Sleep/wake disorders

N6630

Another busy one on tap with a short answer assessment and decision tree.  The therapeutic focus will be on sleep/wake disorders.

Short Answer Assessment:

Address the following Short Answer prompts for your Assignment. Be sure to include references to the Learning Resources for this week.  I would also encourage you to use External Resources.

  1. In 3 or 4 sentences, explain the appropriate drug therapy for      a patient who presents with MDD and a history of alcohol abuse. Which      drugs are contraindicated, if any, and why? Be specific. What is the timeframe      that the patient should see resolution of symptoms?
  2. List 4 predictors of late onset generalized anxiety disorder.
  3. List 4 potential neurobiology causes of psychotic major      depression.
  4. An episode of major depression is defined as a period of time      lasting at least 2 weeks. List at least 5 symptoms required for the      episode to occur. Be specific.
  5. List 3 classes of drugs, with a corresponding example for      each class, that precipitate insomnia. Be specific.

Decision Tree:   Insomnia case

I want you to answer the questions given to you (decision points one, two, and three) before you click on the option. The answers will be based on your decisions made and patient outcomes during the decision tree. I am looking for an essay that is long enough to cover the topic BUT short enough to keep my interest.  The course page suggests writing 1 page per decision – my opinion is that it will be very difficult to justify your treatment decisions and provide scientific evidence in 1 page (especially for decision #1). I do not need you to tell me about the patient or the treatment options available to you – I am very familiar with the cases.  Your introductory page should be an overview of the disease state you are treating along with a purpose statement for the assignment.  Remember this is a Pharmacology class that incorporates Pharmacotherapy and not a class on diagnosing disease. I want you to tell me why you selected an option (why is it the best option- using clinically relevant and patient specific data) AND why you did not choose the other options (with clinically relevant and patient specific data).

Introduction to the case (1 page)

  • Briefly explain and summarize the disease state you are      treating this Assignment. Be sure to include the specific patient factors      that may impact your decision making when prescribing medication for this      patient.

Decision #1

  • Which decision did you select?

·

  • Why did you select this decision? Be specific and support        your response with clinically relevant and patient-specific resources,        including the primary literature. Again, provide STRONG scientific evidence.  Clinical        studies or treatment guidelines are a good place to start!
  • Why did you not select the other two options provided in the      exercise? Be specific and support your response with clinically relevant      and patient-specific resources, including the primary literature.
  • What were you hoping to achieve by making this decision?      Support your response with evidence and references to the Learning      Resources (including the primary literature).
  • Explain how ethical considerations may impact your treatment      plan and communication with patients. Be specific and provide examples.

Decision #2 (1 page)

·

Decision #3 (1 page)

  • Why did you select this decision? Be specific and support      your response with clinically relevant and patient-specific resources,      including the primary literature.

·

  • Why did you not select the other two options provided in the        exercise? Be specific and support your response with clinically relevant        and patient-specific resources, including the primary literature. Again, provide STRONG scientific evidence.  Clinical        studies or treatment guidelines are a good place to start!
  • What were you hoping to achieve by making this decision?      Support your response with evidence and references to the Learning      Resources (including the primary literature).
  • Explain how ethical considerations may impact your treatment      plan and communication with patients. Be specific and provide examples.

Conclusion (1 page)

  • Summarize your recommendations on the treatment options you      selected for this patient. Be sure to justify your recommendations and      support your response with clinically relevant and patient-specific      resources, including the primary literature.

After writing up your rationale at each decision point, I would ask yourself the following questions:

· Have I provided clinical data from a meta-analysis, case report or clinical trial to support the drug I picked being safe, efficacious and the best choice for this patient?

· Have I provided clinical data, etc. to support a clear rationale as to why the other treatment options are NOT optimal?

· Is the focus of my discussion on mechanism of action and receptors/neurotransmitters that the drug acts on? If the answer is YES, you should consider doing additional research to address the above two questions

Also include how ethical considerations might impact your treatment plan and communication with clients.

The rubric, as I interpret it, suggests 5 references cited with every assignment for full credit on this portion (20 points).  References used for your introductory paragraph, ethical considerations or conclusion do not count towards the 5 references required.  As a general rule of thumb, I would encourage you to reference AT LEAST two sources (not including the textbook) for each decision point – this will result in 6 references total for your clinical decision making.

