Current statistics show that at present, there are 1,048 Associate Degree Nursing (ADN) colleges and 696 Baccalaureate Degree Nursing (BSN) institutions in the United States (Cherry & Jacob, 2017). Recent studies show that a correlation exists between patient safety outcomes and whether a nurse has an ADN or BSN. Linda Aiken, PhD, RN, who is associated with the Robert Wood Johnson Foundation (RWFJ), believes that hospitals who employ more BSN-prepared nurses have better patient outcomes and lower mortality rates (RWJF, 2014).

In 2003, Aikens reported on studies done in Pennsylvania hospitals showing a 5% decrease in patient deaths at hospitals that staffed a 10% increase in BSN-prepared nurses (RWJF, 2014). Challengers of Aikens note that these studies raise further questions (Sentinel Watch, 2014). For example, what were the actual causes of death of these patients, and were they related to the action of an ADN nurse? Could there be other factors or causal relationships that affected patient care in these circumstances?

In defense of Aikens, the Sentinel (2014) states that no one is calling ADN nurses “bad nurses”. In fact, errors, infections, and adverse events are often due to process and system problems. Aikens maintains that by promoting evidence-based practice and leadership, the BSN curriculum will better prepare nurses to correct problems and implement solutions.

I agree that it is advantageous for nurses to expand their knowledge by obtaining a BSN degree, but I do not feel that deaths and adverse outcomes are the result if ADNs care, or lack of. Many other factors need to be considered. For example, poor staffing, long shifts, poor communication, fatigue, documentation errors, etc. affect everyone no matter what type of degree they hold. I have been at the hospital where I am currently employed for almost 20 years and have seen no evidence that the care given by ADNs is substandard to the care given by BSN-prepared nurses.

Obesity is increasingly becoming a matter of great medical concern in modern health facilities. Tackling obesity should be prioritized in many health care facilities since it’s on the raise as the many researches carried out have shown. The specific impact of obesity is difficult to assess but its effect to general morbidity and premature death are known.

Body mass index (BMI) method is what is used to measure and determine if a person is obese as described by World Health Organization. This formulae is used in both male and female and gives the most vital indicator of overweight (BMI of 25 or above) and obesity. The BMI is only used as a guide as it does not put into account for muscle mass or for fat distribution which differ between individuals. In the United States the Obesity rates continues to rise at epic proportions. In order to tackle the menace, nurses and other medical professional should be in a position to handle and understand obesity.

The nurses need to know all aspects of obesity which include screening, predispositions, pathophysiology and management of the disease in order to help patients. The knowledge the nurses get will be important in lowering the number of cases of obesity in the country and in long run stop this growing epidemic. The number of obesity cases in children is raising rapidly and hence nurses who have knowledge on its management will be in a better position to help. By educating the nurses about obesity, the risk factors and co-morbidities that manifest due to obesity, will be handled in a professional manner and lead the Country to live in healthier ways.

Nurses will be taught on various causes of obesity in children which include lack of doing enough physical activities and prolonged immobility. Other causes factors leading to obesity include overeating in excess and combined with limited physical activities which will be well inculcated in their learning process. The learning about obesity will help nurses understand the pathophysiology of this life style disease, which is simply taking more calories than required. Nurses will be introduced to the theory about child metabolism and hormone irregularities, which contributes to dysfunction of hormones and energy storage for a long period of time

Part II: Breach of Contract

A  contract consists of a voluntary  agreement that two parties enter into with  the intent of benefitting  each other (Fremgen, 2012). Something of value,  which is termed  consideration, is part of the agreement (Fremgen, 2012). A  contract can  be either expressed or implied. An express contract is an  agreement  that clearly states all the terms. It can be entered into orally or  in  writing (Fremgen, 2012).

A  breach of contract occurs when either  party fails to comply with the terms of  the agreement (Fremgen, 2012).  For example, if a physician refuses to perform  a medical procedure he  or she had agreed to perform, the physician has  breached the contract  (Fremgen, 2012). If a patient does not pay an  agreed-upon fee, then the  patient breached the contract with the physician  (Fremgen, 2012).

Suicide is the taking of own life which is a “tragic reaction to stressful life situations (Suicide and suicidal thoughts, 2015).” The national institute of health reported ages 10-24 year olds in 2014 was the second leading cause of death with a total count of 5,504 deaths in the US (Advancing research to prevent youth,n.d.).  Erickson’s stages of development for the adolescent shows the teen must successfully concur the two stages of development – identity vs. identity confusion and intimacy vs isolation (Miller, 2017).  Teens who are struggling with the first and second stage will become socially isolated (Miller,2017). Erikson’s theory predicts “when adolescents are unable to successfully answer the questions of identity during this stage of development, they may experience feelings of inadequacy and despair, which can eventually lead to depression (Miller,2017).  Depression left untreated can lead to suicide.

Health professionals can utilize primary, secondary, and tertiary methods to help with health prevention of suicide.  “ Primary suicide prevention aims to reduce the number of new cases of suicide in the general population. Secondary prevention aims to decrease the likelihood of a suicide attempt in high-risk patients and Tertiary suicide prevention occurs in response to completed suicides and attempts to diminish suicide contagion (Ganz, n.d.).”

In general, if a true emergency is occurring 9-1-1 should always be the first line to call for help.  When teens are struggling through thoughts of despair and potential suicide the communities do have resources to reach out to.  As followed are some of the resources: Transitional Age youth support- Mental health services: (360)918-7860; Crisis Line: (360)586-2800; National Suicide Prevention Lifeline: (800)273-Talk (community youth services, n.d.).

As the nurse it is our duty to obtain a precise assessment of the teen through thorough questions and observations.  The nurse should be mindful of some risk factors which place this age group at higher risk are as followed: a previous suicide attempt, mental disorders- schizophrenia/social anxiety, substance abuse, abused or mistreated, history found in family, hopelessness, lack of social support, access to means or methods for suicide (Preventing Teen Suicide, n.d.).  A main nursing intervention in assisting a suspected depressed teen is to show active listening and presence.
