In your Journal summarize personal and professional achievements and accomplishments that you have completed throughout the baccalaureate nursing program at EC that refer to EPSLO 4: Incorporate nursing’s professional core values, standards of practice, guidelines, commitment to excellence, and lifelong learning as a basis for professional nursing practice.

Attach a minimum of at least two examples of your work that supports EPSLO 4 noted above. Save your Journal entry as your Self-Appraisal for each module in your word document. The professor will request that you submit your journal entries for informal feedback at the end of each module. A Title page of this assignment with a summary of accomplishments (no more than 2 pages in length) is required with attached files as supportive evidence for each EPSLO. A minimum of one or two examples is required to support each EPSLO.

Assignment 1: Practicum – Week 5 Journal Entry (Due in Week 7)

Learning Objectives

Students will:

• Develop effective documentation skills for family therapy sessions *
• Develop diagnoses for clients receiving family psychotherapy *
• Evaluate the efficacy of solution-focused therapy and cognitive behavioral therapy for families *
• Analyze legal and ethical implications of counseling clients with psychiatric disorders *

Select two clients, you observed or counseled this week during a family therapy session. Note: The two clients you select must have attended the same family session. Do not select the same family you selected for Week 2.

Then, address in your Practicum Journal the following:

• Using the Group Therapy Progress Note in this week’s Learning Resources, document the family session.
• Describe each client (without violating HIPAA regulations) and identify any pertinent history or medical information, including prescribed medications.
• Using the DSM-5, explain and justify your diagnosis for each client.
• Explain whether solution-focused or cognitive behavioral therapy would be more effective with this family. Include expected outcomes based on these therapeutic approaches.
• Explain any legal and/or ethical implications related to counseling each client.
• Support your approach with evidence-based literature.

 Pick one of the following diseases: Scabies, Dermatitis, Influenza, or Pneumonia.

Must use the sample template for your soap note.
 

Follow the MRU Soap Note Rubric as a guide

Use APA format and must include minimum of 2 Scholarly Citations.

Soap notes will be uploaded to Moodle and put through TURN-It-In (anti-Plagiarism program)

Turn it in Score must be less than 20% or will not be accepted for credit. Final submission will be accepted if less than 20%. Copy paste from websites or textbooks will not be accepted or tolerated.

The use of tempates is ok with regards of Turn it in, but the Patient History, CC, HPI, The Assessment and Plan should be of your own work and individualized to your made up patient.  

 Using the information discussed in class for how to create an outline, please read the attached document titled “Is the American Dream Over?” and create an outline of it.
