JC, an at-risk 86-year-old Asian male is physically and financially dependent on his daughter, a single mother who has little time or money for her father’s health needs. Stage II melanoma

 

Mr. B, a 40-year-old avid long-distance runner previously in good health, presented to his primary provider for a yearly physical examination, during which a suspicious-looking mole was noticed on the back of his left arm, just proximal to the elbow. He reported that he has had that mole for several years, but thinks that it may have gotten larger over the past two years. Mr. B reported that he has noticed itchiness in the area of this mole over the past few weeks. He had multiple other moles on his back, arms, and legs, none of which looked suspicious. Upon further questioning, Mr. B reported that his aunt died in her late forties of skin cancer, but he knew no other details about her illness. The patient is a computer programmer who spends most of the work week indoors. On weekends, however, he typically goes for a 5-mile run and spends much of his afternoons gardening. He has a light complexion, blonde hair, and reports that he sunburns easily but uses protective sunscreen only sporadically.

Physical exam revealed: Head, neck, thorax, and abdominal exams were normal, with the exception of a hard, enlarged, non-tender mass felt in the left axillary region. In addition, a 1.6 x 2.8 cm mole was noted on the dorsal upper left arm. The lesion had an appearance suggestive of a melanoma. It was surgically excised with 3 mm margins using a local anesthetic and sent to the pathology laboratory for histologic analysis. The biopsy came back Stage II melanoma.

DERMATOLOGY CASE STUDY

Chief complaint:  “ My right great toe has been hurting for about 2 months and now it’s itchy, swollen and yellow. I can’t wear closed shoes and I was fine until I started going to the gym”.

HPI: E.D a 38 -year-old Caucasian female presents to the clinic with complaint of pain, itching, inflammation, and “yellow” right great toe. She noticed that the toe was moderately itching after she took a shower at the gym. She did not pay much attention. About two weeks after the itching became intense and she applied Benadryl cream with only some relief. She continued going to the gym and noticed that the itching got worse and her toe nail started to change color. She also indicated that the toe got swollen, painful and turned completely yellow 2 weeks ago. She applied lotrimin AF cream and it did not help relief her symptoms. She has not tried other remedies. Denies associated symptoms of fever and chills. 

PMH:

Diabetes Mellitus, type 2.

Surgeries: None

Allergies: Augmentin

Medication: Metformin 500mg PO BID.

Vaccination History: Immunization is up to date and she received her flu shot this year.

Social history:

College graduate married and no children. She drinks 1 glass of red wine every night with dinner. She is a former smoker and quit 6 years ago.

Family history:

Both parents are alive. Father has history of DM type 2, Tinea Pedis. mother alive and has history of atopic dermatitis, HTN.

ROS:

Constitutional: Negative for fever. Negative for chills.

Respiratory: No Shortness of breath. No Orthopnea

Cardiovascular: Regular rhythm.

Skin: Right great toe swollen, itchy, painful and discolored.

Psychiatric: No anxiety. No depression.

Physical examination:

Vital Signs

Height: 5 feet 5 inches Weight: 140 pounds BMI: 31 obesity, BP 130/70 T 98.0, P 88 R 22, non-labored

HEENT: Normocephalic/Atraumatic, Bilateral cataracts; PERRL, EOMI; No teeth loss seen. Gums no redness.

NECK: Neck supple, no palpable masses, no lymphadenopathy, no thyroid enlargement.

LUNGS: No Crackles. Lungs clear bilaterally. Equal breath sounds. Symmetrical respiration. No respiratory distress.

HEART: Normal S1 with S2 during expiration. Pulses are 2+ in upper extremities. 1+ pitting edema ankle bilaterally.

ABDOMEN: No abdominal distention. Nontender. Bowel sounds + x 4 quadrants. No organomegaly. Normal contour; No palpable masses.

GENITOURINARY: No CVA tenderness bilaterally. GU exam deferred.

MUSCULOSKELETAL: Slow gait but steady. No Kyphosis.

SKIN: Right great toe with yellow-brown discoloration in the proximal nail plate. Marked periungual inflammation. + dryness. No pus. No neuro deficit.

