 23-year-old Native American male comes in to see you because he has been having anxiety and wants something to help him. He has been smoking “pot” and says he drinks to help him too. He tells you he is afraid that he will not get into Heaven if he continues in this lifestyle.

The patient is a 23-year-old Native American undergoing an annual physical exam. The main area of concern is that he has been having anxiety issues. The patient seeks to have something prescribed to help him cope with constant anxiety. He states that smoking pot and drinking are some of the ways he has been using to help him cope. He is also concerned that he may not make it to heaven if this lifestyle is not controlled. During the exam, the patient seemed anxious observed from his pacing in the exam room and fidgeting.

Factors Related to Patient’s Health

From the exam, several factors can be linked to the patient’s state of health. The spiritual factor is observed when the patient says he is afraid that he will not get into heaven if he does not stop taking marijuana and alcohol. This shows that the patient has spiritual convictions which could be a contributing factor to his anxiety. The patient’s lifestyle is clear when he confesses that he regularly smokes marijuana and drinks alcohol. Socioeconomic factors are established from observations. The patient appears to be well-nourished and well developed. This shows that the patient is well off as he canremain nourished. Cultural factor also contributes to the health of a patient. Native Americans are thought to be a collectivist culture where the people are interdependent and interconnected with each other (Beckstein, 2014). Research shows that conflict between personal values and societal values can contribute to anxiety disorders and particularly for a collectivist culture (Hofmann & Asnaani, 2010). This can be explained to be the cultural factor that could be related to the patient’s anxiety.

Assessment Domains for Conducting a Comprehensive Nutritional Assessment

A comprehensive nutritional assessment is crucial in the process of collecting information that will be used to make decisions for nutrition-related health issues for a patient as well ass developing patient-centered intervention plans (Frank, 2018). To conduct a comprehensive nutritional assessment of the patient, I would use the following domains. Biochemical measurement and analysis of the biochemical data to analyze lab data such as metabolic rate and electrolytes. The Food/Nutrition related history domain will be used to assess the patient’s history of food and nutrients intake in the past. The Anthropometric measurements domain will be used to assess the physical aspects of the patient such as height, weight, and BMI. Lastly, I would use the Nutrition-Focused Physical Findings domain that considers physical appearance, appetite, and muscle (Frank, 2018).

Functional Anatomy and Psychology of a Psychiatric Mental Health Patient

Mental disorders have been over recent years been linked to biological disorders (Weir, 2012). The brain is one of the organs of the mind and therefore any illnesses of the mind are to be approached as a biological illness. To assess specific functions, nurses need to know several key concepts. These include memory/ orientation, speech patterns, body movements,and posture, and level of consciousness (Fortinash & Worret, 2014). By assessing these concepts, a nurse is in a position to assess the specific functions of the brain that might be contributing or exhibiting the patient’s anxiety issues.

Specific Socioeconomic, Spiritual, Lifestyle, and other Cultural Factors that are related to the Health of our Patient 

The patient comes from a low socio-economic background. This is proven by the fact that the patient depends on his daughter for his survival. This type of background negatively affects the patient type of medication and diets taken. On the spiritual background, the patient comes from an Asian country where most people are Muslims. The religion dictates some aspects of diets and health practices to be done on their people (Ball et al., 2015). The patient has a simple lifestyle on his lifestyle, considering he does not have the capital capability, and the culture dictates so. Many patients about his age are demanded by their religion to be role models and look after their own families. Lastly, the health of our patients may be dictated by the facts of his social organization. The patient seems to have no other people who can support him apart from his daughter, thus worrying more about his life. Patients about his age are supposed to have a family that is taking care of him. 

The Domain I would utilize on my Patient when Conducting a Comprehensive Nutritional Assessment.

