One critical element of any physical exam is the ability of the examiner to put the patient at ease. By putting the patient at ease, nurses are more likely to glean quality, meaningful information that will help the patient get the best care possible. When someone feels safe, listened to, and cared about, exams often go more smoothly. This is especially true when dealing with issues concerning breasts, genitals, prostates, and rectums, which are subjects that many patients find difficult to talk about. As a result, it is important to gain a firm understanding of how to gain vital information and perform the necessary assessment techniques in as non-invasive a manner as possible.

For this week, you explore how to assess problems with the breasts, genitalia, rectum, and prostate.

         In this chapter, the authors explain how to conduct an examination of      female genitalia. The chapter also describes the form and function of      female genitalia.

  • Chapter 20, “Male Genitalia”

         The authors explain the biology of the penis, testicles, epididymides, scrotum,      prostate gland, and seminal vesicles. Additionally, the chapter explains      how to perform an exam of these areas.

  • Chapter 21, “Anus, Rectum, and Prostate”

         This chapter focuses on performing an exam of the anus, rectum, and      prostate. The authors also explain the anatomy and physiology of the anus,      rectum, and prostate.

Chapter 5, “Amenorrhea”
Amenorrhea, or the absence of menstruation, is the focus of this chapter. The authors include key questions to ask patients when taking histories and explain what to look for in the physical exam.

Chapter 6, “Breast Lumps and Nipple Discharge”
This chapter focuses on the important topic of breast lumps and nipple discharge. Because breast cancer is the most common type of cancer in women, it is important to get an accurate diagnosis. Information in the chapter includes key questions to ask and what to look for in the physical exam.

Chapter 7, “Breast Pain”
Determining the cause of breast pain can be difficult. This chapter examines how to determine the likely cause of the pain through diagnostic tests, physical examination, and careful analysis of a patient’s health history.

Chapter 27, “Penile Discharge”
The focus of this chapter is on how to diagnose the causes of penile discharge. The authors include specific questions to ask when gathering a patient’s history to narrow down the likely diagnosis. They also give advice on performing a focused physical exam.

Chapter 36, “Vaginal Bleeding”
In this chapter, the causes of vaginal bleeding are explored. The authors focus on symptoms outside the regular menstrual cycle. • CC: “I have bumps on my bottom that I want to have checked out.”

• HPI: AB, a 21-year-old WF college student reports to your clinic with external bumps on her genital area. She states the bumps are painless and feel rough. She states she is sexually active and has had more than one partner during the past year. Her initial sexual contact occurred at age 18. She reports no abnormal vaginal discharge. She is unsure how long the bumps have been there but noticed them about a week ago. Her last Pap smear exam was 3 years ago, and no dysplasia was found; the exam results were normal. She reports one sexually transmitted infection (chlamydia) about 2 years ago. She completed the treatment for chlamydia as prescribed.

• PMH: Asthma

• Medications: Symbicort 160/4.5mcg

• Allergies: NKDA

• FH: No hx of breast or cervical cancer, Father hx HTN, Mother hx HTN, GERD

• Social: Denies tobacco use; occasional etoh, married, 3 children (1 girl, 2 boys)

Objective:

• VS: Temp 98.6; BP 120/86; RR 16; P 92; HT 5’10”; WT 169lbs

• Heart: RRR, no murmurs

• Lungs: CTA, chest wall symmetrical

  • Tom Joserine, a nurse manager of a 36-bed orthopedic unit, has decided that he is too busy to complete all the work that needs to be done. Nurse Joserine has recently read an article on delegation and has decided that delegating tasks to others is an excellent way to accomplish the mound of work that he has to complete. He believes that there are two tasks that take most of his time, disciplinary problems and quality improvement activities. Nurse Joserine calls the nursing coordinator, Janet Jeleen, into the office and informs her that she will now be responsible for all evaluations and handling of incident reports. Privacy is essential in all parts that involve communication, storage, and processing of data. In this case, hospitals should have a high level of confidentiality when it comes to patient’s health information records(English & Lewis2016). This discussion post will respond to how a health facility handled Jean’s health information. According to the right of privacy, a patient has the right not to have their personal information disclosed. Dr. Grant and the billing office violated the right to privacy of Jenna (Donna, Yilmaz, & Sarah, 2019). Jenna had told the billing office that she would not want the bill sent to her mother’s insurance instead to a college address, which the billing office agreed to do so. Later, after the software upgrade, the billing office sent the bill to the mother’s insurance, which violated Jenna’s right to privacy. On the other side, Dr. Grant disclosed that Jenna had visited the hospital, which she was not supposed to. Mrs. Peterson did not have the right to ask Dr. Grant why Jenna had visited some weeks ago; instead, she should have waited to hear from Jenna why she had gone there.

Putting into consideration how the incident transpired, Jenna has the right to sue for a confidentiality breach. In this case, she would sue them for breach of patient privacy, HIPAA noncompliance, and causing emotional distress. Mrs. Peterson should also sue the billing office for sending her a bill for a visit she did not know. For the organizations to prevent future confidentiality violations, they should use the following risk recommendations. First, it should give patient confidentiality a high priority whereby all parties in that organization should take responsibility. Second, the organization should have a dedicated privacy officer who would help the organization develop consequences for employees who breach patient confidentiality. Lastly, the organization should require that every staff sign a confidentiality agreement when first employed.

For this weeks discussion, I decided to talk about dance. According to Martin (2018), dance may be defined as moving bodies shaping space. He also mentions that dance is rhythmic, unfolding in time, and thus has a common ground with music (Martin, 2018). There are several different type of dance forms that have emerged throughout the years. The movement I chose to discuss is the Ballet dance movement. The origins of ballet usually are traced to the early seventeenth century, when dancers performed interludes between scenes of an opera —eventually ballet was performed independently (Matin, 2018). It is said that classical ballet originated in Renaissance Italy. Ballet is an artistic dance form performed to music using precise and highly formalized set steps and gestures (Oxford, 2020). It is often characterized by light, graceful, fluid movements with the use of pointe shoes (Oxford, 2020). Ballets generally have a pretext, a narrative line or story around which the ballet is built (Martin, 2018). It is our own perception of the movements that allows us to understand the story within the ballet performance.

The dance piece I chose to examine is Swan LakeSwan Lake was composed by Pyotr Tchaikovsky and choreographed by Leon Ivanov and Marius Petipa. It was first performed in 1891. According to artist, Rudolf Nureyev, he mentioned “At the end of Swan Lake, when she left the stage in her great white tutu, I would have followed her to the end of the world”. He was referencing his ballet partner Margot Fonteyn. Critic Jonathan Reeves mentioned, “when Margot Fonteyn was onstage, you couldn’t tear your eyes away from her” (Foster, 2016). In reading through many critics comments, everyone seemed to absolutely idolize Margot Fonteyn. They believed she made the ballet performance in Swan Lake. Others believed that Swan Lake displays a tentative love blossoming between the swan queen and her prince (Foster, 2016).
