Rationale

Informatics and healthcare technologies encompass five broad areas:

• Use of patient care and other technologies to deliver and enhance care;

• Communication technologies to integrate and coordinate care;

• Data management to analyze and improve outcomes of care;

• Health information management for evidence-based care and health education;

and

• Facilitation and use of electronic health records to improve patient care.

Knowledge and skills in each of these four broad areas is essential for all master’sprepared

nurses. The extent and focus of each will vary depending upon the nurse’s role,

setting, and practice focus.

Knowledge and skills in information and healthcare technology are critical to the delivery

of quality patient care in a variety of settings (IOM, 2003a). The use of technologies to

deliver, enhance, and document care is changing rapidly. In addition, information

technology systems, including decision-support systems, are essential to gathering

evidence to impact practice. Improvement in cost effectiveness and safety depend on

evidence-based practice, outcomes research, interprofessional care coordination, and

electronic health records, all of which involve information management and technology

(McNeil et al., 2006). As nursing and healthcare practices evolve to better meet patient

needs, the application of these technologies will change as well.

As the use of technology expands, the master’s-prepared nurse must have the knowledge

and skills to use current technologies to deliver and coordinate care across multiple

settings, analyze point of care outcomes, and communicate with individuals and groups,

including the media, policymakers, other healthcare professionals, and the public.

Integral to these skills is an attitude of openness to innovation and continual learning, as

information systems and care technologies are constantly changing, including their use at

the point of care.

Graduates of master’s-level nursing programs will have competence to determine the

appropriate use of technologies and integrate current and emerging technologies into

one’s practice and the practice of others to enhance care outcomes. In addition, the

master’s-prepared nurse will be able to educate other health professionals, staff, patients,

and caregivers using current technologies and about the principles related to the safe and

effective use of care and information technologies.

Graduates ethically manage data, information, knowledge, and technology to

communicate effectively with healthcare team, patients, and caregivers to integrate safe

and effective care within and across settings. Master’s-prepared nurses use research and

clinical evidence to inform practice decisions.

Master’s-degree graduates are prepared to gather, document, and analyze outcome data

that serve as a foundation for decision making and the implementation of interventions or

strategies to improve care outcomes. The master’s-prepared nurse uses statistical and

epidemiological principles to synthesize these data, information, and knowledge to

evaluate and achieve optimal health outcomes.

The usefulness of electronic health records and other health information management

systems to evaluate care outcomes is improved by standardized terminologies. Integration

of standardized terminologies in information systems supports day-to-day nursing

practice and also the capacity to enhance interprofessional communication and generate

standardized data to continuously evaluate and improve practice (American Nurses

Association, 2008). Master’s-prepared nurses use information and communication

technologies to provide guidance and oversight for the development and implementation

of health education programs, evidence-based policies, and point-of-care practices by

members of the interdisciplinary care team.

Health information is growing exponentially. Health literacy is a powerful tool in health

promotion, disease prevention, management of chronic illnesses, and quality of life–all of

which are hallmarks of excellence in nursing practice. Master’s-prepared nurses serve as

information managers, patient advocates, and educators by assisting others(including

patients, students, caregivers and healthcare professionals) in accessing, understanding,

evaluating, and applying health-related information. The master’s-prepared nurse designs

and implements education programs for cohorts of patients or other healthcare providers

using information and communication technologies.

The master’s-degree program prepares the graduate to:

1. Analyze current and emerging technologies to support safe practice environments,

and to optimize patient safety, cost-effectiveness, and health outcomes.

2. Evaluate outcome data using current communication technologies, information

systems, and statistical principles to develop strategies to reduce risks and improve

health outcomes.

3. Promote policies that incorporate ethical principles and standards for the use of health

and information technologies.

4. Provide oversight and guidance in the integration of technologies to document patient

care and improve patient outcomes.

5. Use information and communication technologies, resources, and principles of

learning to teach patients and others.

6. Use current and emerging technologies in the care environment to support lifelong

learning for self and others.

Opioid and alcohol addiction impacts the nursing profession, quality of nursing care, and healthcare organizations. Nursing care is compromised as the complexity of a patient’s health increases with the use of alcohol and opioids. Addiction also commonly causes withdrawal symptoms, making it difficult to focus on non-pharmacological treatment options (Hagemeir, 2018). First responders become overwhelmed with opioid overdose related calls and it is common for cardiac or injury patient calls to be delayed.

       The American Association of Colleges of Nursing has taken the initiative to reach local levels through academics. Schools are being provided with webinars that discuss the CDC’s recommendations on opioid prescribing for chronic pain management, early interventions, treatments, and appropriate discontinuation of these medications (AACN, 2021).

       As mentioned by CCN (2021), relevant stakeholders related to opioid and alcohol addiction at a local level includes the patient, families, pharmacies, and health care professionals. Due to the increased need for healthcare resources through outpatient or ER visits and inpatient hospital admissions, less available appointments and longer ER wait times are the result (Hagemeier, 2018).