When conducting a comprehensive nutritional assessment, the best domain to utilize to my patient is better communication and questionnaires. Other domains are health, behavior, and mental state (Ball et al., 2015). Many patients feel free to answer written questions rather than face to face questions. I would consider presenting my patient with a questionnaire and giving them quality time to answer. Also, when communicating, I would avoid maintaining a firm gaze since the patients may feel uncomfortable. 

More so, I would research my patient language preference and try to speak to them accordingly. Many people prefer to come open to nurses who understand them better. On other matters, as a nurse, I would consider knowing my patient’s environment as it impacts their health (Sullivan, 2012). As a nurse, I would consider knowing all the patients’ diets to create a comprehensive nutritional assessment table for my patient. Lastly, a good nurse researches the health history of the patient. This dearly helps the nurse to know the drugs to administer to the patient. For example, a patient history clearly outlines the effects of a certain medicine; thus, the nurse will avoid administering it. 

Discussion of the Functional Anatomy and Physiology of a Psychiatric Mental Health Patient Together with the Key Concepts, a Nurse must know to Assess Specific Functions.

The patient’s functional anatomy seems to be out of normal, with bruises on his upper arm and back. Also, there is chronic prostatitis, frequent falls, unexplainable weight loss, and elevated blood pressure (Ball et al., 2015). In my opinion, the patient may be undergoing physical abuse or an unpleasant environment. These symptoms may also point to malnutrition or much medication as the patient is enlisted to various medications types. 

His body seems to be normal on our patient’s physiology, with his various parts functioning normally. His pupils seem well-positioned and normal. Also, his extraocular movements are intact and well-positioned. More so, his arcus senilis seems high, with this disorder been common with older adults. Moreover, his conjunctiva and scleral have inflammation. Furthermore, his bowels sound normal, and his objective data are ranging correctly. His blood pressure, pulse, temperature, and his body mass index all ranging normally.

For a nurse to assess a patient well, they should familiarize themselves with the following key factors; firstly, the nurse should know the systems of administering medications. Secondly, the nurse should create a better relationship with the patient (Sullivan, 2012). The nurse should also learn the art of knowing all given symptoms and coming up with a precise treatment.

Qualitative & Quantitative Research Design

          A researcher chooses a research method or design as a guide to formulate, answer questions, or test hypotheses (Fain, 2017). A research design is a strategy used to conduct a research study. It is a blueprint or plan used to answer specific questions, and it comprises three distinct elements, a plan, a structure, and a strategy (Bloomfield & Fisher, 2019).  According to Clark & Vealé (2018), qualitative research is used to record data that is not in the form of numbers such as opinions, feelings, and experiences; quantitative research is used to measure data in the form of numbers. Although guidelines for conducting qualitative research exist, each project is unique, and the researcher must decide how to proceed. Because the researcher is the primary instrument of data collection and analysis in qualitative research, focus, and interpretive thinking. Qualitative research seeks to understand people’s experiences, the meanings they assign to those experiences, the psychosocial aspects of and language used in interpersonal interactions, and the factors that influence perspectives and interactions (Ramani & Mann, 2016). Through reflection, researchers can better explain the topic under inquiry by minimizing or disclosing their assumptions and biases while collecting, coding, and sorting qualitative data.

          Qualitative research is also conducted in a natural setting, delving deeply into perceptions and meaning of experiences, institutional culture and practices, barriers and facilitators to change, and reasons for success or failure of interventions. Though researchers may design their study informed by existing theories or their observations, the primary goal is to explore participants’ experiences. Qualitative studies are exploratory; questions are open-ended, do not require a priori hypotheses, and are characterized by “how and why” rather than “what” questions. A well-formulated study question determines what understanding is gained and directly influences the study design and methods employed (Ramani & Mann, 2016). In contrast, quantitative research can be characterized as a formal, objective, systematic process used to describe variables, test relationships between them, and examine cause and effect associations between variables. Quantitative research generates numerical data, mainly informed by rationalist or post-positivist ideas and supported by several assumptions. Also, in quantitative research, control must be maintained. Control refers to the researcher’s methods to prevent or minimize factors that may influence or bias the findings Bloomfield & Fisher, 2019).

