The Comprehensive Final Project (Assignment #4) has been designed for the student to identify federal and state policy roots in a specific health services organization.  Each student will select a specific management area for a specific organization.  The student will identify a health services setting where they have access to the policy manual (or a sample policy manual).  The setting may include a single location or an organization with many types of services and many service delivery settings. Select one of the following spheres where you will map out and describe a minimum of five (5) organizational policies linked to federal or state legislative mandates. Note some policies may be required for accreditation and also related to public policy.  This is the case with many NCQA standards.

Prepare a final PowerPoint presentation with no more than 10-15 content slides. A cover and reference slide must be provided.  Draw on what you have learned throughout the class including:

-a summary of each (5 in total) organizational policy and the federal/state legislative roots for each policy (by name and statute),

-any pending changes to the legislation (to each)

-and your metrics for the organization in terms of performance (for each)

You are caring for a 6 year old female patient name Rita in the hospital.  She is 2 hours post-op from acute appendicitis.  She had a laparoscopic appendectomy. She is sleepy but arousable and can answer short questions. 

Her last set of vital signs are:

b/p 100/68, p 90, r 26, t 98.1, sats 96%.  

Her vitals 30 minutes before that were:

b/p 120/79, p 74 r 20, t 98.3, sats 99%.  

You check her abdomen and there are 4 well approximated incisions on her abdomen, closed with surgical glue.  One incision on her lower left quadrant appears to be swollen and appears to have some red drainage seeping from under the surgical glue.  Rita is moaning and grimacing with her eyes closed in the bed.  

She has a 20g IV in her left forearm and is running LR at 100ml/hr, she has 02 at 2l/m via NC in place. She has an indwelling Foley catheter and you have drained 40ml of urine in the 2 hours she has been in your care.

Rita’s mom and dad are anxiously sitting in her room, asking questions regarding her recovery from surgery and when she will be discharged.

As a nurse, you serve an important role in identifying strategies to effectively manage health care resources and in leading health care quality improvement. You must be able to decide what leadership style or strategy to apply in a given situation to achieve an effective resolution of the issue. Read the following two scenarios and select one to focus on in this Discussion. Consider the leadership style or strategy that might be most effective in the scenario you selected.

Scenario 1

You work in a for-profit nursing home, with about 100 beds, on a 20-bed unit that is largely patients with Alzheimer’s disease. Your patient mix is predominantly Medicare and Medicaid patients. Your nursing home is part of a larger system that includes a major medical center, as well as VNA, outpatient dialysis, and a fully integrated network. Your nurse manager is getting feedback from the hospital that your nursing home is sending too many patients to the ED who really don’t need to go. How would you go about figuring out what could be done at the nursing home to prevent avoidable ED visits?

Scenario 2

You’ve been associated with an outpatient cardiology clinic that is part of a large academic medical center. Your patients are mostly charity care and managed Medicaid. Most have a prescription plan, but none have a “family doctor” and use the clinic (and the ED) regularly. Most are unfamiliar with their medications and do not have the resources for care coordination in their family/social network. About 25 CHF patients have been “lovingly,” but inappropriately, called “frequent fliers” because of their inability to manage their own care, their frequent visits to the ED, and their “one night stays” paid at the observation rate. As a staff nurse in this clinic, describe the strategies you could devise for you and your fellow staff nurses targeting these 25 patients. Find at least one article from the professional literature to corroborate your recommendations.

Nature of the concept

The concept of the categorical imperative is a syllogism

1.The first premise is that a person acts morally if his or her conduct would, without condition, be the “right” conduct for any person in similar circumstances (the “First Maxim”).

2.The second premise is that conduct is “right” if it treats others as ends in themselves and not as means to an end (the “Second Maxim”).

3.The conclusion is that a person acts morally when he or she acts as if his or her conduct was establishing a universal law governing others in similar circumstances (the “Third Maxim”).

Kant’s Three Premises Explained

In the case of ” Gone Baby Gone” the philosophical process you will follow in this paper should sound as something like this:

According to Kant’s first premise Patrick behaved moral because he returned the victim he was hired to find and without condition this is the “right” conduct for any detective in cases of kidnapping (the “First Maxim”). Here Kant speaks of doing the right prior to good.

2. On Kant’s second premise Patrick acted moral because he respected the dignity of Amanda and he treated her as an end in herself and not as means to an end like everyone else was doing in the film (the “Second Maxim”). Here Kant speaks of humans as special creatures and ends in themselves. Humans are ends, not means to an end. Los seres humanos son un fin, no un medio para un fin.

3. The conclusion is that a detective act morally when he reports a kidnapping to the police and this behavior should establish a universal law governing all detectives in cases of kidnapping (the “Third Maxim”). Here Kant asks us to test the universality of our action. Will it be OK if everyone does what I am about to do?
