In hospice, one of the goals is to have skilled nurse visits at least 3 days prior to death and at least 3-6 days prior to death. If we track ESAS it is supposed to help indicate who we can expect to pass away based on changes reported and then be able to increase nursing visits-however to be effective the data has to be accurate. Just as we need other infrastructure such as roadways, power grids, water supply or other fundamental services necessary for our society to function well, we need to have a network of public health “infrastructure” to form the base of our public health services.   Franklin Delano Roosevelt stated:  “The success or failure of any government in the final analysis must be measured by the well-being of its citizens. Nothing can be more important to a state than its public health; the state’s paramount concern should be the health of its people”.  

In the United States, governments at all levels (federal, state, and local) have a specific responsibility to strive to create the conditions in which people can be as healthy as possible.

Just like we need an efficient transport infrastructure to facilitate a seamless movement within and outside a given place, efficient health infrastructure is what ensures the nation is healthy. According to Healthy People 2020, the goal of having an efficient Public health infrastructure is ensuring that the federal, state, tribal, territorial, and local health agencies have the necessary infrastructure to provide essential public health services efficiently. With a sufficient public health infrastructure, communities, states, and the nation at large will have the ability to prevent disease, promote health, and prepare for and respond to both acute- emergency threats and chronic- ongoing challenges to health. According to Healthy People 2020, an excellent public health infrastructure depends on a wide range of processes such as immunizations, infectious disease monitoring, quality drinking water, injury prevention, up-to-date information systems, and public health organizations with the ability to assess and respond to community needs.

Looking at the U.S public health infrastructure and some of the key characteristics that would define a robust infrastructure, I do not think our infrastructure is solid enough. Our health care system has been described by a wide range of literature as the most expensive in the world but has a poor quality and safety record compared to other developed nations (Squires & Anderson, 2015). Secondly, looking at the major disease burden affecting the healthcare system, I would say that our infrastructure has been focused more on profit maximization. Most of these conditions are preventable. I cannot understand why an important medication such as insulin for over 30 million diabetic Americans is still a patented product and sold by a few companies. In an article by Belluz, the inventors of insulin thought it was unethical to patent a life saving drug and sold it to university of Toronto for $ 1. However, if the investors were alive today, they would be shocked to death to realize that the price of insulin has been on an upward trajectory and some patients have to forego their injection because of the price.  We are living in nation where some people due to their social-economic status have to choose between seeking preventive care and their daily bread. This is not a characteristic of a solid infrastructure. Additionally, Rosenthal (2018), highlights how our healthcare is a huge business by noting that hospitals for long have been taking advantage of patients and the poor in the community.

When speaking of diversity in nursing and the health care field overall, we refer to the differences that will exist between the individuals that make up a work group, although many times the interests are common, there will be differences in terms of age, sex , race, ethnicity, sexual orientation, religion, skills, experience; the work group usually has differences and similarities among its members (Andrews et al, 2019). This diversity is important because it facilitates decision making due to the different points of view provided by each individuals, as well as the distinct approaches and perspectives will allow for easier problem solving, decision making, greater efficiency, and organization. For work, the results obtained for the patients are better. Many times when seeking medical attention, patients look for people who are similar to them in terms of their ethnic background, culture, language, as they feel more understood and more comfortable.

 The more diverse society becomes, the workforce must also be diverse so that people feel cared for, fulfilling all the expectations from clients. It is stated that in the last thirty years nursing has been undergoing changes and transformations influenced by the aging of the population (baby boomers), by the approval of the health care law (ACA) where health insurance is provided to people who were previously uninsured and finally the preparation in the training of health professionals such as nurses who are better prepared to provide quality care to each patient (Ford, 2019).

 To increase diversity and achieve coordination between the cultural composition and the workforce that serves the population, researchers are studying the influence of corporate culture and organizational climate on increasing diversity and are examining two main approaches to successfully increase diversity, currently fearing diversity management and organizational inclusion (Andrews et al, 2019). This year is declared by the WHO as the year of nursing. It is proposed that diversity is associated with better care for the community which is focused on patient satisfaction, better experience in counseling, patient education and that the doctor, nurse, patient communication is optimal.

  1. The cultural self-evaluation of organizations, institutions, and health care agencies is a process where a series of strategies can be taken in the different health units to evaluate a complete or particular organizational culture of the sea. In this case, the first thing to analyze is demographic and descriptive data, where cultural diversity is an important link for patients, nurses, doctors and other health personnel, with the aim of promoting multiculturalism, the evaluation of cultural strengths is another strategy, looking for programs and activities where Support multiculturalism, and keeping in mind positive comments from customers and staff about the culture (Ford, 2019). Evaluate the resources of the community such as ethnicity, religious groups, political and economic support, observing weaknesses and limitations to work on them to achieve continuous growth. It is of great prominence to identify the leaders who evaluate and communicate in cultural self-assessment, one should evaluate the languages ​​they speak with the aim of having translators if needed. In case it is necessary to make changes, members or stakeholders should be involved, after this implementation of possible changes to be made, if necessary, it is important to evaluate the effectiveness as well as monitor the results, it can be positive to create diversity initiatives and performance measures.

For me diversity means differences, variety, abundance of different things, in this case cultural diversity is nothing more than the coexistence or interaction between different cultures (Ford, 2019). These distinct backgrounds generally provide its people with their perspectives, culture, beliefs, values, and worldview. As diversity in the workforce increases. the quality of medical care tends to improve. Currently the diversity in nurses and health professionals is exponentially rising. With this variety, the health field must protect workers from discrimination due to gender, race, age, or political party; it should work towards integrating more minority populations to receive health care resources. It is argued that this concept of diversity is closely related to interculturality and multiculturalism (Andrews et al, 2019).

 It is important to know that cultural diversity starts from the family itself, with parents first teaching certain cultural elements to their children, so the culture of the children is different from that of the parents because it feeds on the culture of two people, taking elements of each one, merging into a new culture (Ford, 2019).

  1. In the nursing profession there are various barriers to diversity since it brings many backgrounds, cultures, and beliefs together. The language barrier it’s one of these, many people love this profession and due to language problems they cannot realize their dreams. Another barrier is the cost of education, especially for students with low economic levels. Ignorance towards the profession is also an issue brought by diversity along with the absence of adequate IT tools, lack of knowledge about the entire nursing process, ensuring the people on the field have a vocation towards nursing since it’s a highly sacrificing career and not everyone is fit to perform nursing jobs. Nurses are exposed to contracting diseases and the terrible work hours limit people to choose this profession, opting for others where they will feel more comfortable (Andrews et al, 2019).

One of them will be that it guaranteed that numerous of the U.S citizens have coverage. Insurance is particularly vital when trying to obtain medical attention. Furthermore, several screenings are completed at an affordable cost. It advanced healthcare in the U.S to more economical and reliable. The cons involve penalization if an individual is not approved, raised premiums, and difficulties in engaging in this plan.  

3. My image of the nursing future will be to give a satisfactory atmosphere for working. This includes good pay and an enabling environment. It is essential to perceive that several hospitals are normally lacking staff, generating tension on the nurses. This begins to cause fatigue among healthcare individuals.
