WEEK -11

 Madeleine Leininger Transcultural Nursing TheoryAssignment

Discuss the applicability of the theory of culture care diversity and universality to discover nursing knowledge and provide culturally congruent care. Take into consideration the current trends of consumers of health care, cultural diversity factors, and changes in medical and nursing school curricula. The following are examples of trends you may use as a thread to start your discussion:

a. The importance of transcultural nursing knowledge in an increasingly diverse world

b. The growth of lay support groups to provide information and sharing of experiences and support for clients, families, and groups experiencing chronic, terminal, or life-threatening illnesses or treatment modalities from diverse or similar cultures

c. Cultural values, beliefs, health practices, and research knowledge in undergraduate and graduate nursing curricula across the life span

d. Inclusion of alternative or generic care in nursing curricula, such as medicine men, Native American healers, curers, and herbalists in the Southwest and selected substantiated Chinese and Ayurvedic medicine methods shown to be effective for the treatment of both acute and chronic diseases

e. The increased access to health care information from the Internet and the growing number of books, audio recordings, and video recordings published on health maintenance, alternative medicine, herbs, vitamins, minerals, and over-the-counter medications and preparations

f. Spiraling health care costs; use of health maintenance organizations, preferred provider organizations (PPO), or internal plan provider lists; lack of health insurance; increased reliance on self-diagnosis, treatment, and care; and increased availability of diagnostic kits for home-based self-diagnostic testing

g. Problems related to cultural conflicts, stress, pain, and cultural imposition practices

h. Increased suspiciousness and mistrust or distrust of cultural, religious, and political groups because of increased terrorist activities worldwide

Theories & Conceptual Models

Theories, conceptual models, and practice research are the cornerstones of the nursing career. Nursing theories guide to research and improve nursing practice, which influences the healthiness and value of patients’ lives. Effective nursing practice needs to apply skills, caring, knowledge, and art to deliver care to the patients in a considerate, effective, and efficient manner. There are numerous theories and conceptual models used in advanced nursing practice. Nursing theories offer a foundational knowledge of care concepts that enable individuals in the advanced nursing practice to articulate evidence that validates their practice methods (Smith & Parker, 2015).  Nursing conceptual models provide a way for nurses to examine what they do and offer a patient assessment framework. In other words, nursing conceptual models entail related concepts and ideologies applied by nurse researchers in studies: they offer a mutual meaning to basic nursing concepts.

Theories and Conceptual Models for use in The Advanced Nursing Practice

Grand nursing theories, middle-range nursing theories, and practice-level nursing theories play a significant role in advanced nursing practice. Usually, grand nursing theories are complicated, abstract, and broad in scope. As such, they need detailed research for clarification. It is vital to mention that grand nursing theories arise from the nurse theorist’s experience and offer a general framework for nursing ideas on components such as environment, health, and people. Examples of the grand nursing models comprise Neuman’s systems model and Levine’s conservation model.

Unlike the grand nursing theories, the middle-range theories narrow down into specific nursing areas. It is also vital to mention that Middle-range theories emerge from the theories of similar disciplines, research, and nursing practice (Brandão et al. 2017). Examples of mid-range theories include Mishel’s model of uncertainty in illness, Kolcaba’s comfort model, and Roy’s adaptation approach (Guan et al., 2020). Practice-level nursing theories are also significant theories in advanced nursing practice. The theories narrow their focus to specific concepts about a defined patient populace and a specified duration.

The three previously discussed nursing theories help define what advanced practice nurses do and why they do it.  The theories are vital in advanced nursing practice because they help shape the delivery of patient care. Without the theories, the nursing practice would lack a linear focus. Again, the three previously discussed approaches are used in the advanced nursing practice to shape facilities’ healthcare delivery strategies in a manner that aligns with a range of expected outcomes. While the grand theories establish a broader framework, the middle-range and practice level approaches enable the governance of certain types of nurses or healthcare situations (Smith & Parker, 2015). It is essential to note the three nursing theories can only help provide high-quality healthcare if the advanced practice nurses are knowledgeable and skilled enough to work within their parameters efficiently.

It is vital to note that nursing models’ central concepts are health, person, environment, and nursing (de Brito et al., 2017).  In this case, a person is the receiver of the nursing action while the environment is the recipients surrounding. Health is the patient’s illness or wellness state, while nursing refers to the nurses’ actions on behalf of the patient. Nurses use nursing conceptual models to deliver comprehensive, collaborative, and holistic care. Regardless of the nurses’ specialization, nursing theories and conceptual models create guidelines for both specific and general nursing practices by helping the nurses understand their role and purpose in a healthcare setting.

Week 4

This week discussion is about a 65-year-old male with chronic obstructive pulmonary disease. He is seen in a medical facility related to symptoms of fever and a cough lasting for 2 weeks. The patient denies chest pain and night sweats. With his age, I would ask the patient does he have any other chronic conditions. If so, what medications is he currently prescribed. With COPD, it is the third leading cause of death in the United States. Tobacco smoke is a key factor in the development and progression of COPD; with exposure to air pollutants in the home and workplace, genetic factors, and respiratory infections also play a major role (CDC, 2019).

