The patient was presented to our hospital with severe nausea, vomiting and abdominal pain. Assessment and lab work showed the patient’s blood glucose was 605mg/dL, HgA1C 8.5%, cholesterol 180 mg/dl, weight 290 lbs, height 68inches, BP 90/75, pulse 118, Respiration 23 bpm. Patient stated she had been very stressed lately and had mostly been dining out at fast-food restaurants and has not had the time to exercise. Patient had been prescribed insulin however, she stated she has not been checking her blood glucose level like she should have. This patient was presenting with symptoms of diabetic ketoacidosis (DKA). DKA is a medical emergency that occurs when the body is unable to produce adequate insulin to decrease blood glucose level and leads to the production of high levels of acids in the blood called ketones (Alshammari et. al., 2017). This was a medical emergency that required immediate intervention of fluid repletion and the administration of insulin. It also involved the frequent monitoring of patient’s vital signs, glucose levels, and electrolyte levels. Factors that might have influenced pharmacokinetics of insulin the patient was receiving are diet (eating fatty foods or foods high in carbohydrates increases glucose levels in the body), stress (cortisol increases insulin resistance), and lack of exercise. Eating a balanced diet decreases the likelihood that the patient will consume mostly foods high in fats and carbohydrates. When someone is stressed, the body produces stress hormones called cortisol which to a diabetic patient can significantly affect their health because it causes the body to release more glucose and to become resistant to the insulin the patient needs to reduce that blood glucose to a healthy level (Alshammari et. al., 2017). Exercise redistributes and lowers the blood glucose level. It could cause patients to go into hypoglycemia that is why it is important to check glucose levels before, during, and after exercise.The personalized care plan that I would develop for this patient would include; a Dietary and lifestyle change plan – here the patient will be educated on how to count her carbohydrates and caloric intake. The Center for Disease Control has dietary guidelines for the different age groups. The site also provides “My Food Diary” (CDC, 2019) a flow sheet to help individuals keep a day-to-day record of how much food is consumed for breakfast, snacks, lunch, and dinner. I would also recommend she exercise regularly but that to check her blood glucose before, during and after exercise. “The adoption and maintenance of physical activity are critical foci for blood glucose management and overall health in individuals with diabetes” (Colberg, 2016). Also, I would suggest she check her blood glucose before each insulin administration, as this may affect the dosage to be administered.

The Science and Art of Theory Development and Evaluation

Scholars in various disciplines have conducted research and developed various theories to explain different phenomena’ existence or relationship. Hypotheses are created to fulfill a need and are always formed from observations, empirical studies, and literature review to produce a theory that explains a phenomenon, relationship of phenomena, or predicts the occurrence of phenomena. It is essential to evaluate a postulation before its application to test its correctness. Some theories, such as those in health sciences, are not applicable before testing and evaluation. It is only possible to determine a theory’s effectiveness by testing after the predicted phenomenon occurs. However, evaluation is possible using scientific and artistic approaches.

The various criteria for evaluating theories are scope, logical consistency, parsimony, utility, testability, heurism, and test of time. Scope, utility, and logical consistency are arts of theory development and evaluation. A theory must be within the scope, which means somebody cannot develop a postulation beyond their purview. For instance, an expert in the medical field cannot create a political leadership theory because it is not within their purview. Logical consistency means that a hypothesis should be correct at all times. A postulation must not contain contradicting statements at different times. The utility is the art of testing the theory’s usefulness and is evaluated based on its contribution to the field on which it is based (Fawcett, 2005). A postulation must bring a new aspect into the discipline. A hypothesis that repeats existing theories does not pass the test of utility.

Parsimony, testability, heurism, and test of time are scientific approaches to theory development and evaluation. Parsimony means that a theory should have a few concepts to be effective. A hypothesis with many arguments and ideas does not pass the parsimony test (Redmond, 2015). The science behind this is that the higher the number of concepts and arguments, the more difficult it is to produce a practical theory. Heurism refers to the ability of a hypothesis to create new thinking and stimulate new thoughts. A reasonable postulation must contribute to the discipline by bringing a unique aspect or thought to the field. Other researchers and theorists should be able to apply the theory or use the hypothesis as a basis for coming up with other postulations (Avant, 2009).