        The Florida Department of Health (2021), identifies an intervention that the pain management facility that I am currently employed at has adopted to address addiction concerns. In 2009, Legislation in Florida released the E-FORCSE initiative (Electronic-Florida Online Reporting of Controlled Substance Evaluation Program), to promote safer prescribing and to reduce addiction/abuse of prescribed opioid medications within the state. This prescription reporting platform has made a huge difference in patient outcomes and helps identify patients that attempt to “doctor shop” or receive multiple narcotic prescriptions in between monthly follow up visits and refills. The medication, amount, dosage, prescribing physician, pharmacy, and date of all opioid medications is recorded on a patient’s E-FORCSE and is updated the day of their monthly follow up to ensure accuracy. The E-FORCSE reporting system is used within all our clinics scattered throughout Florida and three Alabama locations.  The MME’s or Morphine Milligram Equivalent score related to a patients daily dose of opioid medication is also provided to ensure that the optimal range of 30 or less is maintained.  The MME per day metric is a tool used to identify those with higher risks of overdosing and to ensure that closer monitoring is implemented (Dowell, Haegerich, & Chou, 2016). From my personal perspective I feel that this intervention is effective in addressing the addiction practice problem; however, there are still some improvements that need to be made with E-FORCSE reporting. All prescribing pharmacies do not use the E-FORCSE reporting platform to record all filled medications, which could lead to overprescribing and ultimately death.

Leadership in nursing is a key part in providing high quality healthcare to patients and in creating a conducive environment where staff are empowered. A nurse leader basically shows the other nurses the way and acts a bridge between them and the administrative leaders of the hospital. One of the responsibilities of a nurse leader is advocating for great patient care and needs of their unit nurses (Al-Dossary, 2017)by publicly supporting them. Another role is setting clear goals and accomplishing them successfully as well as building rapport with their followers. Application of creative thinking and problem solving skills should be part of day-to-day of a nurse leader. Other responsibilities include supervision of healthcare delivery, staffing and delegation of tasks to the nursing staff.

Education greatly contributes to nursing leadership skills; therefore, nursing education is crucial. A master’s education encompasses a course in leadership and care delivery which increases the effectiveness of leadership nursing as opposed to a newly registered nurse (Al-Dossary, 2017). For one to be an influential and successful nurse leader, strong interpersonal skills such as empathy and openness are very essential. This helps understanding the feelings of the staff and responding appropriately to them. Emotional intelligence is also significant since it leads to positive relationships between the nurse leaders and the staff (Hughes, 2017).This refers to the ability of one managing their emotions and those of others. These traits and more such as flexibility and integrity enhance good patient care, teamwork and promote a healthy working environment for nurses.

One of the most significant factors in empowering and encouraging nurses, who make up the vast majority of the healthcare workforce, to perform at the highest level of their licensure is nursing leadership. American Nurses Association (n.d) describes nursing leadership as “a nurse interested in excelling in a career path, a leader within a healthcare organization who represents the interests of the nursing profession, a seasoned nurse or healthcare administrator interested in refining skills to differentiate them from the competition or to advance to the next level of leadership.”

In my experience, one of the formal roles as a nurse leader is charge nurse for the respective department or unit. Although it’s normal to concentrate leadership efforts at the top of a hospital or health system, middle-management leaders (such as nurse practitioners) are critical to an institution’s progress. They have a direct influence on many front-line caregivers and healthcare staff. One of the formal roles of nurse leaders is the charge nurse for the respective department or unit. In our organization, to hold a position of charge nurse, one must have a bachelor’s degree and undergo charge nurse training classes held every month in terms of educational preparation. According to Felicia and Natalie (2020), the important responsibilities of charge nurse are;

  • Prioritizing patient safety protocols
  • Have vision and strategy to function the unit
  • Empower the entire healthcare team
  • Excel in problem-solving skills
  • Encourage interdependence and teamwork
  • Focus on quality patient care
  • Handle unit budget
  • Liaison between management and healthcare employees
  • Advocate for nurses as well as patients
  • Tackle staffing shortage issues

To be successful in this role, one must have qualities like good communication skills, competence, and confidence, willingness to learn new things, patience, good listening skills, and work together with the team. Encourage feedback and suggestions related to quality patient outcome, capacity to adapt in any situation, acknowledge work of team members, share credits of quality patient outcome, set ground rules, and maintain therapeutic relationships with patient and her team members. Charge nurses are suggestive of holding mentioned qualities to achieve planned patient outcomes and maintain a good relationship with interdisciplinary units and management. It is also needed to understand her staff and meet their needs to avoid conflicts and ensure patient safety. Ultimately nurses are continually confronted by new problems, practices, and resources as the healthcare landscape evolves. It will also make or break a nurse’s decision to stay or leave an institution whether they have a good nurse leader to help them handle these shifts.
