Essential VI: Health Policy and Advocacy

Rationale

The healthcare environment is ever-evolving and influenced by technological, economic,

political, and sociocultural factors locally and globally. Graduates of master’s degree

nursing programs have requisite knowledge and skills to promote health, help shape the

health delivery system, and advance values like social justice through policy processes

and advocacy. Nursing’s call to political activism and policy advocacy emerges from

many different viewpoints. As more evidence links the broad psychosocial, economic,

and cultural factors to health status, nurses are compelled to incorporate these factors into

their approach to care. Most often, policy processes and system-level strategies yield the

strongest influence on these broad determinants of health. Being accountable for

improving the quality of healthcare delivery, nurses must understand the legal and

political determinants of the system and have the requisite skills to partner for an

improved system. Nurses’ involvement in policy debates brings our professional values

to bear on the process (Warner, 2003). Master’s-prepared nurses will use their political

efficacy and competence to improve the health outcomes of populations and improve the

quality of the healthcare delivery system.

Policy shapes healthcare systems, influences social determinants of health, and therefore

determines accessibility, accountability, and affordability of health care. Health policy

creates conditions that promote or impede equity in access to care and health outcomes.

Implementing strategies that address health disparities serves as a prelude to influencing

policy formation. In order to influence policy, the master’s-prepared nurse needs to work

within and affect change in systems. To effectively collaborate with stakeholders, the

master’s-prepared nurse must understand the fiscal context in which they are practicing

and make the linkages among policy, financing, and access to quality health care. The

graduate must understand the principles of healthcare economics, finance, payment

methods, and the relationships between policy and health economics.

Advocacy for patients, the profession, and health-promoting policies is operationalized in

divergent ways. Attributes of advocacy include safeguarding autonomy, promoting social

justice, using ethical principles, and empowering self and others (Grace, 2001; Hanks,

2007; Xiaoyan & Jezewski, 2006). Giving voice and persuasion to needs and preferred

direction at the individual, institution, state, or federal policy level is integral for the

master’s-prepared nurse.

The master’s-degree program prepares the graduate to:

1. Analyze how policies influence the structure and financing of health care,

practice, and health outcomes.

2. Participate in the development and implementation of institutional, local, and state

and federal policy.

3. Examine the effect of legal and regulatory processes on nursing practice,

healthcare delivery, and outcomes.

4. Interpret research, bringing the nursing perspective, for policy makers and

stakeholders.

5. Advocate for policies that improve the health of the public and the profession of

nursing.

Patient is a 26 year old female of Korean decent that was recently admitted for a 21 day hospitalization due to onset of acute mania. She was diagnosed with bipolar 1 disorder. Genetic testing was performed, patient is positive for CYP2D6*10 allele. Hospital had described Lithium, but patient stopped two weeks ago since being discharged. For decision point 1, I chose to restart the patients lithium at a dose of 300 mg PO BID. Patient reports non compliance stating she “takes it when she feels like she needs it”. For decision point 2, I chose to educate the patient and continue treatment of lithium at original ordered dose. Patient then states she is having nausea and diarrhea. Decision point 3, I decided to change patients lithium to sustained release and if symptoms worsen can then change to depakote. As mentioned pharmacotherapeutics is the study of beneficial and adverse effects of drugs. Given these are common side effects of lithium if the patient is able to take as directed with tolerable symptoms than the benefits of lithium far outweigh the risk and is an appropriate plan of treatment for the patient.Though the mechanism of action of lithium in preventing recurrences of bipolar disorder remains only partially understood the common theme emerging from pharmacological and genetic studies is that lithium affects multiple steps in cellular signaling, usually enhancing basal and inhibiting stimulated activities (Alda, 2015). It is apparent by the patients manic behavior, conversation, and inappropriate attire that she would benefit from this medication. Additionally, side effect concerns assuredly play some role in lithium nonadherence (Gitlin, 2016). Education would need to be thorough and reinforced for this patient.

Attention-deficet/hyperactivity disorder is a neuropsychiatric disorder affecting 5-11% of children with boys being two to three times more likely to diagnosed (Rosenthal DNP ACNP, Laura & Burchum DNSc APRN BC, Jacqueline, 2020). A diagnosed with ADHD can be made if the symptoms began before the age of seven and have to have been exhibited for at least six months (Rosenthal DNP ACNP, Laura & Burchum DNSc APRN BC, Jacqueline, 2020). There is thought to be a genetic component to ADHD to which I fully agree as my son has ADHD and while I do not have a formal diagnoses of it I have most definitely diagnosed my self and my father. Children typically respond to the medication based on the mechanism of action meaning that it will either cause a release or blockage of norepinephrine, dopamine, and serotonin. 

In the scenario we were told that it is an eight-year-old girl whose parents were told by her teacher that she felt that she might have ADHD and now the parents have sought out professional help in determining if she does and if so what the corse of treatment would be. We are then asked to choose what medication we feel she should be started on (wellbutrin, intuniv, or ritalin) and I feel she should be started on Ritalin 10mg in the am. During her next follow up it is noted that the teacher feels that her attention has improved for the first part of the day however by the afternoon she is back to being unable to concentrate. It was also noted during the appointment that her pulse was elevated. At this point I felt that it was necessary to change her medication to Ritalin LA and to monitor her heart rate as it is a side effect of taking a stimulant. At her next follow up it was noted that she has been able to remain attentive during the entire school day and that her heart rate has returned to a normal rate. From this point I feel it would be acceptable to continue with the current regimen. 

Ritalin or methylphenidate works by increasing the release of norepinephrine and dopamine while also preventing the reuptake of the same neurotransmitters. A study was conducted regarding how to properly dose Ritalin and what they noted was that it should be started at 5mg not the 10mg that the scenario suggested and should be given twice a day with increased increments of 5-10mg based on the effectiveness of the medication(Childress et al., 2016). This same study also concluded that children tend to do better and are more compliant with the extended release Ritalin because they do not require a second dose while at school (Childress et al., 2016). It should also be noted that the goal of any medication should be to be on the lowest possible dose while achieving the desired affect. There are times when adding a second medication to the treatment can enhance the affect of Ritalin on in particular is memantine (namenda) a medication that is typically used to treat Alzheimer. What one study found is that memantine is able to improve the symptoms associated with ADHD without adding any adverse side effects (Riahi et al., 2020).

ADHD is a condition that can be treated in a multitude of ways with multiple differed medication options and combinations that can be tailored to fit the needs of each individual patients. It is important to make sure that the child is not only on the correct medication but also that they are taking the lowest possible dose. Educating the family regarding dietary restrictions and when the medication should be given is also extremely important.