A good theory must pass testability and test of time. Testability means that its basis should be reliable evidence, and it should be possible for people to verify the evidence behind the theory. A theory can is not a statement without any backing of empirical research, literature review, or any other form of verifiable proof (Avant, 2009). The test of time means that a theory must be predictive and powerful. Predictability relates to the ability of the postulation to give a good reason to be adopted. This concept requires a good and convincing explanation of the thinking behind the hypothesis. A hypothesis should be influential because it should have a considerable impact on people’s lives or the relevant discipline. A postulation must provide a milestone in the field (Fawcett, 2005). If a theory does not have a significant influence, it does not pass the power test. Lastly, a postulation must withstand the test of time. Similar studies on similar objects or subjects at any time should be able to give similar conclusions and the same theory.

Theory is a framework or model for understanding and observation, which determines what we perceive and how we perceive it. Hypothesis lets a researcher form links between concrete and abstract statements. Postulation asserts a link between different phenomena and explains the relationship between various variables. Hypothesis organizes different ideas and guides research. Facts may not be useful if they lack meaning unless combined into a theoretical framework. The strength of theory increases as supporting evidence becomes available. Postulations can form a basis for prediction and can help produce new research. When evaluating a theory, it is crucial to consider various aspects such as coherency and logic of the hypothesis, testability of theory-based claims and predictions, the fitness of postulation to available data, and its potential impact on the world

Obesity is a complex disorder that is characterized by an excess amount of body fats with victims having an abnormally high body mass index. Researchers have proved obesity to be a risk factor for the development of other diseases. Evidence translation is a process that entails production of evidence through investigations and integration of the evidence in practice to help reach goals and produce a change of behaviors and lifestyles for better health.

Common barriers.

Translation of evidence in correcting obesity and causing change is not always smooth, but faced with challenges. Lack of resources and support, for instance, challenges the translation of any evidence on to the actual process since change of diet is a signi9ficant factor to consider individuals may not have enough money to get the recommended diet and end up not being able to correct the disorder. Insufficient knowledge and failure to clarify the ideal strategies to correct obesity may cause difficulty in the implementation of the set policies. Additionally, target individuals may not be willing to partake in the policies. For instance, people may not be willing to do exercise and that would pose a barrier to effective evidence translation (White, et al. 2016)

Inadequate evidence among the evidence translators can be a challenge when a translator faces questions from the targets and may result to unwillingness of individuals to change (White et al. 2016). Poor communication skills when teaching the audience about any evidence may result to conflicts that hinder the process of change.

Adopting strategies to be aware of new evidenceet al.

To get new ideas, I can read recent researches that have been conducted. I could also choose to follow the health organization on social media to get every single update that is posted. Moreover, I can choose to conduct research involving both health care providers and other residents to get their views and gain new knowledge. To effect any change, I need to present enough evidence why obese is bad and therefore I would gather information enough to convince the target individuals to change. Interview can be of significant help too. Interviewing healthcare practitioners as well as researches can provide me with a wide range of information that I can use as my evidence or a basis for further research.

Determination of evidence to implement.

Determining what evidence to implement is critical if anyone wants to start any change. The first thing is to determine the cause of the obesity. Some may be inherited, some drugs, others developed due to lifestyles and others because of other illnesses. Different cases may require different approaches or even combine approaches. Also, I will investigate the cost of the change and the financial status of the target group (Dang, et al. 2018). Age also matters when choosing the type of evidence to choose. For instance, obesity caused by sedentary lifestyles like drinking alcohol, eating fast foods and foods with much fat and lack of exercise can be solved by encouraging individual to take exercises. Exercises can involve walking to work instead of driving, or reducing alcohol and fast food consumption. Obesity caused by other diseases can be solved by addressing the underlying disease and adding healthy diets and exercise.

Lack of knowledge for the risks associated with obesity and possible ways of getting away with it can be addressed by educating the target group, while the one caused by certain drugs can be solved by using an alternative drug with less effect.

Ensuring continuation and sustainability of change.

To ensure continuity and sustainability of the change, I will set goals with each person and remind them how important their change is in relation to their health. I will also access them on regular bases and see whether they are doing the expected. I will also organize programs to educate them to keep them on their feet (Dang, et al. 2018). I will also check for any available motivation that can keep them on track. Asking individuals to keep a record that I should check on a regular basis can also be of help.
