Post by Erin :  The article I chose to summarize is titled, “Federal, state, and organizational barriers affecting nurse practitioner workforce and practice.” Poghosyan (2018) claims that NPs are the fastest growing workforce between 2013 and 2025 because expected growth is 93%. It is essential that NPs have the ability to deliver primary care to the millions of Americans who desperately need it. However, barriers at the state, federal and organizational levels impede NPs from delivering primary care to those who need it most.

Some examples of barriers to care include Medicare reimbursement rates, federal regulation requires physicians to sign NP homecare orders for validity, certain states restrict NPs from practicing independently, and organizationally NPs are not given the same support from medical assistants that physicians have, which underutilizes an NP’s skill set (Poghosyan, 2018).

I do agree with the author’s claims in this article. NPs are needed in a primary care setting. With barriers at the federal, state and organizational levels, ultimately those barriers are limiting access to quality care and improving patient outcomes. I believe it is up to the individual provider to know their scope of practice, limitations, and refer patients when necessary. With primary care providers being in high demand in rural areas, it is frustrating to see states restrict an NP’s scope of practice (Xue, Ye, Brewer & Spetz, 2016). Many primary care physicians do not want to practice in rural areas for personal and/or financial reasons. I know many NPs that are willing to work in rural areas across the U.S., however, they are limited based on individual state laws that restrict them from opening an NP-run clinic or a private practice to serve those vulnerable populations.

Unfortunately, I live in one of the five worst states to practice as an APN (MidlevelU.com, 2013). Florida has some of the strictest laws in the nation. A collaborating physician is required for an NP to practice, in addition, the method of supervision between a physician and the NP must be in written form. NPs can prescribe Schedules II-V controlled substances under physician supervision and in written protocol as well. NPs can prescribe controlled substances in outpatient and hospital settings when prescribing authorized substances only. A 3-day supply is allowed for schedule II drugs, and children under 18 years of age may not be prescribed psychotropic medications unless the NP is a certified Psychiatric Mental Health Nurse Practitioner. NPs are not allowed to prescribe controlled substances in pain management/relief clinics. However, the collaborating physician may limit these restrictions further if they see fit to do so (Scopeofpracticepolicy.org, 2019).

Post By Jacqueline Reid : The growing population of the aged and ill in our country is increasing, and additional resources are needed to help manage this trend.   For some, living longer comes with chronic conditions and comorbidities.  Advanced Practiced Nurse Practitioners (APNP) are a great resource in helping patients age with better health outcomes.

              According to Heale, James, Wenghofer, and Garceau, (2018) “Traditional family practices have struggled to meet all the healthcare needs of this growing group of patients. In Canada, one response to these challenges is primary healthcare reform, which includes a shift from solo family doctor practices to the development of new models of primary healthcare”.  Canada’s solution to managing this trend is to establish nurse practitioner-led clinics (NPLC) in Ontario. 

           Areas which have primary care access and processes in place to manage the health of a community lead to better health outcomes. This model accommodates areas with reduced access to a primary care provider.  The significance of the role of the NPLC is that these clinics are managed by nurse practitioners who are the primary providers and not a physician.  In this model physicians are consultants (Heale, James, Wenghofer, & Garceau, 2018, p. 554).

           Utilizing an APNP as an NPLC is using the role of an APNP to its full potential.  It is exciting to know the level of trust which has been instilled in these providers in caring for an underserved community, and to not be hindered by limitations within their scope of practice.  This is a win-win situation, and I look forwarded to advances in the role of an APNP such as that of the NPLC to come to fruition in Georgia.

             In comparison to the scope of practice in Georgia, APNP work under the license of a physician.  According toRules and Regulations of the State of Georgia, (2019) an APNP must “adhere to a written nurse protocol which is a written document mutually agreed upon and signed by the nurse and licensed physician which specifies delegated”. This also includes medical acts delegated by the physician to the nurse and provides for immediate consultation with the delegating physician or a physician designated in the absence of the delegating physician”.  This also includes having physician oversight for the ordering and administering of controlled substances, medical treatment, and ordering of diagnostics studies. 

          However, at my clinical rotation as well as several APNP I have spoken with, have experienced autonomy within their practice.  APNP continuing to practice with integrity, and providing the best care for their patients will aid in advancement of our role.  Ultimately, improving patient medical outcomes and quality of life is the goal.

Often the United States health care system is considered treatment for the sick or simply diagnosis and treatment, but it should also prevent patients from getting sick.  It is evidenced based best practice to work toward a health system with a strong focus on preventative health care so that we can move toward a healthier population and reduce health problems in the future. 

The Affordable Care Act (ACA) covers certain health promotion services, but unfortunately not all and some services are only partially covered.  Health preventative services that are not covered or only partially covered may create a financial challenge or barrier for the patient.  For example, ACA covers some preventative birth control and other medications, but it does not cover brand name (Preventive Services Covered by Private Health Plans under the Affordable Care Act, 2015). 

Opportunities that are related to the integration of the ACA includes the ability to offer patients preventative health screening and services at no cost or low cost to them.  This is important because it can help patients who would not normally participate in preventative health measures possibly due to social determinants of Healthcare, to start participating in preventative health services and thus prevent future health issues (Daniel, Bornstein, & Kane, 2018).   Another opportunity is with the increased mental health issues in our population, the ACA covers many preventative mental health screening and treatment. The ACA also provides the opportunity to expand the knowledge and quality of preventative health services in the community.

About 50% of American adults suffer from at least one chronic condition and government programs like the ACA support research and other federally sponsored programs that can benefit this large part of the population (Leppin, A.L., Schaepe, K., Egginton, J. et al, 2018).

Challenges related to the ACA and preventative health services include billing issues that may cause charges to the patient, including if the reason for the visit is not preventative or if the office visit and preventative service are billed separately (Preventive Services Covered by Private Health Plans under the Affordable Care Act, 2015).  Another potential issue that can cause additional costs is if out-of network providers are used, when in network providers are available plus finding and waiting for an appointment at an in network provider could cause delays in care (Preventive Services Covered by Private Health Plans under the Affordable Care Act, 2015).

In the AANP (2012) Health policy brief: Nurse practitioners and primary care: “advocates say changes in federal and state laws are needed to remove barriers to the provision of primary care by nurse practitioners. By contrast, some physician groups are opposed” (p. 3). What are the professional implications of this statement? What are the practice requirements in your state? Why is it important for advanced practice nurses to remain aware of and engage in health policy?

The implications of this statement is that advanced nurse practitioners can practice at a full primary care level and prescribe medications. Since 2015 twenty-one states have changed the laws and regulations that APNP’s have prescribing rights and full practice rights with extensive training and education. Seventeen states have a reduced practice standard, with twelve states having a restricted practice that the APNP has to provide care under the direct supervision of a Doctor (NCBI, 2015).

In the state of Wisconsin the practice requirements are limited. According to AANP, (2018) Limited practice is “State law requires a career-long regulated collaborative agreement with another health provider in order for the NP to provide patient care, or it limits the setting of one or more elements of NP practice” (pg,1). The NP has to meet the educational requirement and a national certification, then they are eligible for credentialing which allows them to prescribe medications. They must hold a valid RN license before they can become an ANP (Nursing Licensure, 2019).

It is important for the advanced practice nurse to remain aware of the health policy so that they are working up to their ability and training and staying up to date on their education and working under their scope of practice. Healthcare is continuously changing and the providers have to stay up to date to provide the best quality care that they can to all patients.
