You reviewed each case, selected one case for further research, and created draft evidence-based concept map to illustrate an approach to individualized care for the patient. In this assessment, you will build upon and refine your draft concept map and develop a supporting narrative.
    Create your concept map and narrative as separate documents. Be sure to note the areas where you need to include your evidence-based support and where you need to make clear your strategies for communicating information to the patient and the patient’s family.
    Note: Many organizations use the spider style of concept maps (see the Taylor & Littleton-Kearney article for an example). Also, if a specific style of concept map is used in your current care setting, you may use it in this assessment.
    Create your concept map and narrative as separate documents. Be sure to note the areas where you need to include your evidence-based support and where you need to make clear your strategies for communicating information to the patient and the patient’s family.

Module 02 Assignment – Interpersonal Process Recording (IPR)

Purpose

This assignment will help prepare you for conversations with clients with a mental health condition.

Course Competency

Describe the foundations of mental health nursing.

Transferable Skill

Communication: Displaying capability in writing, reading, and oral communication; understanding of non-verbal language.

Instructions

It will be carried out with two students working together. This is a “role play exercise,” in which one student is the student nurse, and the other will role play as a client. In this assignment, the nursing student will be demonstrating the therapeutic nurse-client relationship and analyzing the therapeutic and nontherapeutic techniques used. You will take turns in the roles.

This is not about always having therapeutic responses. It is about learning from practice and review. It is expected that you will think of other ways to respond or interact with the client that may be more therapeutic after the interaction is terminated. After the conversation is over, you will complete in IPR form.

The client roles are:

  1. Mr. Jones is a 69-year-old retired engineer. He was admitted to the in-patient psychiatric unit the previous day. His daughter had called the police when he locked himself in his bathroom and refused to come out. She thought he was suicidal. He was brought to the hospital by the police and was admitted on involuntary status. He has been reticent since admission but told his daughter he had no reason to live since his wife died.
    Diagnosis: MajorDepressive Disorder
    Nursing Diagnosis: Risk for suicide.
  2. Mrs. Alvarez is a 34-year-old female who has lived in this country for the past 10 yrs. She is a stay at home mom with three small children. Her husband works two jobs to support the family. Lately, she has been extremely anxious and fears that her children will become ill or injured. This seems to be an unrealistic concern, but she has been unable to sleep well and has lost 15 lbs. in the past month. She is a voluntary admission and states she knows she needs help.
    Diagnosis: Generalized Anxiety Disorder
    Nursing Diagnosis: Ineffective coping

You will select the role you wish as a client and take turns switching roles. After the conversation, you will document the interactions using the form provided and following the guidelines instructions for each column.

  • The first column is for you, the nursing student’s, comments/ questions, and responses. You may want to include the introduction process. It is understandable if the initial conversation is more superficial to establish rapport. You may not want to include all of this but move on to the more focused interaction. Your nonverbal communication is included here.
  • Then, the conversation will move to a focus on the client’s feelings, concerns, issues as he/she has identified them. After the introduction, it may be beneficial to establish with the client a short- term goal for the interaction.
  • The second column is for the client’s comments, questions, responses. Also, the nonverbal reactions of the client are included in this column.
  • The middle column is for you to write what you are thinking or feeling.
  • The last two columns are for you to document the technique that you used in that portion of the conversation. If it is a nontherapeutic response, you are to write the response and think of how you could revise to more therapeutic response. You can change any response if you think of how you could have responded differently but, identify and change all nontherapeutic responses.

Remember, this is an assignment that will assist you in all communication with others.