The objective findings I would assess is the patient’s lungs sounds which would display wheezing, a productive cough and shortness of breath with movement. Some may experience fatigue, edema to bilateral lower extremities, chest tightening and loss of appetite due to inability to breath. Increased respiratory rate will be observed with a decreased oxygen saturation level.

The diagnostic test used to diagnose COPD is a Lung (pulmonary) function tests. These tests measure the amount of air you can inhale and exhale, and whether your lungs deliver enough oxygen to your blood. During the most common test, called spirometry, you blow into a large tube connected to a small machine to measure how much air your lungs can hold and how fast you can blow the air out of your lungs. Other tests include measurement of lung volumes and diffusing capacity, six-minute walk test, and pulse oximetry. A chest X-ray can show emphysema, one of the main causes of COPD.

An X-ray can also rule out other lung problems or heart failure. A CT scan of your lungs can help detect emphysema and help determine if you might benefit from surgery for COPD. CT scans can also be used to screen for lung cancer. Arterial blood gas analysis measures how well your lungs are bringing oxygen into your blood and removing carbon dioxide (CDC, 2019). There are many chronic conditions that can affect the airway that has similar symptoms such as Upper Respiratory infections.

URI is a common viral infection that affects the nose, throat, and airways. The symptoms may include fever, fatigue, cough, headache and difficulty breathing. Adult onset-Asthma is a condition in which your airways narrow and swell and may produce extra mucus. This can make breathing difficult and trigger coughing and Shortness of breath. This condition will not occur until you are an adult. Symptoms include Coughing, especially at night, during exercise or when laughing, Difficulty breathing, Chest tightness, Shortness of breath and Wheezing.

Moreover, another chronic condition that can have similar symptoms is pneumonia. Pneumonia is an infection that inflames the air sacs in one or both lungs.  The symptoms an individual may experience with pneumonia are fever, a dry cough, headache, muscle pain, and weakness. The symptoms typically get worse within one or 2 days, with increasing cough, shortness of breath and muscle pain (Jarvis, 2016). A chest X-ray will be used to diagnose this disorder and based on your lab test, antibiotic therapy will be initiated.

Module 4 Discussion

Nicole Bachich

St. Thomas University

This patient is a 75-year-old woman who presents with complaints of chest pain. Chest pain is a common presenting symptom with various causes (Ayerbe, González, Gallo, Coleman, Wragg, & Robertson, 2016). This patient’s chest pain began when she was walking up the steps and at that time, she rated it an 8 out of 10. In order to collect more subjective data, I would ask this patient to describe her pain in more detail. First, I would ask her to describe the pain. This patient described her pain as burning or aching. I would want to know if the burning, aching pain was constant or if it would come and go. The patient denied that the pain radiated to her arm, but I would ask if it radiated to the upper chest. I would ask her if she was diaphoretic when the pain occurred. I would also ask the patient if laying down caused more pain. Other questions to ask this patient would be if she took anything to try and relieve the pain and I would ask her about any drug or alcohol use in her present or past. Cocaine use can cause coronary spasms (Rhoads & Wiggins Petersen, 2021). I would also ask the patient if she has a history of anxiety.

When conducting the physical assessment on this patient I would gather more objective data. I would assess the patient’s skin color, mucous membranes, and lips to assess for cyanosis or anemia. I would assess her skin temperature as well and check to see if she was diaphoretic. I would also assess the patient’s eyes and check for Xanthelasmas and any skin or nail changes, which may be present in a patient with angina (Zitkus, 2010). Changes in skin texture and hair distribution may also be present in peripheral vascular disease.

The diagnostic tests that I would order would be an electrocardiogram (ECG), a troponin I level, an X-Ray of the digestive system and chest, and an endoscopy. The ECG and troponin lab would be done to rule out any cardiac abnormalities or if the patient may have had a myocardial infarction, and this patient does not appear to be in acute cardiac distress. The other tests would be done to assess for a possible gastrointestinal cause for her chest pain. Non-cardiac chest pain can cause angina like pain in patients without heart disease.

Three differential diagnoses that I would give for this patient are Gastroesophageal reflux (GERD), esophageal motility disorders, and gastric ulcer. I believe that this chest pain is related to a gastrointestinal cause because gastrointestinal pain can be described as a burning sensation in the middle of the chest (Rhoads & Wiggins Petersen, 2021). However, thee diagnoses can only truly be made after ruling out any cardiac related chest pain, as cardiac related chest pain caused by a myocardial infarction may have similar symptoms (Frieling, 2018). GERD is a common cause of non-cardiac related chest pain, and it is attributed to up to 60% of non-cardiac chest pain diagnoses (Frieling, 2018).
