  Note: The assessments in this course are sequenced in such a way as to help you build specific skills that you will use throughout your program. Complete the assessments in the order in which they are presented.
  For your final capstone project submission you will synthesize the work you completed in the previous four assessments. Please make sure that you have made relevant revisions as suggested by your instructor, as well as relevant additions that you uncovered during your practicum experience. The only brand-new content that you will need to create for this assessment is an Abstract and an Introduction.
  This final submission will be graded using the seven program outcomes (POs) for the Master’s of Science in Nursing program. As a reminder they are:
  1. Lead organizational change to improve the experience of care, population health, and professional work life while decreasing cost of care.
  2. Evaluate the best available evidence for use in clinical and organizational decision making.
  3. Apply quality improvement methods to impact patient, population, and systems outcomes.
  4. Design patient- and population-centered care to improve health outcomes.
  5. Integrate interprofessional care to improve safety and quality and to decrease cost of care.
  6. Evaluate the ability of existing and emerging information, communication, and health care technologies to improve safety and quality and to decrease cost of care.
  7. Defend health policy that improves the experience of care, population health, and professional work life while decreasing cost of care.
  8. In addition, you will be assessed on how well you incorporated the feedback you received from your instructor on your previous work in this course via the following criterion:

(Note: The bullet points in the outline correspond to the grading criteria from your previous assessments. It may be worth putting in some extra revisions on the material related to criteria on which you did not previously score as well as you would have liked. You may also wish to read the Guiding Questions: Final Project Submission document to better understand how each aspect of your submission will be assessed.) It is important to remember that if you do a quality job addressing the points below, you will meet all of the program outcomes. The alignment is provided for transparency, but do not become preoccupied with how each point will feed into the scoring guide.

For this assignment you will prepare a 10-12-slide PowerPoint presentation on teaching techniques for diverse learners.

Presentation Criteria:

Prepare a plan for designing and delivering instruction that addresses both the needs of the selected population as well as other diverse students.

1. Select a population that represents a type of diversity, which may include ethnic, learning styles, age, or cognitive ability. The selected population may be nursing students, staff members, or patients.
2. Explain how your selected population’s characteristic impacts their learning styles.
3. Provide three teaching strategies an educator could use with that particular group of learners.
4. Include a rationale for each strategy as to why it would be effective.
5. Use at least three scholarly, peer-reviewed resources less than 5 years old in addition to course materials.

Presentation Requirements:

Online students are required to provide comprehensive speaker notes.

Students in traditional classes are not required to include speakers’ notes, but will present their work in class and be evaluated on professional teaching style.

While APA format is not required for the body of this assignment, solid academic writing is expected and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.

Creating a Plan of Care

Utilizing the information, you have gathered over the last four weeks regarding the specific illness group you identified, this week, you will create a plan of care for your chronic illness group.

In a Microsoft Word document of 5-6 pages formatted in APA 7 style, (this page requirement includes the holistic care plan). Include the following in your plan:

• Provide a brief introduction describing the chronically ill group you selected and rationale for selecting this illness and the participants.
  • Clearly identify the Healthy People 2020 topic chosen and why this topic was selected.
• Develop a holistic plan of care including patient, family and friends acceptance      of the diagnosis, coping and impact on plan of care.
• Summarize the information gathered in each week (Weeks 1–4) over 2 to 3 pages. This is not to be copied and pasted from previous assignments.
• Create a care plan for your chronic illness group organized using the following headings:
  • Nursing Diagnoses (at least 3 related to topic and interview results)
  • Assessment Data (objective and subjective)
  • Interview Results
  • Desired Outcomes
  • Evaluation Criteria
  • Actions and Interventions
  • Evaluation of Patient Outcomes
• Identify strategies for the family or caregiver in the care plan and provide your rationale on how they will work.

Support your responses with examples and information from library resources, textbook and lectures.

On a separate references page, cite all sources using APA 7 format.

Please note that the title and reference pages should not be included in the total page count of your paper.