Complete and submit your Comprehensive Psychiatric Evaluation, including your differential diagnosis and critical-thinking process to formulate primary diagnosis.

Incorporate the following into your responses in the template:

  • Subjective: What      details did the patient provide regarding their chief complaint and      symptomology to derive your differential diagnosis? What is the duration      and severity of their symptoms? How are their symptoms impacting their      functioning in life?
  • Objective: What      observations did you make during the psychiatric assessment?
  • Assessment: Discuss      the patient’s mental status examination results. What were your      differential diagnoses? Provide a minimum of three possible diagnoses with      supporting evidence, listed in order from highest priority to lowest      priority. Compare the DSM-5 diagnostic criteria for each differential      diagnosis and explain what DSM-5 criteria rules out the differential      diagnosis to find an accurate diagnosis. Explain the critical-thinking      process that led you to the primary diagnosis you selected. Include      pertinent positives and pertinent negatives for the specific patient case.
  • Reflection notes: What      would you do differently with this client if you could conduct the session      over? Also include in your reflection a discussion related to      legal/ethical considerations (demonstrate critical thinking beyond      confidentiality and consent for treatment!), health promotion and disease      prevention taking into consideration patient factors (such as age, ethnic      group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural      background, etc.).

Assignment: Asthma and Stepwise Management

Assignment: Asthma and Stepwise Management

Asthma is a respiratory disorder that affects children and adults. Advanced practice nurses often provide treatment to patients with these disorders. Sometimes patients require immediate treatment, making it essential that you recognize and distinguish minor asthma symptoms from serious, life-threatening ones. Since symptoms and attacks are often induced by a trigger, advanced practice nurses must also help patients identify their triggers and recommend appropriate management options. Like many other disorders, there are various approaches to treating and managing care for asthmatic patients depending on individual patient factors.

One method that supports the clinical decision making of drug therapy plans for asthmatic patients is the stepwise approach, which you explore in this Assignment.

To Prepare
  • Reflect on drugs used to treat asthmatic patients, including long-term control and quick relief treatment options for patients. Think about the impact these drugs might have on patients, including adults and children.
  • Consider how you might apply the stepwise approach to address the health needs of a patient in your practice.
  • Reflect on how stepwise management assists health care providers and patients in gaining and maintaining control of the disease.
By Day 7 of Week 3

Create a 5- to 6-slide PowerPoint presentation that can be used in a staff development meeting on presenting different approaches for implementing the stepwise approach for asthma treatment. Be sure to address the following:

  • Describe long-term control and quick relief treatment options for the asthma patient from your practice as well as the impact these drugs might have on your patient.
  • Explain the stepwise approach to asthma treatment and management for your patient.
  • Explain how stepwise management assists health care providers and patients in gaining and maintaining control of the disease. Be specific.

When explaining the structure of HIV/AIDS to the patient, always ensure that they should understand it and do not  use much of jargon .The main goal of the meeting should be to explain the concept of HIV AIDS to the client. Therefore, use a simple accurate and concise approach.

It is Human Immunodeficiency Virus which mean a virus that can cause deficiencies in human immune system. The immune system is our army against the diseases. If HIV is not treated it leads to AIDS- Acquired immune deficiency syndrome (immune less state) – because of HIV the patient acquires these immune deficiencies and become susceptible to all sorts of infections. It takes months to years to reach from HIV to AIDS.

HIV attack on our CD4 cells and T helper cells- the cells are warriors in our immune cell and over the time take over them. It overtakes the immune cell machinery and use it against the body immune cells.

HIV is round shaped virus with two single strands of RNA.RNA is used to carry genetic information. HIV virus enters the cell copies its genetic information. For HIV to be able to enter the cell the cell must have CD4 receptor. With passage of time, it keeps killing immune cells and fail the immune system of the human. When CD4 cell count goes below 200 cell/mm3.A healthy human has CD4 count from 500-1000 cells/mm3. The viral load is the number of HIV particles in a milliliter (mL) of blood. Higher the viral load means more copies of HIV in blood and more infection. Antiretroviral therapy can make the viral load low and reduce the HIV effect and delay or reduce chances of AIDS.
