Hi, I’m looking for help on a quick assignment. The intervention I chose to implement is a “self-management diabetic program“. 1. Evidence-Based Practice Paper – Each student will select an intervention to implement in a nursing practice setting. The intervention must be supported by best practices or empirical research, as noted in the literature. You must cite at least three articles that illustrate this type of support. Briefly (3pages) describe the preparation, design and evaluation process you would use to facilitate success. Feel free to create tables to display some of the information below, such as organizational values with concise definitions and design elements with short explanatory phrases.
