Discussion Question:

This week’s discussion post is on identification of a community health assessment model. Be sure to click the link to review the learning materials to see what an assessment model looks like. Address the questions that are listed in the discussion area of the classroom.

Identify a Community Health Assessment Model. (This document from the CDC lists commonly used community assessment models: https://www.cdc.gov/stltpublichealth/cha/assessment.html ) 

What are some benefits of using a specific model to assess a community? What agencies should be included with the assessment? What is a reasonable time frame in which to complete a community wide assessment? What is the role of the public nurse in implementation of the chosen model?

Your initial posting should be at least 400 words in length and utilize at least one scholarly source other than the textbook.

Increased Body Weight

In recent years, high fructose corn syrup (HFCS) has been blamed for the increased prevalence of overweight and obesity in the U.S. (Bray, Nielsen, & Poplin, 2004). It is present in many foods, including those we would expect to find it in, such as candy, to those unexpected, including salad dressing.

If your last name starts with the letters A-L you are assigned the “pro” side. If your last name starts with the letters M-Z, you are assigned the “con” side. The “pro” side will make a case that HFCS has contributed to the increased body weight of Americans. The “con” side will make a case that HFCS has not contributed to the increased body weight of Americans. Consider why Americans may or may not choose foods with HFCS, including cost, effect on nutritional status, and what other nutrients may be provided in foods that typically contain HFCS.

You must back up your argument with reputable references, which includes scientific journal articles that can be accessed through the South University Online library. Remember that we are discussing this topic to learn from one another’s research and that the side that someone may be defending may not be what they truly believe; please be kind and considerate in your posts.

 Subjective Data 

CHIEF OF COMPLAIN:  “I am here for my annual physical exam and have been having vaginal discharge.”  

HISTORY OF PRESENT ILLNESS:  32-year-old pregnant lesbian – her pregnancy has been without complication thus far. She has been receiving prenatal care from an obstetrician. She received sperm from a local sperm bank. 

DRUG HX:  Current Meds: prenatal vitamins and takes Tylenol over the counter for aches and pains on occasion 

FAMILY HX:  She has a strong family history of diabetes. Gravida 1; Para 0; Abortions 0.
 

REVIEW OF SYSTEM:

GENERAL:  No fatigue, fever, or chills.
 

NECK:   No pain or injury
 

INTERGUMENTARY:  multiple piercings, and tattoos. Old scars related to “cutting”
 

MS/NEURO:  no syncopal episodes or dizziness, no change in memory or thinking patterns; no twitches or abnormal movements.
 

OBJECTIVE DATE:

PE:  B/P 128/76; Pulse 83; RR 16; Temp 99.0; Ht 5,6; wt 128; BMI 20.98
 

HEENT:  Atraumatic, normocephalic, PERRLA, EOMI, conjunctiva and sclera clear; nares patent, nasopharynx clear, good dentition. Piercing in her right nostril and lower lip.
 

LUNGS:  CTA AP&L

CARDIO:  S1S2 without rub or gallop
 

ABD:  benign, normoactive bowel sounds x 4
 

GU: external genitalia intact, no lesions or masses. White copious discharge with an amine odor; no cervical motion tenderness; adnexa intact.
 

EXT.  no cyanosis, clubbing, or edema
 

INTERMENT:  intact without lesions masses or rashes.

NEURO: No obvious deficits and CN grossly intact II-XII

Assignment: Early Onset Schizophrenia

Children and adolescents with schizophrenia have more difficulty functioning in academic or work settings, and significant impairment usually persists into adulthood. They may have speech or language disorders and in some cases borderline intellectual functioning. These individuals are more likely to complete suicide attempts or die from other accidental causes. Schizophrenia is characterized by positive and negative symptoms. Positive symptoms include hallucinations, delusions, and behavior disturbance. Negative symptoms include blunted affect and attention, apathy, and lack of motivation and social interest.

In this Assignment, you compare treatment plans for adults diagnosed with schizophrenia with treatment plans for children and adolescents diagnosed with schizophrenia. You also consider the legal and ethical issues involved in medicating children diagnosed with schizophrenia.

Learning Objectives

Students will:

• Compare      evidence-based treatment plans for adults versus children and adolescents      diagnosed with schizophrenia
• Analyze legal      and ethical issues surrounding the forceful administration of medication      to children diagnosed with schizophrenia
• Analyze the role      of the PMHNP in addressing issues related to the forceful administration      of medication to children diagnosed with schizophrenia

The Assignment (2 pages):

• Compare at least      two evidence-based treatment plans for adults diagnosed with schizophrenia      with evidence-based treatment plans for children and adolescents diagnosed      with schizophrenia.
• Explain the      legal and ethical issues involved with forcing children diagnosed with      schizophrenia to take medication for the disorder and how a PMHNP may      address those issues.