Explanation of the Rubric Criteria for Interpersonal Process Recording (IPR)

  • Communication (20 verbal entries, ten nonverbal, ten techniques)
  • Criteria 1 Nursing Interaction (verbal and nonverbal)
    Nursing statements and questions demonstrate a positive, caring approach and show insight into the client needs. These interchanges will include the introduction process, but not the more superficial interchanges. Nonverbal communication techniques are also recognized. Thoughts and feelings are identified and documented.
  • Criteria 2
    The client verbal statements and questions are documented. Nonverbal communication is also observed and interpreted. You may also identify defense mechanism(s) used by the client.
  • Criteria 3
    All communication techniques used are identified (using titles previously reviewed).
  • Criteria 4
    All non-therapeutic techniques to be identified and changed to a therapeutic method. If in the review of the interaction, a more appropriate or therapeutic approach (verbal or nonverbal) is identified, document how this response/ statement would be changed

Stakeholders are important parts of a successful business or to create a successful project. Without stakeholders, there would be a lack of financial and practical support. Internal stakeholders are within a facility that provides interest in the company’s success and they work within the company to reach success. Internal stakeholders are a key component in implementing by providing their knowledge and leadership support for a proposed change (M.J, 2019). These stakeholders are vested and interested in their company or organization. The internal stakeholders are nurses, physicians, administrators and other health care providers. They commit their time and efforts to improve the company or organization. External stakeholders “include patients, suppliers and financial organizations including government, health care insurance providers among others” (M.J, 2019). The support of external stakeholders is important in having a successful organization. External stakeholders provide finances to support potential changes including structures, training and recruitment. 

     Stakeholders are the answer to implementing change because without their support, there would be a lack of resources, skills and knowledge required for implementing a new process or change (M.J, 2019). I will secure support from external and internal stakeholders by creating awareness, building a trusting relationship and be clear about the goals of my project of workplace violence and advocating for the staff that has been harmed. By utilizing physicians, residents and emergency room health care staff and educating them on the facts and lack of resources, capturing their attention and gaining their interest. Gaining their interest and support can help me create a successful project that will help keep staff safe and unharmed.

Technology is essential to the implementation of the projects. Health Information Technology (Health IT) involves the processing, storage and exchange of health information electronically following privacy and security protocols (HHS.gov, 2020). It improves communication and collaboration with providers in a different health care setting. This form of technology helps to improve the quality of health care, prevent medical errors, reduce health care cost, increase administrative efficiencies, decrease paperwork, and expand access to affordable health care (HHS.gov, 2020). A study shows that only 12% to 34% of discharge summaries reached outpatient care teams by the time the patient saw a physician (Mansukhani et al., 2015). Considering the benefits stated above, this is a perfect technology to help implement my capstone project change proposal, which is improving the transition of care. Using this technology, providers can coordinate important patient information to other health care settings, which promote safe and effective continuity of care that led to positive patient outcomes.

  1. Think of 2-3 examples of potential ways health communication interventions can address such issues/needs among Bonnie and her peers or key influentials/stakeholders. For example, health communication programs can: 1) “Engage Bonnie, her peers, and her community in discussing their perceptions and opinions about the pros and cons of immunizations as well as any barriers, social norms, or other socially determined factors that may influence their decisions; 2) “Encourage peer-to-peer support by establishing venues, events, and social media-based forums where new mothers can discuss immunization and be supported on their decisions; 3) Advocate for policies, mandates, and other regulations that would increase ease of access to timely immunization, convey the importance of immunization in child and community protection, and also be inclusive of vulnerable and underserved populations as it may relate to their specific needs and concerns” (select examples from Schiavo, 2013, Introduction). Please use different examples from the ones listed here or in your textbook.
  2. What kinds of research questions would you like to explore to complement the information provided in this case study? What would you ask/would like to find out to complete a potential situation and audience analysis? Please focus only on your two (2) top research questions.
  3. Use the definition/examples of “overall program goal” and “outcome objectives” discussed in your book to develop a preliminary program goal and outcome objectives for a potential health communication program that seeks to engage (a) Bonnie and her peers (US parents in the same age group and with similar profile) and (b) one potential key stakeholder (e.g. health care providers, grandparents, etc,) whom you think may influence Bonnie’s behavior. For the purpose of this exercise, please develop only two outcome objectives (behavioral only) as intended for Bonnie and her peers, and two outcome objectives (behavioral only) for the stakeholder group. Note that this exercise focuses only on behavioral objectives, which is only one of the 3 kinds of outcome objectives (behavioral, social, and organizational) discussed in your book.