           In quantitative research, the researcher’s involvement is defined as detached; in qualitative research, the role is participatory. Qualitative researcher gains access to the participants’ natural environment and is the leading research instrument used to collect and analyze data. In quantitative research, the researcher is objective and distant; however, qualitative research and interpretivism understand that individuals are complicated and respond differently to their environment, and do not require objectivity. Subjectivity is inevitable and often seen as valuable in qualitative research. Acknowledging that it is challenging to eliminate biases, qualitative researchers present their assumptions, values, and reasons for choosing the research topic directly in a positionality statement included in the study’s findings. Both research methods are essential and useful in evidence-based advanced practice nursing. However, the qualitative way best influences it because it records data related to individuals’ opinions, feelings, and experiences instead of numbers.

The characteristics of a performance-driven team consist of many integral parts. First, a performance-driven team needs a leader who is someone that sets goals and instills a purpose for the team and the organization. Open communication, trust, mutual respect, shared leadership, effective working procedures, building on differences, flexibility, adaptability, and continuous learning are all essential characteristics of a good performance-driven team (Neumann, 2014). The performance-driven team is a team that works together towards a common goal and feeds based on their overall performance. 

Intrinsic motivation involves engaging in behaviors because it is personally rewarding to oneself, this type of motivation comes from within the individual (Cherry, 2019). Extrinsic motivation occurs when someone is motivated to perform a behavior or engage in an activity to earn a reward or avoid punishment (Cherry, 2019). Extrinsic and Intrinsic motivation are both key components to team performance because they both help the team achieve their goals. With Intrinsic motivation, the team member will likely perform the behavior or task at hand because he/she feels good about helping the team in achieving the overall goal and can feel a sense of pride and accomplishment. Extrinsic motivation is also a key component of team performance because it too allows for the team to achieve the overall goal. If a person is being motivated extrinsically by some type of monetary compensation or because he/she may get punished for not helping the team, the individual will perform the task at hand in order to receive compensation or avoid punishment.

The ability to navigate a complex project is not something I expected to learn to such a high level of detail when I first planned to work toward my graduate degree.  When I first learned that this was the plan, I thought it was a good idea; but I can’t say that I fully appreciated the impact this would have on my ability to actively engage others in change management.  I deal with projects quite a bit.  I am not the project manager and am always grateful to be paired with a person who loves this side of change management.  I would like to share a couple of moments that have happened during the time of this class.

The first challenge that I was able to address occurred a handful of weeks ago when I was assigned to transition one of our sites from a fully functional express care to a designated COVID test site for pre-surgical patient testing.  Without diluting the purpose of this post with too many details, the site leadership had significant concerns regarding the requested change and so I offered to conduct a SWOT analysis to help the team understand if their concerns were valid or an emotional response to a stressful situation.  In the parking lot of the facility, the entire team participated in a brainstorming activity to complete the analysis.  What resulted was a well-thought proposal where the team was able to outline a plan to achieve the critical elements of the request while acknowledging local weaknesses with shifts in how the plan was executed. 

The next example happened one week ago.  One of my projects is evolving beyond expectations.  Some of the work will be shifted into a distinct phase that can be temporarily backlogged.  A great deal of work needs to be done to ensure that the team is addressing the true root of the problem.  We had just completed the work breakdown structure and so it was fresh in my mind.  Despite an awareness of how complex the document can become, I offered this as a way for the team to navigate the growing number of project tasks.  Only a few sections are completed because there is so much discovery to be done.  Even with that, the team is finding clarity, direction, and an awareness of how this intimidating number of tasks can be organized in a way that makes them achievable. 

I feel empowered by these experiences and look forward to finding more opportunities like the two I have mentioned.  On one hand, these forms are taking more time and increase the work.  On the other hand, they are improving communication and contributing to the way the team perceives the work.  From my perspective, that is a win.
