In the modern era, there are few professions that do not to some extent rely on data. Stockbrokers rely on market data to advise clients on financial matters. Meteorologists rely on weather data to forecast weather conditions, while realtors rely on data to advise on the purchase and sale of property. In these and other cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s body of knowledge.

Of course, the nursing profession also relies heavily on data. The field of nursing informatics aims to make sure nurses have access to the appropriate date to solve healthcare problems, make decisions in the interest of patients, and add to knowledge.

In this Discussion, you will consider a scenario that would benefit from access to data and how such access could facilitate both problem-solving and knowledge formation.

Personhood

The concept of personhood, as defined by Schoenhofer, is a central concept of the theory of Nursing as Caring Theory.  Nursing as Caring Theory is grand nursing theory developed by Theorists Boykin and Schoenhofer that evolved from theories of Paterson and Zderad (Duffy, Donnell, & Snowden, 2014).  The ideologies of these theorists are cultivated around a care model that evaluates and defines nursing as a discipline and a profession as it relates to the patient and the nurse.

The field of nursing has progressed from a bedside profession to an advance art of discipline. The evolution of nursing from a profession to an advance discipline was made possible due to advancement, analysis, and applications of principles of medicine and philosophy. Philosophically personhood is defined as the status of being a person.  Nursing embodies this concept and adapts it to each individual meaningfully and appropriately.  Theorists Boykin and Schoenhofer developed the grand nursing theory of Nursing as Caring around the central ideas of “the between”, “the call for nursing”, ”the nursing response”, and “personhood” (Duffy, Donnell, & Snowden, 2014). These four central ideas are interconnected and offer a conceptual framework in caring for patients and enhance and afford the ability for all those involved in the nursing process the ability to be “a person”; an integral role for the nurse and patient because without this authenticity the nursing process will not be able to reach its fullest potential (Schoenherr, 2002).

The call of nursing is the initial step in the methodology of the Nursing is Caring Theory.  It is during this stage the nurse accepts the ‘call’ to nurture.  The call of the nurse is then proceeded by the ‘nurse’s response’. This nurse response is a testament to all her knowledge and experiences and the ability to translate and relate to the task at hand.  Once the nurse’s knowledge and experiences are uniquely appropriated for the environment, trust is gained from the patient and the acknowledge they are seen as a person and able to exhibit their personhood to the nurse.  Nurses must do this on a daily basis and multiple times within in the day due to the various dynamics they are faced with, different patient demographics, their caregivers, and other staff members.  When a nurse enters the environment of the patient it is where they are most vulnerable and allowed to exihibit personhood and adapt their abilities and develop their abilities to care for the patient.  It is in this raw instance of vulnerability and personhood that a nurse is able to be receptive and learn the needs of the patient and afford them the best care.

The most impactful and meaningful takeaway from Nursing as Caring Theory is that, above all else, nurses instinctively and inertly will act upon the intent to help, heal, and nurture.  Due to the ubiquitous nature of the discipline and profession of nursing, a nurse can easily adapt their nursing response to be suitable for the environment and call of nursing and appropriate to be purposeful for each individual.  This flexibility and adaptability of a nurse is a skillset that facilitates the holistic art of nursing that is conducive to learning, teaching, healing, and promoting wellness in individuals.

Watson’s philosophy and science of caring has four major concepts: human being, health, environment/society, and nursing Butts & Rich, 2015). In Watson’s view, the disease might be cured, but illness would remain because, without caring, health is not attained. Caring is the essence of nursing and connotes responsiveness between the nurse and the person; the nurse co-participates with the person. Watson contends that caring can assist the person to gain control, become knowledgeable, and promote health changes.

According to Watson (2009), the core of the Theory of Caring is that “humans cannot be treated as objects and that humans cannot be separated from self, other, nature, and the larger workforce.” Her theory encompasses the whole world of nursing; with the emphasis placed on the interpersonal process between the caregiver and care recipient. The theory is focused on “the centrality of human caring and on the caring-to-caring transpersonal relationship and its healing potential for both the one who is caring and the one who is being cared for” (Watson, 2009). The structure for the science of caring is built upon ten carative factors. Among them are human altruistic values, faith-hope, sensitivity to one’s self or other, trust, human caring relationship, and promotion of self-expression (.

Watson defines Human being as a valued person to be cared for, respected, nurtured, understood, and assisted, in general a philosophical view of a person as a fully functional integrated self. Personhood is viewed as greater than and different from the sum of his or her parts which are mind-body-soul-connection (Butt & Rich 2015)   

The personhood concept in Watson theory of caring implies that patients are not all the same. Each person brings a unique background of experiences, values, and cultural perspective to health care encounter. Caring facilitates a nurse’s ability to know a patient, allowing the nurse to recognize a patient’s problem and find and implement individualized solution on the patient’s unique needs.

Knowing the person allows the nurse to avoid assumptions, to center on the one cared for (Keller, 2013).  It also gives the nurse to opportunity to assess thoroughly by seeking clues to clarify the issue that the individual is going through.

The concept of personhood also integrates the human caring processes with healing environment, incorporating the life-generating and life receiving processes of human caring and healing for nurses and their patient. The concept put emphasis on developing a caring relationship with the person as a nurse and listen to the person’ stories to fully understand the meaning an impact of the individual’s condition. This information and understanding helps in the development and delivery of individualized patient centered care. The transpersonal caring theory rejects disease orientation to health care and places care before cure. When the nurse focuses on the person, she or he looks for deeper sources of inner healing to protect, enhance and preserve a person’s dignity, humanity, wholeness, and inner harmony.

