered Nurse certification by 2007. At this time, I began working in a home health setting. This basically involved, but was not limited to: wound care, diabetic management, colostomy management and performing intravenous antibiotics. In 2011, I then began a career at an inpatient psychiatric hospital setting. In 2017 I began my FNP course which I about to finish in this university. Working in the BH environment is what has motivated me to attain a future Post Master in Psychiatry.

MODULE 07 CASE STUDY

S.is a 48-year-old divorced woman with one adult daughter and three grandchildren. She is currently working as an LPN part-time in a nursing home and also works at a convenience store one or two days per week. She has had many jobs over the last 22 years, usually changing every one or two years. S notes that she has been called less often to work in the convenience store and worries that they don’t like her anymore. She reports being written up several times for arguing with customers. She also reports that her supervisor at the nursing home “is a bitch”; although she really liked her supervisor at first, she says “Now I hate her; she’s trying to get me fired.” S. reports that she has tried to get fulltime jobs five times in the last four years, was hired for three, but only lasted one or two weeks at each one. 

S. reports that she is currently not talking to her daughter because “she is horribly mean to me and she needs to apologize or I won’t talk to her again”. She is upset that she hasn’t seen her three small grandchildren in about a year. She sends them presents and cards frequently that says “I still love you! Grandma”, but hasn’t called them since she stopped talking to her daughter. She is considering reporting to the county that her daughter is keeping her grandchildren from her. 

S. is very unhappy that she isn’t in a relationship. She was abused by her ex-husband and has a pattern of meeting and dating men who eventually abuse her. She states that her last relationship was very good, however; the man was not abusive and “I loved him very much”. The relationship ended for reasons that S. doesn’t understand, although she does report many arguments that ended in “scenes” such as her throwing chairs, stomping out of the house, making crank phone calls to his family, and calling the police with false reports. But S. also reports that she “couldn’t have loved him more and I showed it”. She gives examples of going to her boyfriend’s place of work with flowers, buying him expensive presents, surprising him with tickets to Mexico at the last minute – she was very upset that he wasn’t willing to drop everything and go with her. S. reports asking him why he didn’t love her and what she was doing wrong on a regular basis. When the boyfriend asked to break up, S. reports sitting outside his house for weeks, crying; she called his mother, called his boss, and called and texted him until he filed a restraining order. This occurred about 4 months ago.

S. admitted herself to the mental health unit when she felt suicidal. She reports that she had stopped her individual psychotherapy 3 months ago and stopped going to DBT. She also stopped her anti-depressant at that time, as she felt it wasn’t working, and missed her last two psychiatrist appointments.

Questions:

Week 3 – Ethical Dilemma

Please reply to the following discussion with one reference. 

Many ethical dilemmas are illustrated in the documentary The Invisible Patients.  The documentary follows Jessica, a nurse practitioner, as she confronts the challenges of caring for homebound patients.  One of her patients is Roger, a 30 year-old male with muscular dystrophy who is nearing end of life.

Jessica is faced with navigating the ethical issues pertaining to establishing a “do not resuscitate” order (DNR).  In the beginning of the film Roger is hesitant to sign a DNR.  He states that he is not ready to die and desires all available interventions that might allow him to continue living, even if that means “bringing him back.” 

Jessica feels that resuscitation would be a painful prolongation of the inevitable.  This stirs internal conflict for her.  She is faced with two opposing possible courses of action.  According to Hamric, an ethical dilemma occurs when a person is faced with two or more obligations that cannot be simultaneously met (2014).  This creates tension due to the differing and opposing demands of these obligations (Hamric, Hanson, Tracy, O’Grady, 2014)  Jessica has undoubtedly seen first-hand the violent and painful process of cardiopulmonary resuscitation, and she obviously knows that these efforts are usually futile, only resulting in unnecessary pain and suffering.  On the other hand, she is duty bound to respect the patient’s autonomy and his personal wishes. 

Jessica handles this situation very professionally.  She gently communicates to Roger and his family the reality of the progressive nature of Roger’s disease.  She tries to put aside her own emotions during the discussion.  Jessica asks open ended questions and empathetically listens to his responses, giving him positive, affirming non-verbal feedback.  In doing this, she is gathering information to help her navigate this ethical dilemma.   This end-of-life discussion was carried out subtly, tactfully, and at length over multiple visits to the patient in his own home. 

These home visits are themselves an example of the NP as a disruptive innovator.  Traditional medicine as practiced today takes place in a clinical setting or a tertiary care center (Hamric, Hanson, Tracy, O’Grady, 2014).  Jessica provides a service that is outside of the status quo by delivering superb patient-centered primary care in the patient’s own home.

I feel I would handle this situation in the same manner.  I believe that it is a fundamental right for people facing terminal illness to decide their own fate.  However, I also believe that fear of death and the unknown that follows too often drives these decisions.  I believe my role is to reassure, encourage, and help relieve this fear so the patient can feel at peace with a decision that reflects the reality of their situation.