PSYCH: Normal affect. Cooperative.

Labs: Hgb 13.2, Hct 38%, K+ 4.2, Na+138, Cholesterol 225, Triglycerides 187, HDL 37, LDL 190, TSH 3.7, glucose 98.

Primary Diagnosis: Proximal subungual onychomycosis

Differential Diagnosis:  Irritant Contact Dermatitis, Lichen Planus, Nail Psoriasis

Special Lab: Fungal culture confirms fungal infection.

Delirium  is an acute intermittent neuropsychiatric condition generally  reversible but serious condition that occurs in all age groups but high  risk in the elderly(Gabbard, 2014).  It ultimately represents the decompensation of brain function as a  result of one or more pathophysiological processes(Fong, 2009). Delirium  is linked with a variety of negative effects, leading to extended  hospital stay, institutional care demands, poor functioning and high  medical costs(Fong, 2009).Due to these negative consequences it is essential to prevent, detect and treat delirium as early as possible.

Diagnostic criteria

Accordint to the DSM-5, diagnostic criteria for delirium is :

A.  A disturbance in attention (i.e., reduced ability to direct, focus,  sustain, and shift attention) and awareness (reduced orientation to the  environment) (American Psychiatric Association, 2013).

B.The  disturbance develops over a short period of time (usually hours to a  few days), represents a change from baseline attention and awareness,  and tends to fluctuate in severity during the course of a day(American  Psychiatric Association, 2013).

C.An  additional disturbance in cognition (e.g., memory deficit,  disorientation, language, visuospatial ability, or perception) (American  Psychiatric Association, 2013).

D.The  disturbances in Criteria A and C are not better explained by another  preexisting, established, or evolving neurocognitive disorder and do not  occur in the context of a severely reduced level of arousal, such as  coma(American Psychiatric Association, 2013).

E.There  is evidence from the history, physical examination, or laboratory  findings that the disturbance is a direct physiological consequence of  another medical condition, substance intoxication or withdrawal( due to a  drug of abuse or to a medication), or exposure to a toxin, or is due to  multiple etiologies(American Psychiatric Association, 2013).

 Psychotherapy and Psychopharmacologic Treatment 

Delirium  is a medical emergency that involves proper diagnosis, recognition of  the cause and management of symptoms.The first-line treatment is to  identify and address pre- disposing factors, provide supportive care,  and manage symptoms through behavioral strategies(Fong, 2009). Treatment  is geared toward symptom management and patient centered.Medications  are used to control agitation and reverse hyperactive delirium (Fong,  2009).Medication treatment includes antipsychotics such as haloperidol  and chlorpromazine.The goals of these pharmacological agents is to  reduce agitation and address other symptoms associated with hyperactive  deliria (Gabbard, 2014).Non-pharmacological  treatment of delirium stresses the removal or reduction of medical,  sensory and environmental factors that can lead to delirium,such as  modifying sensory deficits in order to improve engagement with the  environment, particularly when coupled with sufficient intellectual  stimulation during wake-up hours (Gabbard, 2014).Lastly,  immoboizing devices can be used but should be reserved when harm to  self and others is immenent, and should be short and behaviroal targeted(Gabbard, 2014).

Benefits and Risks of Therapy

When choosing any drug, attention should always be given to the possible advantages,

dangers  and pressures of each medicine, as well as to the patient and family  care priorities (Grover, 2018).The use of antipsychotics have many  adverse effects that include sedation, wiegth gain, changes in appetite,  cardiac effects(such as QT prolongation), neaurological effects, falls  and long term need for use that is inappropriate but in many cases it is  the best course of action.According to Fong (2009), studies some  beneficial roles of antipsychotics such as lowering severity of delirum,  reducing harm to self and others are the main reasons why these agents  are used.However, Grover (2018), emphasizes that antipsychotics for the  treatment of delirium in adult inpatients did not improve patient  outcomes, with little evidence of neurologic harms but a tendency for  more frequent potentially harmful cardiac effects.