In conclusion, theory of caring provides an excelling beginning to understand the behavior and processes that characterized caring. Patients continue to value nurses, effectiveness in performing tasks, but clearly patient value the affective dimension of nursing care. Nurses must provide presence, touch, attention to their patient. Most importantly they must know the person they are caring for because healthcare is placing emphasis on patient satisfaction.

This week discussion involves a 68 -year-old Caucasian male who complained of burning when urinating.  He admits to the pain increasing with the more he urinates and it has been going on for the past 5 days.  His past medical history includes prostatic hyperplasia, high cholesterol and hypertension. The medications he is prescribes are Olmesartan 20mg and Crestor 20 mg. He has been diagnosed with nocturia and dysuria. The subjective data I would inquire is his diet. How much water does he consumes daily? Was there an odor present when urinating?  Due to your history of UTI’s, are you taking any probiotics as needed and were you prescribed with antibiotics, if so, how long? With his systolic Blood pressure in the 150’s, when is the last time you took the Olmesartan. Did the fever occur before or after the Burning started, and did you take any acetaminophen or Motrin to break it?

             The Labs that I would order is a urine culture with sensitive, CBC, BMP and lipid panel. A kidney, ureter, and bladder (KUB) X-ray may be performed to assess the abdominal area for causes of abdominal pain, or to assess the organs and structures of the urinary and/or gastrointestinal (GI) system. A KUB X-ray may be the first diagnostic procedure used to assess the urinary system.

           A few diagnoses that can relate to the symptoms will include Bladder infection. They are the most common type of urinary tract infection caused by bacteria and can lead to problems like pain in your lower belly and having to pee way more often than usual (Rhoads, 2021). Symptoms may include Pain in the bladder, groin, lower abdomen, or pelvis that can occur during urination or during sexual intercourse.  The body may experience fatigue, fever, or malaise. Chronic Kidney Disease is a condition in which the kidneys are damaged and cannot filter blood as well as they should. Because of this, excess fluid and waste from blood remain in the body and may cause other health problems, such as heart disease and stroke (CDC,2019) CKD can cause Anemia, increased infections, abnormal electrolytes, loss of appetite or depression.

         A Kidney infection (pyelonephritis) is a type of urinary tract infection (UTI) that generally begins in your urethra or bladder and travels to one or both of your kidney. Symptoms observed in a kidney infection are Fever, Chills, Back, side (flank) or groin pain, Abdominal pain, Frequent urination, Strong, persistent urge to urinate and Burning sensation or pain when urinating.

                    Furthermore, patient teaching would include increasing intake of water and/or cranberry juice. Fluids can help remove bacteria from your body when you urinate. Kidney scarring can lead to chronic kidney disease, high blood pressure and kidney failure. When the infection remains in the blood, septicemia can occur.  Your kidneys filter waste from your blood and return your filtered blood to the rest of your body.  Urinate as soon as you need to. Avoid delaying urination when you feel the urge to urinate. It is important to empty the bladder after intercourse. Urinating as soon as possible after intercourse helps clear bacteria from the urethra, reducing your risk of infection (CDC, 2019).

 Data that has already been provided is that a right temporal headache that gets worse with light.  Patient explains she experiences frequent headaches with menses.  Does the patient have a family history?  Can she identify any triggers? Additional questions that should be asked are related to the pain she is experiencing.  On a pain scale of 0-10, how would she rate the pain? How long does the pain last without rest? Does the pain radiate? Describe the pain; is it throbbing, pulsating or stabbing?  Patient takes ibuprofen for the headaches.  Does the medication help reduce the pain? If it does, what is the pain rate?

What other objective findings would you look for?

  Patient is wearing dark glasses in a dark room due to photophobia.  Migraine headaches cause intense, throbbing pain, often on one side of the head. Nausea, vomiting and sensitivity to light, sound and exertion often accompany migraines, which can last several hours or up to three days. Some migraine sufferers experience a visual disturbance called an aura prior to the onset of the migraine. Auras are flashing lights, wavy lines, blurry vision or blind spots (Radiological Society of North America (RSNA) and American College of Radiology (ACR), 2020).

What diagnostic exams do you want to order?

 Blood chemistry and urinalysis can rule out any other medical conditions such as dehydration, diabetes or thyroid disease. An MRI can be used to examine the anatomy of the brain and to assist in the diagnosis of tumors, developmental abnormalities, blood vessel problems and disorders of the eyes and the inner ear.  A CT scan can detect similar abnormalities.  A lumbar puncture also known as a spinal tap, this helps physicians diagnose infections such as meningitis, encephalitis or inflammatory conditions such as multiple sclerosis.

 Primary headaches are the most common type of headaches, they are painful but not dangerous.  Secondary headaches are characterized by excruciating pain which are usually caused by another more serious condition.  Migraine is the most common form of headache, but not all headaches are migraines. The term “migraine” refers to a headache which is usually (but not always) on one side of the head. It is a headache that lasts from two to seventy-two hours, typically, and it is often associated with nausea and/or vomiting, sensitivity to light and/or sound. The character of the pain is typically a throbbing pain (Stanford Health Care, 2017).  Tension-type headache is most likely the second most common headache type after migraine. They are often referred to as “hatband” headaches because they typically painful around the back of the head, the temples and forehead, as if a tight hat were in place.  A cluster headache is a headache which is short lasting usually between 20 minutes and two hours. It is always one-sided and is associated with symptoms such as a stuffy nose on one side, tearing, an enlarged pupil, or a droopy lid.
