Scenario: You are a program manager at a regional center that serves children, adults,
and families who have varying disabilities. The regional center provides oversight of
residential care; basic needs; facilitation of social, vocational, and behavioral supports;
and assuring the providers with whom you work meet all local, state, and federal
regulations. As a program manager, you supervise eight full-time service coordinators
who provide case management services to adults with disabilities in local residential
programs. Your service coordinators are responsible for conducting an interdisciplinary
meeting to develop an initial service plan, determine needs, goals, and strategies for
accessing services, and monitoring Rocky’s progress. Your service coordinators also
make recommendations for various services based on individual needs. Once a service
plan is created, it is reviewed quarterly and annually. You must review all service plans

ROCKY JOHNSON

Rocky Johnson is a 35-year-old male, with Down syndrome. He has lived with at home
with his 75-year-old mother. Rocky is treated with Dilantin since childhood for a mild,
frontal lobe seizure disorder. His seizures have been controlled for several years. Rocky
has a severe hearing impairment and uses hearing aids, which he tends to dislike and
will often take them out. His last known seizure was in his early 20s. He has no history
of falls or trauma due to seizure activity. Rocky is verbal, although there is a clear
impediment, and his words are often hard to understand. He supplements his
communication by using basic American sign language. Rocky has always been happy
and helpful. He is kind-hearted and loves to laugh.

His father died when he was 31 and his mother, Martine, has been his caregiver his
entire life. Before his father’s death, Rocky and his father enjoyed fishing and hiking
near the lake. Rocky and his father would swim at the YMCA every Saturday and often
go to the movies. Since his father’s death, Martine is his main source of support and
they have a very close relationship, although her arthritis prevents her from engaging in
large physical activities. Rocky has an older sister, Gala. And he has a friend, Darryl,
who lives down the street from his home. He sees Darryl on Sundays.

Rocky is 5 feet 4 inches tall and weighs 152 pounds. He has sandy brown hair and
brown eyes. He and his family identify as Hispanic-Americans. Rocky enjoys helping his
mother cook and garden and attend church. He likes to listen to music, especially
country or hard rock. Rocky’s form of vocation is helping his mother around the house© 2021 Walden University, LLCand taking walks. According to his sister, Gala, Rocky does not like chocolate or sitting
for long periods of time.

Three months ago, Martine suffered a stroke and was hospitalized. She was then
moved to a rehabilitation center. Rocky has been living with his sister, but she does not
feel that she has the experience or time to dedicate to Rocky to meet his daily needs.
Rocky is highly anxious about his mother’s illness and will often pace, cry, or yell when
he feels overwhelmed. Gala does not feel equipped to help her brother effectively. She
loves him but feels he needs more. She also is concerned that her mother is getting
older and—now that she requires more assistance due to her medical conditions—she
will not be able to support Rocky. She has come to the regional center for assistance.
Rocky said he is scared and wants to go home.

All additional information has been received through intake, including his financial status
with social security, and his benefits. Martine has been his representative payee and
Gala has been managing finances for the time being. Confidentiality has been

communication by using basic American sign language. Rocky has always been happy
and helpful. He is kind-hearted and loves to laugh.

His father died when he was 31 and his mother, Martine, has been his caregiver his
entire life. Before his father’s death, Rocky and his father enjoyed fishing and hiking
near the lake. Rocky and his father would swim at the YMCA every Saturday and often
